Neurological Manifestations In Type- 1 DiabetesMellitus In Children

Abstract

Background: Diabetes mellitus type-1 is the most common endocrine metabolic disorder in childhood. Mononeuropathy, generalized polyneuro-pathy and autonomic neuropathy are frequent complications of diabetes mellitus and may give rise to troublesome manifestations. Methods: sixty children suffering from type 1 diabetes mellitus admitted in Children Welfare Hospital- Medical City-Baghdad in the period from 1st Dec2000-31stJuly 2001 were included in this prospective descriptive study. History, especially symptoms of peripheral neuropathy, examination specially signs of peripheral neuropathy, absent sinus arrhythmia and postural hypotension and investigations like nerve conduction study were all performed and analyzed.
Results:
The study showed that out of 60 diabetic children 26(43.3%) had symptomatic polyneuropathy, 18 (30%) had postural hypotension and 16 (26.6%) had absent sinus arrhythmia. Neurological manifestations of diabetes mellitus occur frequently in diabetic children with long duration of illness. Polyneuropathy is sensory more than motor and affected the lower limbs more than the upper limbs. Conclusion: Early diagnosis of autonomic neuropathy in diabetic children is simple easy and requires minimum cooperation, so as detection of peripheral neuropathy in asymptomatic by nerve conduction study.

Abstract

Abstract: Insulin-dependent diabetes mellitus (IDDM) is characterized by completedestruction of β-cells, this result in little or no production of insulin & /or Cpeptidewhich is a portion of insulin & is released in amounts equal to the insulin.C-peptide level can indicate how much insulin is being produced by the pancreas.Patients & Materials: Fifty six diabetic patients were enrolled in this study (15IDDM & 41 NIDDM). C-peptide levels in serum & saliva were estimated usingRadio-immuno assay (RIA) method, with measuring serum FBS in both groups.Results: The serum & salivary levels of C-peptide as well as serum FBS levels weremarkedly higher in NIDDM than in IDDM with a significant negative correlationbetween salivary C-peptide & FBS in IDDM group.Conclusion: Salivary C-peptide level estimation can be used as an adjunct to serumFBS to measure the amount of insulin produced by β-cells, as well as to measurethe response of insulin therapy in IDDM.

Abstract

ABSTRACT:BACK ROUND:The long duration of insulin dependant diabetes mellitus eventually ends with complications like renal impairment especially if it was badly controlled ,the first sign of renal involvement is the elevation of serum uric acid above or near upper normal values(1).OBJECTIVE:One hundred and thirty type1 diabetic patients were enrolled in the study, they represent a selected sample of patients who attend the Specialized Center for Endocrinology and Diabetes (Baghdad Russafa Directorate) during the period from November 2006 to April 2007 compared with forty healthy individuals as control group of similar age group.METHOD :The determination of fasting blood sugar ,uric acid and blood urea were done by enzymatic colorimetric test following the Procedure performed by manufacturers.RESULTS:The level of serum uric acid and blood urea were normal in all cases of juvenile diabetes but as the duration of the disease increases the values of the serum uric acid started to raise and could be the first sign of renal impairment in diabetic patients even before albuminuria..CONCLUSION:The concentration of uric acid in the blood is an independent marker of failing kidneys and may even play a causative role in the decline of renal function.

Abstract

The aim of this study is to determine the activity of serum Gamma Glutamyl Transferase (GGT) elevation in first group of 38 patient of type II In Dependant Diabetes Mellitus (IDDM) whose treated with insulin injection second group of 15 diabetic patients were tested for three different activities GGT, Alkaline Phosphatase (ALP) and Amylase, results were compared with control group of 15 normal healthy population ,the results show that there were significant increase in the levels of GGT,ALP and Amylase in diabetic patients in comparison with normal population group.

في مرضى السكري Amylase ,ALP ,GGT تحديد مستوى انزيمات يهدف هذا البحث إلى حيث تم اخذ مجموعتين من المرضى شملت المجموعة الاولى على 38 مريض وتم قياس مستوى ومقارنته مع مجموعة السيطرة والتي احتوت 14 شخص. اما المجموعة الثانية فشملت GGT في عدد من المرضى 15 مريض ومقارنتة مع مجموعةAmylase, ALP , GGT دراسة السيطرة 15 شخص. اظهرت النتائج ارتفاع معنوي في مستوى الانزيمات الثلاثة مما يدل على وجود علاقة بين مرض السكري وهذه الانزيمات.

Abstract

This study showed that among older adults presented with newly diagnosed diabetes there is certain percentage actually have type 1 diabetes as detected by positive anti-GAD65 antibody test and they shouldn’t be misdiagnosed as having type 2 diabetes.

استهدفت الدراسة معرفة مدى انتشار الجسم المضاد بين مرضى السكري البالغين الكبار لتحديد Anti-GAD65 antibody وتمييزالمصابين بالنوع الاول لداء السكري لأهمية ذلك في اختيار العلاج المناسب.
