Morphometric analysis of mandibular canal course and position in relation to gender and age of Iraqi sample using digital panoramic imaging

Abstract

Background: The knowledge of the course and position of the mandibular canal in relation to anatomical landmarksof the jaw is of great importance in certain dental interventions; therefore it involves preservation of the anatomicstructures that pass through it. Morphometric study by means of digital panoramic radiography reveals thedifferences and inherent alterations between genders or ages, it has become a useful tool in research that analyzesthe craniofacial complex development process, which are important for planning the dental, clinical and surgicalprocedures. The aim of the study was undertaken to analyze mandibular canal morphology and position along itscourse and its relation to mandibular anatomical landmarks by using digital panoramic radiography.Materials and Methods: 300 subjects, (150 male and 150 female) were examined, age distribution of them wasranging between (20-49) years old, factors considered included the age and sex of the patients. Ten linear verticalmeasurements were performed on the radiographic image of each subject on both right and left sides of themandible (600) sides, to evaluate the position of the mandibular canal along its course. The relation of proximitybetween the mandibular canal and the roots of the mandibular posterior teeth were also evaluated.Results: Statistically significant differences were observed in six of the linear measurements between genders, malesalmost have higher measurements than females. However, no significant linear trend was evident between agegroups of the same gender for any of the measurements. The most frequent position of the mental foramen wasbetween the two mandibular premolars, the mandibular canal presented a relation of proximity to the roots of themandibular third molar, moving gradually away from the roots of the other posterior teeth.Conclusion: the measurements related to the mandibular canal performed in this study can be influenced by thegender but are independent of age

Abstract

Background: mandibular canal course variations are found very rarely, nevertheless, their recognition may play avery important role prior to any surgical procedure involving the lower jaw. This study aimed to identify the variationsin the different parts of the radiographic course of mandibular canal using digital panoramic imaging.Materials and Methods: The courses of the mandibular canal in 319 panoramic images of Iraqi subjects wereevaluated carefully and any rare radiographic finding was recorded when identified.Results: A total of (5.95 % of the radiographic images examined in the present study demonstrated mandibular canalvariations. The most common variation observed in the present study was bifid mandibular canal (1.88 %), followedby indistinct mental foramen (1.57%), then indistinct mandibular canal walls (1.25%) and indistinct mandibularforamen (1.25%).Conclusion: The course and configuration of the mandibular canal should be carefully observed in different locationsbecause it posses many variations. Recognition of these variations is important in preventing the intra and postoperative complications in certain dental procedures

على الرغم من أن التباین في مسار قناة الفك الاسفل ھو من الحالات النادرة الا ان اكتشاف ذلك التباین لھ دور بالغ الاھمیة قبل اجراء اي عملیة جراحیة في الفك الاسفل. الغرضمنھذه الدراسة ھو تحدید التباین الموجود شعاعیا في مناطق مختلفة على طول مسار قناة الفك الاسفل باستخدام التصویر بالاشعة البانورامیة الرقمیة.تم تقییم مسار قناة الفك الاسفل بدقة في 319 صورة اشعة بانورامیة لمواطنین عراقیین وقد تم تسجیل جمیع الحالات النادرة في مسار قناة الفك الاسفل حیثما وجدت.اثبتت النتائج ان % 5.95 من العینة الكلیةالمفحوصة لدیھم تباین في مسار قناة الفك الاسفل. كانت قناة الفك الاسفل المنشطرة ھي النوع الاكثر انتشارا بین تلك التباینات اذ شكلت (%.( 1.88 ) یتبعھا عدم وضوح الثقب الذقني (% 1.57 ) ثم عدم تمییز جدار قناة الفك الاسفل (% 1.25 ) واخیرا عدم وضوح ثقب الفك الاسفل (% 1.25ان المسار والشكل العام لقناة الفك الاسفل یجب ان یلاحظا بدقة في مواقع مختلفة بسبب التباینات المتعددة لھذه القناة. ان لتمییز ھذه التباینات اھمیة في تلافي المضاعفات التي قد تحدثاثناء وبعد بعضعملیات طب الاسنان

Abstract

BackgroundResorption of alveolar bone is the best recognized feature of mandibular aging inthe edentate subject. The aim of this is study to evaluate the mandibular canal wallresorption using panoramic radiographs among edentulous male and female.Material and methodThe sample of this study was collected from patients who attended TeachingHospital of the College of Dentistry at Al- Mustansiria University and panoramicradiographs were taken in Al-Karama specialized center for dentistry. Forty threepatients were selected in this study with age range between 59-85 years (25 malesand 18 females) . Panoramic radiographs for all patients and sides were examinedand evaluated .ResultsChi-square between males and females of angular cortical thickness (ACT) ≥1mm and ACT ˂ 1mm was (37.06) which statistically significant with p-value(0.008) and between resorption (grades) and age of males was(34.72) which itstatistically significant p-value(0.049) also between resorption (grades) and age offemales was(15.6) which was statistically significant p-value (0.003) . Spermancorrelation showed statistically significant moderate correlation between gender andage (r=0.581,p-value=0.01) and there was a moderately statistical significantcorrelation between the grades of mandibular canal resorption and the angularcortical thickness measurements in both gender (r=0.532,p-value=0.033).ConclusionThe resorption of the wall is found more often in edentulous females than in themales due to hormonal imbalance.

Abstract

To make comparative evaluation of gonial cortical bone thickness,antegonial index, gonial angle values and mandibular canal boneresorption, pathologies like ground-glass appearance in jawbonesand brown tumor in Iraqi patients undergoing dialysis due tochronic renal failure and patients from the healthy control groupon panoramic .Panoramic radiographs were taken from 40 patientsdivided into two groups (20 normal and 20 dialysis patients).Gonial cortical thickness, antegonial index and gonial anglevalues with the mandibular canal wall resorption were assessed ,pathologies such as ground-glass appearance and Brown tumor as“available” or “not available.” Mandibular canal wall resorption(Chi-square:16.52 , P<0.01) was found highly statisticallysignificant difference between the patient and control groups,resorption detected mainly in right side of male patients.Panoramic radiographs showed decreasing in values of themeasuring parametersin patients receiving dialysis treatment dueto chronic renal failure compared to the control group.

Abstract

The mandibular canal appears below or superimposed over the apices of themandibular molar teeth but it has some variations regarding its distance to both lowerfirst molar and the base border of the mandible that may make a problem specially forthe oral surgeon in his work during operations like implantation, so, this study wasdone to estimate the position of the mandibular canal and its relation to the lower firstmolar and the base border of the mandible by the aid of digital panoramicradiographs.The sample of this study was collected from patients who attended Al-Karamaspecialized center for dentistry. Forty patients were selected in this study with the agerange between 20-60 years (males and females) that divided in to four groupsaccording to a special criteria. Forty digital views (OPG) were taken for Iraqi patients,using computerized digital panoramic x-ray machine. All radiographs were examinedand then the position of the mandibular canal for each patient was estimated.The results revealed that the mandibular canal is most commonly located awayfrom the root apices of lower first molar, and the distance between the mandibularcanal and the base border of the mandible is indirectly proportional with age.
