Abstract

BackgroundMany studies has been found that the pentoxifylline (PX) and L-carnitine (LC) as motility stimulants showed a positive effect on the activation of sperm in vitro and improve the forward movement after cryopreservationOb,iectivesThe present study aim to investigate the possibility of using the motility stimulants(i.ePX ,LC and mixture of PX with LC medium) for invitro sperm activation after cryopreservation of epididymal sperms and their effects in vitro on fertilization rate (FR) and early embryonic development (ED) following in vitro fertilization (IVF) process as a model for obstructive azoospermia in men.MethodsFifty mature male and Fifty mature female mice, 8-12 weeks old were used. Epididymal sperms were obtained from caudal region and in vitro direct activation technique was done after cryopreservation by using four media namely; PX alone, LC alone, a mixture of PX with LC and Ham's F-12 (as control media). The four media were usedfor IVF of mature ova and FRwith EDwere recorded.ResultsIn vitro activation of epididymalsperms with PX , LC and mixture of PX with LC has shown positive effect on sperm concentration, sperm motility, and grade activity of progressive forward movement after cryopreservation. There was a significant (P<0.05) increase of FR in treated media compared with the control medium after 24 hours of insemination. The study showed that the quality and quantity of embryosConclusionIt is concluded from the results of the present study that adding PX, LC and mixture of PX with LC medium to the culture medium of cryopreserved epididymal sperms andIVF leads to an improvement in certain sperm function parameters, supports the FR and early ED rate.

Abstract

L-Carnitine (LC) is highly concentrated in the epididymis and plays a crucial role in sperm metabolism and maturation.They are related to sperm motility and have antioxidant properties. The objective of this review is to summarize the multiple roles played by LC in male reproduction, and to highlight their limitations as well as their benefits in the treatment of male infertility. A variety of studies support the conclusion that LC at total daily amounts of at least 500mg per day can significantly improve both sperm concentration and total sperm counts among men with astheno – or oligoasthenozoospermia. Although many clinical trials have demonstrated the beneficial effects of LC in selected cases of male infertility. Additional, a well – designed study is necessary to further validate the use of carnitines in the treatment of patients with male infertility, specifically in men with poor semen quality .

(الكارنتين) يتركز بشدة في البربخ ويلعب دورا حاسما في استقلاب الحيوانات المنوية والنضج.ترتبط بحركة الحيوانات المنوية ولها خصائص مضادة للأكسدة. الهدف من هذه المراجعة هو تلخيص الأدوار المتعددة التي تلعبها الكارنتين في إعادة إنتاج الذكور، وإبراز دورها وكذلك فوائدها في علاج العقم عند الذكور. تدعم مجموعة متنوعة من الدراسات الاستنتاج القائل بأن الكارنتين الكميات اليومية التي تستخدمها الشخص والتي لا تقل عن 500 ملغ في اليوم يمكن أن يحسن بشكل كبير كل من تركيز الحيوانات المنوية وعدد الحيوانات المنوية الكلي لدى الرجال المصابين بالوهن أو قلة في عدد الحيامن. على الرغم من أن العديد من التجارب السريرية قد أثبتت الآثار المفيدة لي الكارنتين في حالات معينه ومختارة من العقم عند الذكور، إضافية، دراسة مصممة بشكل جيد ضروري لمزيد من التحقق من صحة استخدام الكارنتين في علاج المرضى الذين يعانون من العقم عند الذكور، وخاصة في الرجال الذين يعانون من نوعية السائل المنوي الفقيرة ( قليلة في العدد والنشاط).

Abstract

ABSRACT:BACKGROUND:Carnitine is highly concentrated in the epididymis and Spermatozoa, where it may serve as an intramitochondrial vehicle for theAcyl group, a substrate of energy production. This study is aimed to determine any correlation between male infertility and semen quality with Concentration of L- carnitine in human seminal plasma.OBJECTIVE:This case-control study performed at the Institute of Embryo Research and Infertility, University of Al-Naharin, Baghdad. Semen samples of 40 infertile men (azoospermia, n=12, oligoasthenozoospermia, n=16, teratozoospermia, n=12) and 12 fertile men controls were collected.METHODS:The level of total L- carnitine in seminal plasma was determined by high performance liquid chromatography and the results correlated with sperm parameters as sperm count, motility and morphology.RESULTS:The level of L-carnitine was significantly reduced in infertile male groups; azoospermia, oligoasthenozoospermia and teratozoospermia in comparison with that of fertile male controls (P< 0.045, P< 0.023 and P< 0.04; respectively). There was a borderline significant positive correlation between the seminal L-carnitine and the sperm count in fertile group(r=0.63, P< 0.51).CONCLUSION:The results of this study indicated the significance of determination of seminal plasma L-carnitine in evaluation of infertility in men and the importance of recognition of those infertile male who are benefit from administration of carnitine supplement.

Abstract

Background:L-carnitine is an important antioxidant present in a high concentration in epididymal fluid. It supplies energy for sperms to acquire their motility in the epididymis.Objective:To evaluate the effect of L-carnitine on improving some of the seminal fluid parameters in infertile males.Methods:Ninety-six infertile males were divided into two groups, a group received L-carnitine and the other group received multivitamin. Seminal fluid analysis performed before and after treatment in both groups. Analysis and comparison of the results were carried out between both groups for all patients then for patients with primary and secondary infertility in each group.Results:The patients received L-carnitine showed no improvement in semen volume but a significant raise in sperm concentration observed in all patients. A highly significant raise of sperm motility was observed especially in patients with primary infertility.Conclusion:L-carnitine significantly improves sperm concentration and sperm motility especially in patients with primary infertility. It is recommended for oligoasthenozoopermic infertile men.Key words:Male infertility, antioxidants, L-carnitine, azoospermia.

Abstract

Background:Epilepsy is a common chronic neurological disorder characterized by recurrent unprovoked seizures.Objective:To investigate the possible antiepileptic effect of both atorvastatin and L-carnitine on seizure induced by pilocarpine.Methods:Fifty male albino mice weighing between 30-35 gm were equally allocated into five groups (each group contained 10 mice) and were given one of the following; control group; distal water group (0.1 ml), diazepam group (1mg/kg), atorvastatin group (5 mg/kg) and L-carnitine group (300 mg/kg). All animals (except normal group) were injected with Pilocarpine hydrochloride (350 mg/kg) to induce generalized tonic-clonic seizures 30 minutes after the tested drugs had been administrated. The mean onset of seizure were determined as well as the mean serum concentration of electrolytes, glutathione (GSH) and malondialdehyde (MDA) were measured after seizure had been induced.Results:Pilocarpine induced seizure at approximately 7 minutes after injection, while both atorvastatin and L-carnitine produced highly significant increase in mean onset of seizure 14 ± 0.471 and 14.5 ± 0.909 respectively as compared to that of D.W. group, also both drugs produced highly significant changes in mean serum concentration of electrolytes, GSH and MDA.Conclusion:Atorvastatin and L-carnitine had antiepileptic effects against seizures induced by pilocarpine when used at applied doses.Key words:Epileptic seizure, Atorvastatin and L-carnitine.

Abstract

The dietary supplement industry is constantly developing, continuously offering a wide variety of products for athletes and non-athletes. This study has conducted to investigated the biological activity of L-carnitine on some physiological criteria in the sera of sixty sportsmen (volunteers) male only in age of ( 18-24 ) years in Kirkukcity in spring of ( 2015 ) , the particular testing period continue for (4) weeks and the volunteers were randomly divided into (2) groups each group include (30) volunteer as following: The first group of athletes was control groupin which did not supplysupplementations,the second group of athletes who have used L- carnitine (500 mg) per day and an average of twice a day for 15 dayssupplementations. The results have been indicated the following: The sportsmen whom received L- carnitine every day for 15 days showed a significant increase in the concentrations of High-density lipoproteinHDL –cholesterol while there was a significant decrease in the concentrations ofAspartate Transaminase (GOT) ,Alanine Transaminase (GPT) andAlkaline phosphatase(Alp) enzymes , total cholesterol(Cho) and Triglycerides(T.G) in comparison with control group before and after the exercise. The current study revealed that using L-carnitine as supplements for sportsmen have an important effects on most of physiological and biochemical activities of the body and a positive effect to improve exercise performance.

صناعة المكملات الغذائية تتطور باستمرار، وتقدم مجموعة واسعة من المنتجات للرياضيين وغير الرياضيين. أجريت الدراسة الحالية على ستين رياضي (متطوعين)ذكور فقط في سن (18-24) سنة في مدينة كركوك في ربيع عام (2015)، واستمرت الاختبارات لمدة (4) أسابيع , تم تقــسيــم المتــــطوعين عشوائـــيا ً الى مجموعتييـــــن كل مجموعة تضم (30) متطوع على النحو التالي:المجموعة الاولى (مجموعة السيطرة)تضم الرياضيين دون اخذ اي نوع من المكملات والمجموعة الثانية تضم الرياضيين الذين استخدموا L- الكارنيتين بكمية (500 ملغ) يوميا وبمعدل مرتين في اليوم لمدة 15 يوما.هدفـــت هذه الدراسة الــتحـقــق من النشاط البيولوجي Lـ كارنيتين على بعض المعايير الفسيولوجية في مصل الرياضييناظهرت النتائج زيادة كبيرة في تركيزHDL -cholesterolفي الرياضيين الذين حصلوا على Lـ كارنيتين كل يوم لمدة 15 يوما , في حين كان هناك انخفاض ملحوظ في تراكيز انزيمات GPT GOT,وAlpوالكوليسترول الكلي والشحوم الثلاثية مقارنة مع مجموعة السيطرة قبل وبعد ممارسة الرياضة. يستنتج من هذه الدراسة أن استخدام L ــ كارنيتين كمكملات للرياضيين لها تأثيرات هامة على معظم الأنشطة الفسيولوجية والبيوكيميائية للجسم ولها تأثير إيجابي على تحسين أداء التمارين الرياضية.

Abstract

The aim of the present study is to investigate and compare the efficacy of L-carnitine, multivitamins and their combination therapies on semen characteristics in idiopathic male infertility. Idiophathic infertile patients were randomly divided into three groups who had received three different treatment regimens for three months: group A (45 patients) has received 2 grams daily of L-carnitine alone; group B (55 patients) had received the combination of L-carnitine (2 grams daily) plus one tablet daily of multivitamins (Stresstabs®); and group C (29 patients) had received one tablet daily of multivitamins alone. The study was started on 1/11/2009 and completed on 31/3/2010 and performed at Rizgari Teaching Hospital in Hawler City/Erbil Governorate. Thirty fertile male volunteers were used as a control group as well. Seminal fluid analysis has been done before treatment and then monthly after treatment for three month. The results of the present study clearly demonstrated that the combination therapy of L – carnitine and multivitamins was more efficient than each drug therapy alone and this evidenced by improvement and significant increase in the semen parameters; sperm concentration (72%), sperm count (79%), actively motile sperm (29%), and progressive motile sperm count (125%) when compared to pretreatment values than either L – carnitine or multivitamins therapy alone (53%, 60%, 19% and 84%), (21%, 30%, Non Significant, 34%) respectively.The results demonstrated that the combination therapy of L-carnitine and multivitamins was more efficient and produced more significant improvement in semen characteristic than either therapy alone, and that L-carnitine therapy was more efficient than multivitamins.Improvement in these semen parameters can aid in the treatment of idiopathic male infertility.

إن هدف هذه الدراسة هو التحقق عن و مقارنة كفاءة علاجات لـ - كارنتين أوالفيتامينات المتعددة أوكلاهما معا على خصائص السائل المنوي (حجم السائل المنوي, تركيز الحيامن, عدد الحيامن, شكل الحيامن, حركة الحيامن, عدد الحيامن ذات الحركة المتقدمة, وعدد الخلايا المدورة) .في عقم الذكور مجهول السبب في محافظة اربيل, هادفة إلى تحسين هذه الخصائص خلال ثلاثة أشهر من العلاج ولاكتشاف الطريقة الصحيحة لتقديم البيانات المختبرية لتحليل السائل المنوي والتي يتم عملها في محافظة اربيل. في هذه الدراسة تم تقسيم 129 مريض ( والعقم مجهول السبب ) بطريقة عشوائية إلى ثلاثة مجاميع كل منها اخذ علاج يختلف عن الأخر لمدة ثلاثة أشهر مستمرة: المجموعة – أ – (45 مريض ) تناولوا 2 غرام من لـ - كارنتين يوميا, المجموعة – ب – (55 مريض ) تناولوا 2 غرام من لـ - كارنتين مع حبة واحدة من الفيتامينات المتعددة ( ستريستابز®). المجموعة – ج – ( 29 مريض ) تناولوا حبة واحدة من الفيتامينات المتعددة ( ستريستابز®). وقد تم اخذ وتسجيل مجموعة أخرى ( الكونترول ) في هذه الدراسة والتي تضمنت 30 متطوع ذكر خصب. لقد تمت متابعة المرضى وتم تحليل السائل المنوي قبل البدء بأخذ العلاجات وشهريا خلال اخذ العلاج لمدة ثلاثة أشهر. هذا وقد تم استخدام فحص (t) ذو النموذج الزوجي للمقارنة بين القيم قبل العلاج والقيم المقابلة لها بعد شهر وشهرين وثلاثة أشهر من العلاج, وقد اعتبرت قيمة احتمالية الصدفة (P) ذات أهمية إحصائية عندما تكون قيمتها اقل من 0,05 . لقد أظهرت نتائج هذا البحث بصورة واضحة إن لـ - كارنتين المستخدم بجرعة 2 غرام يوميا لمدة ثلاثة أشهر مستمرة ممكن أن يحسن تركيز الحيامن, عدد الحيامن, نسبة الحيامن المتحركة بصورة فعالة, وعدد الحيامن ذات الحركة المتقدمة عند الرجال الذين يعانون من العقم ( قلة تركيز الحيامن او قلة سرعتها او كلاهما) مجهول السبب, وان العلاج الذي يجمع بين لـ - كارنتين و الفيتامينات المتعددة لمدة العلاج نفسها هو اكثر كفاءة ( حصول زيادة مهمة احصاءياً في خصائص السائل المنوي المذكورة أعلاه بمقدار 72%, 79%, 29%, 125% بالتعاقب مقارنة مع القيم المقابلة لها قبل العلاج ) من علاج لـ - كارنتين لوحده ( حصول زيادة مهمة احصاءياً في خصائص السائل المنوي المذكورة اعلاه بمقدار 21%, 30%, غير مهم احصاءياً, 34% بالتعاقب مقارنة مع القيم المقابلة لها قبل العلاج ) لتحسين خصائص السائل المنوي هذه ومن الممكن أن يساعد في علاج عقم الرجال مجهول السبب. بالإضافة لذلك, علاج لـ - كارنتين من الممكن أن يكون أفضل من علاج مجموعة الفيتامينات المتعددة باستخدام نظام العلاج الذي تم تطبيقه في هذه الدراسة في تحسين خصائص السائل المنوي علاج عقم الرجال مجهول السبب

Abstract

This study was conducted at the Poultry Farm of Department of Animal Resource, College of Agriculture, University of Baghdad during the period from 15 / 2 / 2011 to 1 / 8 / 2011. The aim of this study was to investigate the effect of dietary supplementation with different levels of carnitine on fertility and hatchbility of guinea fowl.A total of 42 guinea fowl male, 30 weeks of old were used in this study. Birds were randomly distributed into 4 treatment groups (C0, C100, C200, C300) which is added to the diets carnitine levels 0, 100, 200, 300 mg Carnitine / kg diet. Birds were fed during the whole period of birds rearing which lasted 22 weeks on diet contain 18.38 % crude protein and 2962.1 Kcal metabolic energy / Kg. L-carnitine was added to the diets of birds at the beginning of 34 weeks of birds age till the end of experiment which lasted 18 weeks including 2 weeks which considered as preliminary carnitine treatment period. Guinea fowl were reared during experimental period in separated cages.The resulted Showed significant improvement (P<0.05) or (P<0.01) in fertility and hatchability guinea fowls.Conclude from this study that the addition of Carnitine to the diets of Guinea fowl is leads to improved productive and reproductive performance in guinea fowl male

أجريت هذه الدراسة في حقل الطيور الداجنة التابع لقسم الثروة الحيوانية / كلية الزراعة / جامعة بغداد للمدة من 15/2/2011 ولغاية 1/8/2011، لبحث تأثير إضافة مستويات مختلفة من الكارنتينL – Carnitine إلى العليقة في الإداء الإنتاجي لدجاج غينيا. أستعمل في التجربة 24ذكر و 72 أنثى من دجاج غينيا بعمر 30 أسبوع. وزعت عشوائيا ًعلى أربعة معاملاتC0،C100، C200، C300، التي تمثل إضافة الكارنتين إلى العلائق بمستويات 0، 100، 200، و300 ملغم كارنتين / كغم علف.غذيت الطيور طوال مدة التربية على عليقة موحدة تحتوي 17.77% بروتين خام و 2933.8 كيلو سعرة / كغم طاقة ممثلة. أضيف الكارنتينL – Carnitine إلى تلك العلائق إبتداءً من عمر 34 أسبوع ولغاية نهاية التجربة البالغة 18 أسبوع بضمنها فترة أسبوعين استخدمت بمثابة فترة معاملة تمهيدية بالكارنتين. اشارت نتائج التجربة الى ان اضافة الكارنتين الى علائق دجاج غينيا ادت الىارتفاع عالي المعنوية في نسب الخصوبة والفقس من البيض الكلي والبيض المخصب وانخفاض عالي المعنوية في نسب الأجنة الهالكةلصالح معاملات إضافة الكارنتين الثلاث، وقد سجلت المعاملة الثالثة (C300) أفضل النتائج مقارنةً بمجموعة السيطرة والمعاملتين الأولى والثانية (C100 و C200).يستنتج من هذه الدراسة أن إضافة الكارنتين إلى علائق دجاج غينيا يعمل على تحسن نسب الخصوبة والفقس لدجاج غينيا ليحسن بذلك الأداء الانتاجي والتناسلي للطيور نفسها

Abstract

ackground:Pretreatment of semen samples of the infertile patients and in vitro preparation techniques with the certain sperm stimulant agents prior to use for assisted reproductive techniques (ART) has been reported to have beneficialeffectsonpregnancyrates.Objectives:The objective of this study is to improve certain sperm function parameters in vitro of infertile patients semen by using novel combination motility stimulants substances that improved medium to be used for ART in future.Patients and Methods:One hundred infertile men involved in the current study. They were divided into four groups. Each semen sample was divided into four portions. One part was considered as a control and in vitro activated by using culture medium only. The other three portions were considered as treated groups and activated in vitro by adding pentoxifylline(PX,1mg/ml) and/or L-carnitine (LC,0.5mg/ml) to the culture media. Certain sperm function parameters were examined before and following in vitro activation using layering and sedimentation techniques.Results:The results showed a highly significant(P<0.001)improvementinthespermconcentrationand percentage of progressive sperm motility grade(A), grade (B) and grade (A+B) with a highly significant(P<0.001)increaseinthepercentageofmorphologicallynormalsperm(MNS) after addition of PX and/or LC to the culture medium in comparison with control medium using layering and sedimentation(layering under paraffin)techniquesintheinfertilepatients. .Conclusion:The results of this study indicate that adding 20%PXand/ or LC to the culture medium for in vitro sperm activation using layering and sedimentation techniques leads to an improvement in certain sperm function parameters.

This study was designed to assess the effect of different concentrations of L-carnitine and Co enzyme Q10 (COQ10) supplied to SMART Pro- medium on human sperm motility and sperm grade activity ( progressive, non-progressive and immotile) during in vitro sperm activation.Eighty seven samples of semen of infertile and fertile men were randomly collected from the subjects at the High Institute for Infertility Diagnosis and Assisted Reproductive Technologies / Al- Nahrain University. The mean age of the men was (32.034 ± 0.57) years with mean duration of infertility (4.644 ± 0.25) years. Seminal fluid analyses were done involving macroscopic and microscopic examinations according to WHO criteria. Swim-up technique was dependent for in vitro sperm activation , the washed samples were divided before using centrifugation swim-up technique into 3 groups: control group (G1) without L-carnitine and CoQ10. While, in G2, G3 two concentrations of L-carnitine and CoQ10 (20 μg , and 40 μg ) were used. Moreover, all groups within post-activation enriched with SMART- Pro media. The sperm motility and sperm grade activity were evaluated after the addition of L-carnitine and COQ10. The results showed a significant (P<0.05) differences were showed in sperm motility and sperm grade activity of post-activation when compared with the pre-activation. In G2 and G3, with dose of ( 20 μg and 40 μg) of L- carnitine and CoQ10, the results showed a significant (P<0.05) increase in the percentages of sperm motility, progressive sperm motility in spite of the increase in the percentages of sperm motility, progressive sperm motility in G3 compared to G2, there was non significant (P>0.05) differences between G2 and G3. The results for non-progressive sperm motility% and total progressive sperm motility millions/ mL. for 40 μg ( G3) groups were significantly (P< 0.05) decreased when compared to control group (G1) with non-significant (P>0.05) differences between 20μg (G2) group, with significant (P< 0.05) decreased in immotile sperm percentage in post-activation when compared to the pre-activation. From the results of the study, it can be concluded that the addition of 40 μg of L-carnitine and CoQ10 to washed sperms can improve sperm motility and sperm grade activity in vitro. Data were analyzed statistically using complete randomized design (CRD) (one way ANOVA).
