8.INTERLEUKIN-4 SINGLE NUCLEOTIDE POLYMORPHISM C-590T POLYMORPHISMS IN RELATION TO ASTHMA

Abstract

Background:Single nucleotide polymorphisms in the promoter regions of genes encoding for some interleukins may associate with occurrence of asthma.Objective:To investigate the association of single nucleotide polymorphisms of interleukin-4 (IL-4) (C-590T) and asthma.Methods:Forty-five patients with asthma and 40 apparently healthy subjects (represent the control group) were enrolled in this study. Blood samples were collected from both patients and controls. DNA was extracted from blood samples and gene fragments corresponding to IL-4 C-590T were amplified with specific primers using conventional PCR technique. Results:The heterozygote genotypes of IL-4 C-590T (CT) showed significant association with asthma (OR = 3.922, 95% CI= 1.153-13.339, P = 0.028). Conclusion:These results suggest the significance of IL-4 C-590T polymorphism as a risk factor for asthma.Keywords:Asthma, interleukin-4, polymorphismCitation:Gaidan, Abbas AA, Hassan MA, Hashim HM. Anti-inflammatory Role of IL-4 in Patients with Rheumatic Mitral Valve Stenosis

Abstract

Background: IL-4 is an antibodies inflammatory cytokine which has an important role in protecting against the inflammatory reactions in most of diseases. Here, we try to highlights the role of this cytokine in chronic Rheumatic heart disease and its correlation with the extent of histopathological abnormalities.Patients and Methods: Rheumatic mitral valve surgical fragments were taken from a total of 48 Iraqi patients with chronic rheumatic heart disease under mitral valve replacement surgery in Ibn Al-Bitar Hospital for Cardiac Surgery-Iraq-Baghdad. Paraffin embedded mitral valve tissue sections were prepared. IL-4-expressing cells were detected by using immunohistochemical staining technique and histopathological picture was studied by using hematoxylin and eosin staining.
Results: There were lower numbers of IL-4 positive cells in all patients under study in general, but very lower IL-4 positive cells percentage was recorded in high risk group patients. There was no significant difference in the distribution of IL-4 mean positive cell’s count among all patients (p > 0.05). There was a correlation of IL-4 with the extent of histopathological abnormalities, and odds ratio results displayed that the histopathological abnormalities which recorded in the case group 4 times than that occurred in the control group.Conclusion: IL-4 plays an important role in regulating the inflammatory responses against the heart in chronic rheumatic heart disease

Abstract

Summary:Background: There is a general acceptance which illustrated that auto antibodies act as a central immunological disturbance in most of the auto immune diseases, among these auto immune disease lies the SLEPatients and Methods: Thirty five patients with SLE were compared to twenty age and sex matched, control subjects and studied for the presence of auto antibodies, plus IL-4 and IL-6 using Elisa method and immune fluorescent method (for ANA only)Results: Data showed that IL-6 detectable levels were statistically significant in patients with positive anti ds-DNA, but not significant statistically in ANA positive patients although it was detected in 24 (70.6%) of positive ANA patients, while there was no statistically significant correlation between IL-4 detectable level and autoantibodies production.Conclusion: There is apparently a positive correlation between IL-6 and anti ds DNA production in lupus patients.Key words: Autoantibodies, Interleukin-4, Interleukin-6, systemic lupus erythematosus

Abstract

Thirty adult male of New Zealand Albino white rabbits were used in this article and these rabbits are haphazardly divided into five different experimental groups. Ovalbumin was used in a dose (0.1 mg /kg) to induce allergy and sensitization challenge was confirmed by the presence of the clinical symptoms of wheezing, sneezing, chest tightness, shortness of breath, changes in the level of total white blood cell, neutrophils, eosinophils, Immunoglobulin E and Interleukin-4. Eight days after allergy induction, Ginkgo biloba giving in the dose (25 mg/kg) and in a combination with prednisolone in the half of recommended dose (12.5 + 0.5) mg/kg (Ginkgo biloba + prednisolone) was giving orally for 8 days. The results showed that ovalbumin could induce sensitivity and significantly increased the level of total white blood cell, neutrophils, eosinophils, Immunoglobulin E and Interleukin-4. Oral treatment with Ginkgo biloba in the dose (25 mg/kg) and in a combination with prednisolone as the half of recommended dose (12.5 + 0.5) mg /kg of Ginkgo biloba + prednisolone showed high significant decrease (p≤ 0.01) in the number of total white blood cell, neutrophils and eosinophils and serum levels of Immunoglobulin E and Interleukin-4levels. The effects of this plant extract were comparable with the observed effect of prednisolone-treated group to be present with high efficacy of herbal medical therapy than that of prednisolone treatment a combined with adverse effects when use alone.

Abstract

Background: Osteoporosis is a frequent disease that is manifested by reduced in mineral density and raised in fracture risk. Recent studies have indicated that osteoporosis is caused by composite connections among local and systemic regulators of bone cell function.Objective: The purpose of this study was to investigate the relationship between interleukin-2, interleukin-4, and some biochemical markers in Iraqi patients with osteoporosis.Patients and Methods: Forty five osteoporotic patients were incorporated in this study (30 women and 15 men). Serum fasting glucose, lipid profile, alkaline phosphatase activity, calcium, magnesium, interleukin-2, and interleukin-4 were measured in osteoporotic patients and compared them with the control group.Results: There was a significant increase in serum fasting glucose, lipid profile except high density lipoprotein cholesterol, and serum alkaline phosphatase activity in osteoporotic patients as compared to the control, (P=0.001). Also, there was a significant increase in interleukin-2 and interleukin-4 in osteoporotic patients as compared to the control, (P= 0.01). While there was a decrease in serum calcium and magnesium levels in osteoporotic patients as compared to the control. Also, there was a significant positive correlation between serum alkaline phosphatase activity with interleukin-2 and interleukin-4 levels in osteoporotic patients, (P= 0.01).Conclusion: Elevated levels of interleukin-2 and interleukin-4 along with some biochemical markers like; serum alkaline phosphatase in osteoporotic patients might include significant functions in the pathogenesis of osteoporosis.Keywords: Osteoporosis, Interleukin-2, Interleukin-4.

خلفية البحث: هشاشة العظام هو مرض شائع يتمثل بانخفاض في كثافة المعادن وارتفاع خطر الكسر. وقد أشارت الدراسات الحديثة إلى أن هشاشة العظام ناجمة عن العلاقة بين عوامل نظامية وموضعية منظمة لوظيفة الخلايا العظمية.الهدف من البحث: كان الهدف من هذه الدراسة هو تشخيص العلاقة بين الانترلوكين-2، الانترلوكين-4، وبعض الدالات الكيموحيوية لدى المرضى العراقيين الذين يعانون من هشاشة العظام.المرضى وطرق العمل: تم اختيار 45 مريضا ً من الذين يعانون من هشاشة العظام في هذه الدراسة (30 امرأة و 15 رجلا). تم قياس الكلوكوز في الدم، صورة الدهون، فعالية أنزيم الفوسفاتيز القاعدي، الكالسيوم، المغنيسيوم، الانترلوكين-2، والانترلوكين-4 لدى المرضى المصابين بهشاشة العظام ومقارنتهم مع مجموعة السيطرة.النتائج: كانت هناك زيادة معنوية في كلوكوز الدم، صورة الدهون باستثناء البروتين الدهني عالي الكثافة، فعالية أنزيم الفوسفاتيز القاعدي في الدم لدى المرضى المصابين بهشاشة العظام بالمقارنة مع مجموعة السيطرة، ((P= 0.001. وكذلك توجد زيادة معنوية في مستويات الانترلوكين-2 والانترلوكين-4 لدى مرضى هشاشة العظام بالمقارنة مع مجموعة السيطرة، (.(P= 0.01 في حين كان هنالك انخفاض في مستوى الكالسيوم والمغنيسيوم في الدم لدى المرضى المصابين بهشاشة العظام بالمقارنة مع مجموعة السيطرة، ولكنه غير معنوي.وكذلك توجد علاقة معنوية بين فعالية انزيم الفوسفاتيز القاعدي ومستويات الانترلوكين-2 والانترلوكين-4 لدى المرضى المصابين بهشاشة العظام، (.(P= 0.01 الاستنتاجات: المستويات المرتفعة من الانترلوكين-2 والانترلوكين-4 بالإضافة الى بعض الدالات الكيموحيوية مثل؛ أنزيم الفوسفاتيز القاعدي في الدم لدى المرضى المصابين بهشاشة العظام قد تتضمن وظائف أساسية تكون السبب في هشاشة العظام.كلمات مفتاحية: هشاشة العظام، الانترلوكين-2 ، الانترلوكين-4.

Gene Expression and Polymorphism of Interleukin-4 in a Sample of Iraqi Rheumatoid Arthritis Patients

Abstract

It was aimed to understand the interleukin-4 (IL-4) role in etio-pathogenesis of rheumatoid arthritis (RA). Two approaches were adopted. In the first one, a quantitative expression of IL4 gene was assessed by reverse transcription-quantitative polymerase chain reaction (RT-qPCR), and such findings were correlated with some demographic, clinical and laboratory parameters, which included gender, duration of disease, disease activity score (DAS-28), rheumatoid factors (RFs), C-reactive protein (CRP) and anti-cyclic citrullinated peptide (ACCP) antibodies. In the second approach, a single nucleotide polymorphism (SNP) of IL4 gene (rs2243250) was inspected by DNA sequencing using specific primers. Fifty-one Iraqi RA patients (22 males and 29 females) were enrolled in the study. They were under therapy, which was a single weekly subcutaneous dose of 25 mg of etanercept (Enbrel) for a period of 3-5 years. The results of gene expression (2-ΔΔCt) revealed an increased expression of IL4 mRNA (Mean ± SEM: 8.247 ± 2.442), especially female patients compared to male patients (11.545 ± 3.928 vs. 3.537 ± 1.530; p = 0.03). The expression was also subjected to variations that were related to clinical and laboratory findings. With respect to IL4 gene SNP, allele and genotype frequencies showed no significant differences between RA patients and controls. In addition, the SNP genotypes had no effect on IL4 gene expression. In conclusion, an up-regulation of IL4 gene expression was observed in RA patients, and it was more pronounced in female than male patients by approximately four folds, while no association between the IL4 SNP alleles or genotypes and RA was observed.

كان الهدف بيان دور البين ابيضاض-4 في امراضية التهاب المفاصل الرثوي. اعتمدت طريقتين؛ وتم في الأولى تحديد التعبير الكمي لجين البين ابيضاض-4 بطريقة الاستنساخ المعاكس الكمي-تفاعل انزيم البلمرة المتسلسل, ودرست العلاقة بين هذه النتائج وبعض المعايير الديموغرافية والسريرية والمخبرية والتي تضمنت الجنس ومدة المرض وعامل نشاط المرض والعامل الرثوي والبروتين التفاعلي-سي ومضاد ببتيد السترولينايد الدوري. اما في الطريقة الثانية، فقد تم التحري عن تعدد اشكال النيوكليوتيدة المفرد (SNP) لجين البين ابيضاض-4 (rs2243250) وذلك باستخدام تقنية تسلسل الحامض النووي وباستخدام بوادئ متخصصة. شملت الدراسة 51 مريض من مرضى التهاب المفاصل الرثوي العراقيين (22 ذكور و29 اناث)، والذين كانوا تحت العلاج بواقع حقنة أسبوعية تحت الجلد (25 ملغم) من الدواءEnbrel لفترة 3-5 سنوات. اظهرت نتائج التعبير الجيني 2-ΔΔCt)) زيادة ملحوظة في تعبيرة (المعدل ± الخطأ القياسي: 8.247 ± 2.442) خصوصا في المرضى الاناث مقارنة بالذكور (11.545 ± 3.928 مقابل 3.537 ± 1.530؛ الاحتمالية = 0.03). وخضع هذا التعبير لتغايرات لها علاقة بالمعايير السريرية والمخبرية. اما بخصوص تعدد اشكال النيوكليوتيدة المفردة لجين البين ابيضاض-4، فلم تظهر تكرارات الانماط الاليلية والجينية اختلافات ذات دلالة إحصائية بين مرض داء المفاصل الرثوي والسيطرة. وفضلا عن ذلك، فلم يكن هناك تأثير للانماط الجينية على التعبير الجيني لجين البين ابيضاض-4. وفي الاستنتاج فقد لوحظ زيادة في التنظيم لتعبير جين البين ابيضاض-4 في مرض التهاب المفاصل الرثوي، وكان ذلك اكثر وضوحا في المرضى الاناث مما هو عليه في المرضى الذكور بحوالي اربع مرات، في حين لم تلاحظ مصاحبة بين الانماط الاليلية والجينية لتعدد اشكال النيوكليوتيدة المفردة لجين البين ابيضاض-4 والتهاب المفاصل الرثوي.
