Assessment of Disease Activity in Rheumatoid Arthritis: A Comparative Study of Clinical Evaluation with Ultrasonography

Abstract

ABSTRACT: BACKGROUND: In patients with rheumatoid arthritis (RA) a poor relation on an individual joint basis, has been observed between clinical signs of synovitis and ultrasound measures of synovial disease. OBJECTIVE:To compare the traditional clinical measures of disease activity with the ultrasound (US) features of synovitis, and investigate the relationship between composite US measures and disease activity score in 28 joints (DAS28), clinical disease activity index (CDAI), their components and other variables of disease activity in RA.METHODS:Fifty patients with RA were enrolled in this study. The following 28 joints including bilateral glenohumeral, elbow, wrist, metacarpophalangeal (MCP), proximal interphalangeal (PIP) of the hands, and knee joints were assessed for tenderness and swelling. DAS28 and CDAI were determined for each patient. A systematic US examination was carried out by a radiologist for the 28 clinically examined joints. Each joint was evaluated for the presence of synovial hypertrophy (SH), power Doppler (PD) signals, and effusion. The following composite US measures of synovial disease were made: SH joint count (SHJC), effusion joint count (EJC), PD joint count (PDJC), SH index (SHI), and PD index (PDI).RESULTS: Joints with tenderness only showed significantly less PD scores than other groups. SH and PD signals were detected in 32.1% and 27.8% of the Nil group respectively. SHJC and SHI showed moderate correlation with TJC and high correlation with SJC, evaluator global assessment (EGA), patient global assessment (PGA), DAS28, CDAI, and erythrocyte sedimentation rate (ESR). PDJC and PDI showed moderate correlations with tender joint count (TJC), and high correlation with swollen joint count (SJC), EGA, DAS28, and ESR. PDI showed high correlation with PGA, and CDAI. CONCLUSION:Traditional clinical signs used in the evaluation for disease activity may bear different relation to the US features of synovitis (SH, PD signals). Composite US count and indices for SH and PD relate significantly to the DAS28-ESR, CDAI, and their components..

Abstract

Abstract:Rheumatoid arthritis (RA) is a chronic inflammatory polyarthritic disease associatedwith remissions and exacerbations and characteristic genetic, clinical, pathological, andimmunological features. The present study was designed to examine the evidence linkingProteus mirabilis to RA in some Iraqi patients. The study was carried out on 70 Iraqi RApatients, during the period from March 2010 to March 2011. For purposes of comparison, 10of Systemic Lupus Erythematousus (SLE) patients and 10 of apparently healthy subjects wereinvolved as a control groups. After bacterial isolation and identification, Enzyme linkedImmuno sorbent Assay (ELISA) technique has been applied for estimation of antibacterialantibodies.Results of this study revealed that out of 70 urine samples of RA patients, E.coli waspresent in 12.9%, followed by P. mirabilis which present in 7.1%. Regarding SLE andhealthy control group, urine samples were negative from any bacterial strains except E. coli,which present in about 10% of each group. Frequency of Proteus mirabilis in RA group washighly significant when compared with that of other groups (p<0.01). All rheumatoid arthritispatients and control groups were tested by ELISA technique for detection of IgM, IgG andIgA antibodies specific to Proteus mirabilis. Results demonstrated that the levels of theseantibodies were elevated in the sera of RA patients with a high significant degree, incomparison with control groups (p<0.01). The same sera were tested for anti- E. coliantibodies and the results showed that there was no significant difference in the IgM and IgGantibodies level to E. coli in RA patients when compared with control groups (p>0.05).In conclusion, these findings suggested a possible association between infection withProteus mirabilis and rheumatoid arthritis. The elevated levels of Proteus mirabilis antibodieswithin RA sera could be helpful in the identification of those patients during early stages ofthe disease

الخلاصة:التھاب المفاصل الرثوي ھو مرض مزمن یصیب المفاصل ویمر باطوار متناوبة من الشفاء والانتكاس حیث انProteus لھ صفات جینیة، سریریة، مرضیة، ومناعیة ممیزة. صممت الدراسة الحالیة لفحص دلائل ارتباط بكتریا البمرض بالتھاب المفاصل الرثوي. mirabilisأجریت الدراسة على 70 مریضا عراقیا مصابا بمرض التھاب المفاصل الرثوي للفترة من اذار 2010 – اذار2011 . ولغراض المقارنة فقد شملت الدراسة ایضا على 10 مرضى مصابین بداء الذِئْبَةٌ الحُمَامِیَّةٌ المَجْموعِیَّة و 10اشخاص اصحاء ظاھریا. بعد عزل وتشخیص البكتریا طبق اختبار الانزیم المرتبط الممتزللتحري عن الاجسام المضادةE. coli للبكتریا المعزولة. من مجموع 70 عینة ادرار عائدة لمرضى التھاب المفاصل الرثوي عزلت بكتریا البنسبة % 7.1 ، في حین كانت النسب المناظرة في مجامیع السیطرة معدومة Proteus mirabilis بنسبة % 12.9 والالتي ظھرت في % 10 من عینات الادرار لكلا المجموعتین. كانت نسبة E. coli لكل انواع البكتریا باستثناء بكتریا الفي مرضى التھاب المفاصل الرثوي مرتفعة معنویا اذا ما قورنت نسبتھا مع Proteus mirabilis عزل بكتریا ال. (p< المجامیع الاخرى ( 0.01ارتفاعا معنویاعالیا في (IgM, IgG, IgA) Proteus mirabilis أظھرت نتائج التحري عن اضداد بكتریا الProteus mirabilis فضلاً عن إن ایجابیة اضداد ال ،(p< مستوى جمیع الاضداد مقارنة مع مجامیع السیطرة ( 0.01في ضوء .(p< كانت مرتفعة معنویا في مصول مرضى التھاب المفاصل الرثوي مقارنة مع مجامیع الدراسة الاخرى ( 0.01ومرض التھاب المفاصل الرثوي Proteus mirabilis النتائج اعلاه یمكننا الاستنتاج بان ھنالك علاقة بین بكتریا الفي مصول الاشخاص المصابین بھذا المرض یمكن ان یساعد في Proteus mirabilis حیث ان ارتفاع اضداد التشخیص ھؤلاء المرضى خلال المراحل المبكرة منھ.

The Role of Anti-TNFα Therapy in the Amelioration of Disease Burden in Patients with Refractory Rheumatoid Arthritis

Abstract

Abstract:To evaluate the changes in anti cyclic citrullinated peptide (Anti-CCP) antibodies,rheumatoid factor (RF), interleukins 17 and 10 (IL-17 and IL-10), high sensitivity C- reactiveprotein (hs CRP) and T regulatory cells (Treg) count following an anti TNF-α biological agent(etanercept) therapy in patients with rheumatoid arthritis.Refractory rheumatoid arthritis patients who failed treatment with DMARDs (diseasemodifying anti rheumatic drugs) were treated with etanercept for three months. Serum and bloodsamples were tested before and after therapy for six markers including ACCP, RF, IL- 10, IL 17,hs CRP, and Treg cell. A significant dropping down in the serum level of ACCP, RF and hs CRPwas documented 3 months after etanercept therapy. On the other hand a non- significant elevationin IL-10 serum level and peripheral blood Treg cell count, as well as a non significant droppingdown in the serum level of IL-17 were also reported. Etanercept is a successful anti-TNF-αtherapy in refractory RA patients for the first three months of the course therapy as indicated byclinical improvement and amelioration in 6 disease activity markers.Key words: rheumatoid arthritis, IL-17, etanercept, Treg, TNF-α

الخلاصة:تقییم التغیرات في اضداد الببتید الحلقي، العامل الرثوي، انترلوكین 10 و 17 ، البروتین التفاعلي عالي التحسس نوعس والخلایا اللمفاویة التنظیمیة بعد العلاج باستخدام العلاج بمضادات العامل المنخر السرطان نوع الفا (ایتانارسبت) فيمرضى التھاب المفاصل الرثواني.تم علاج مجموعة من مرضى التھاب المفاصل الرثواني المستعصین على العلاج التقلیدي وذلك باستخدام ایتانارسبتلمدة ثلاثة اشھر. تم تقییم عینات دم ومصل الدم لھم قبل وبعد اعطاء العلاج لستة مؤشرات مرضیة شملت اضداد الببتید الحلقي،العامل الرثوي، انترلوكین 10 و 17 ، سي ریكتف بروتین العالي التحسس، والخلایا اللمفاویة التنظیمیة. بعد ثلاثة اشھر منالعلاج بالاینارسبت تم توثیق انخفاض معنوي كبیر لاضداد الببتید الحلقي, العامل الرثوي ,و سي ریكتف بروتین العاليالتحسس وارتفاع غیر معنوي لمستوى انترلوكین 10 والخلایا اللمفاویة التنظیمیة وانخفاض غیر معنوي لمستوى انترلوكین17 . عقارالایتانرسبت كمضاد للعامل المنخر للسرطان كان ناجحا لدى استخدامھ لمدة 3 اشھر في مرضى التھاب المفاصلالرثواني المستعصي بدلالة التحسن السریري والتحسن في مستویات ستة من العلامات النشاطیة للمرض.

Abstract

ABSTRACT:BACKGROUND:Abnormal lymphocyte activation status have been described in rheumatoid arthritis. Suggesting an increased need for iron as a source of energy.OBJECTIVE: To evaluate the cellularexpression pattern ofTransferrin receptor(CD71)proteinin the peripheral blood lymphocytes,PATIENTS &METHODS: This study involved forty-six rheumatoid arthritis patients 42 female and 4 male, age ranged from 25-66 years collected from AL-Kadhemia teaching hospital were examined and compared with 17 healthy control individuals of similar ages. Lymphocytes were separated from peripheral blood samples,the assessment of their cellular expression of cluster of differentiation 3 and CD71by immunofluorescent staining method. RESULTS: The results showed abundant accumulation of CD3 T lymphocytes in the peripheral circulation of rheumatoid arthritispatients in comparison with controls with increased transferrin receptor expression in rheumatoid arthritisgroup (p<0.001).Furthermore,the diffuse pattern was not significant (0.280), lower percentage of intercellular pattern among RA patients (21.31±16.23) than controls (45.43±12.95) with highly statistical significant difference (p<0.001). While, membranous pattern was higher among RA patients (51.22±28.43) than controls (23.77±18.57) with highly statistical significant difference (<0.001).CONCLUSION:This study demonstrate increased expression in CD71 with membranous expression pattern amongrheumatoid arthritis patientsthan thatcontrols, suggesting its important role in up taking transferrin from blood as a source for energy..

Abstract

ABSTRACT:BACKGROUND: Rheumatoid arthritis (RA) is a chronic, deforming arthritis that can lead to disabilities and poor quality of life. Cytokines produced by inflammatory cells play a pivotal role in synovial inflammation and joint destruction in rheumatoid arthritis; they are the final mediators and/or regulators of inflammatory process.OBJECTIVE: This study was designed to assess serum IL-15 and IL-17 levels in a sample of patients with RA and to evaluate the correlation between their levels and various clinical, laboratory parameters of RA disease activity and severity.SUBJECTS AND METHODS: A case – control study was carried out from January 2013 till October 2013. It was conducted on 80 RA patients who attended Basra General Hospital outpatient rheumatology clinic. Eighty apparently healthy adults, age and sex matched, were also included as control group. Peripheral blood samples were collected from both patients and controls and used for estimation of serum IL-15, IL-17 and anti-CCP antibodies by ELISA, RF and CRP by slide agglutination test and ESR by Westergren method. RA activity was measured using disease activity score28 (DAS-28).RESULTS: RA patients showed significantly higher serum IL-15 and IL-17 levels than controls (P<0.01). In addition, IL-15 and IL-17 were found to be significantly higher in RA patients with active disease compared to those at remission. No significant correlations were noted between IL-15 serum level and RF, CRP, ESR, anti-CCP but it was significantly correlated with DAS-28, TJC and SJC. Significant positive correlations of serum IL-17 level and ESR, CRP, anti-CCP, DAS28, TJC and SJC were found. Also there was a significant direct correlation between IL-15 and IL-17concentration.CONCLUSION: The current study demonstrated that serum IL-15 and IL-17 levels were significantly higher in RA patients than the controls confirming their important role in the pathogenesis of RA and possible target for future therapy .Ele¬vated IL-17 levels could be considered as a possible indicator of more severe clinical course, This is underscored by relatively strong correlations between almost validated disease activity and severity markers including CRP, ESR, anti-CCP, DAS28, TJC and SJC.

Abstract

ABSTRACT:BACKGROUND: Interleukin 6 (IL-6), a pro-inflammatory cytokine, plays a key role in the development of rheumatoid arthritis (RA). Transforming growth factor-beta1 (TGF-β1) is an anti-inflammatory cytokine which is considered to be an important (down) regulator of inflammation in RA.OBJECTIVE:To evaluate serum concentration of IL6 to TGF-β1 ratio in a sample of Iraqi patients with RA. METHODS: A case control single center study was conducted for 11 months. A total of 50 patients with RA diagnosed according to the 1987 American College of Rheumatology (ACR) and 30 individuals matched in age and sex as control group were included. Serum concentration of IL-6 to TGF-β1 ratio and serum IL-17 were determined in both RA groups using enzyme-linked immunosorbent assay (ELISA). The cut off value was assessed by receiver operating characteristics (ROC) test and correlation by spearman's Rholinear correlation coefficient.RESULTS: Frequency of females was more than males in patients and controls (88% and 76.7% respectively). Ages of patients range between 20-70 years with a median 43.76 years while in controls, ages range between 20-60 years with a median 37.67 years. Serum concentration of IL-6 toTGF-β1 ratio was significantly more in patients than those in controls (p=0.018). Area under the curve (AUC) at value 0.686 was statistically significant (p=0.018) and had intermediate accuracy. Serum IL-6 to TGF- β1 ratio≥ 2.4pg/ml was the optimum cutoff value that can differentiate between RA and healthy controls with accuracy 71.9%. There was a significant positive linear correlation between IL-6 to TGF-β1 ratio and IL-17( r=0.56, p<0.001) and simple linear regression analysis showed that for each 1unit increase in IL6 to TGF-B1 ration there was a significant increase of 0.7 pcg/ml in IL-17(R2=0.467, P < 0.001)CONCLUSION: Serum concentration of IL6 to TGF-β1 ratio was significantly higher in Iraqi sample of RA patients compared to controls. This may help in early diagnosis of RA and suggest potentially an early effective treatment.
