Abstract

THE ROLE OF ALLERGIC RHINITIS IN THE AETIOLOGY OF NASAL POLYPSHashim AL-Abdul Wahed*, Husam Haider#*CABS, FICMS. ENT Dept., Basrah general hospital, Basrah, Iraq, #FRCS, DLO. ENT Dept. Basrah Medical College, Basrah, IraqAbstractDespite the prevalence and long history since nasal polyps were recognized as a clinical entity, many questions still exist with respect to their aetiology & pathogensis, this study aims to assess the role of allergy in the aetiology of nasal polyps based on their epidemiology in Basrah governorate in Iraq & by utilizing the skin test, from January 2000-december 2000 84 patients with simple nasal polyps 50 males & 34 females with age range between 18-70 years were studied by collecting data regarding the history, physical examination& investigations in a questionnaire form .88 normal subjects were also included as a control group. Both patients & controls were subjected to intradermal skin test using seven common aeroallergens.Out of the 84 patients 58 (69%) had a positive skin test to one or more of the tested allergens, while only 20 (23%) of the control group had a positive test. This difference is statistically significant (p<0.00001), odd ratio=8.0 which means allergic persons were 8 times more prone to have polyps than normal subjects.Allergy appears to play an important role in the aetiology of nasal polyps and the skin test is useful in the assessment of patients with polyps who may therefore benefit from immuno therapy.

Abstract

NASAL GLIOMA, A CASE REPORT AND LITERATUREREVIEW���������.Nasal glioma (also known as nasal glial heterotopia) has been used to describe a congenitalbenign tumor of the nasal region containing neural tissue. It arises from failure of closure offoramen caecum at about the third week of gestation. The patient described in this report isan 18 months old girl who presented with a nasal bridge swelling for three months. Thedifferential diagnosis included nasal encephalocele, nasal dermoid and epidermoid cysts. Allare due to failure of ectoderm and neuroectoderm embryologic separation. CT scan and MRIimaging can be used to look for probable concomitant intracranial tumors and the existence ofa connection between nasal tumor and the brain. Surgical resection is the usual method ofmanaging such pathologies. In this case, an open rhinoplasty approach was used to resectthis mass after the radiologic evaluation was complete.

Abstract

Aims: To reveal the changes of the soft tissue profile of the nose, that include; length, height and depth of the nose among four age groups. Materials and Methods: The studying sample subjects included 48, 41, 50 and 44 individuals of age 11, 12, 13 and 14 years respectively. The subjects were Iraqi individuals of class I normal occlusion, who live in center of Mosul City. All subjects were radiographed with the lateral cephalometric films, these films were traced and drawing the linear parameters of the nose, the tracing involve the length (N` – Prn), width (N – Sn) and depth (Prn – Prn`). The results were subjected to the descriptive and variance analyses. Results: The results appeared in males the length of the nose increase in dimension with no significant differences when compared among the four age groups, whereas in females showed significant increase when compared the 11, 12 years with 13 years and with 14 years age groups. While, the h weight of the nose (N`– Sn) appeared in males and females significantly increase at 14 years age group when compared with other age groups. But significantly increased at 12 years age group as compared with 11 years age group, in males only. Mean while, the depth of the nose (Prn – Prn`) displayed significantly increase at age 14 years group when compared with 11 and 12 years age groups for both sexes. The comparison between sexes for the nose parameters demonstrated that the length, width and depth of the nose displayed significantly increase in males than females at 11 years age group and no significantly differences at other age groups. Conclusions: The conclusions of the study are that the nose have significantly increase between 11 and 14 years age group in both sexes, the nasal profile parameters have insignificant difference in dimensions among 12, 13 and 14 years age groups.

Abstract

Aim: To construct a nasal prosthesis for a patient with Rhino cerebral mucormycosis disease and of
near total Rhinectomy. Materials and method: a permanent soft lining material was adapted over
frame work of nose made from visible light cure denture base material, then cured, finished and
polished. Results the finished nose was retained by engaging undercut and by the use of eye glass.
Conclusion: A custom sculpted prosthesis combined with soft lining material gives a natural texture of
the nose was accomplished within 24 hour.

Abstract

Background: Chronic respiratory obstruction can be produced by prolonged inflammation of the nasal mucosaassociated with allergies or chronic infection. It can also be produced by mechanical obstruction any where withinthe nasorespiratory system from the nares to the posterior nasal change. There may be a relation between theskeletal features of the facial skeleton and the respiratory pattern and this relation should be studied carefully. Thisstudy aimed to study and compare the relation between the nasal obstruction and the skeletal features of the facialskeleton in adults between the sample and control groups and to find the Pearson correlation coefficient betweenthe variables related to the nasomaxillary complex for the sample and control groups.Materials and methods: The sample is composed of 50 Iraqi Arab adult patients out of 167 with age range from 18-35years, having nasal obstruction had been examined in Tikrit general hospital and identified by the E.N.T. specialist.The control group will be 50 subjects out of 150 in the same age group and also had been examined and identifiedby the E.N.T specialist in Tikrit general hospital as having no nasal obstruction. Twelve linear measurements and a ratiowere determined on each radiograph. For each variable, the arithmetic mean, the standard deviation, and thestandard error were calculated. For statistical evaluation, an independent-samples t-test was performed. ThePearson correlation coefficient was calculated to indicate the relationship between nasal and maxillary variables.Results: There was no significant difference between the sample and control groups for the skeletal features of thefacial skeleton except the ramal height of the right and left sides and there was weak correlation between the nasaland maxillary variables for the sample and control groups.Conclusion: There is no effect of the nasal obstruction on the skeletal features of the facial skeleton in Iraqi adultsample.

Abstract

Prof. of Otolaryngology, Dept. of Surgery, College of Medicine, Basrah University. #MB,ChB, CABS, Basrah General Hospital, Basrah, IRAQ.Abstract Nasal packing is carried out in many nasal surgeries specially in septoplasty for hemostasis and internal stabilization of the cartilaginous & bony skeleton of the nose. However, these packing methods are not a harmless procedures and for this reason their benefit has been tested. The study was performed to compare the use of plastic intranasal splints with or without anterior nasal packing to determine the need of nasal packing after septoplasty.This is a prospective comparative study which was carried out in Basrah General Hospital, Otolaryngology Department, Basrah, Iraq in the period from July 2014 to April 2015.Forty seven patients of different age and gender groups were included in the study. Analysis was done regarding their history and examination including nasendoscopy which was done before septoplasty. The patients were randomly selected at the end of the procedure into two groups; one had plastic intranasal splints without anterior packing and the other had plastic intranasal splints with two different types of packing materials (ribbon gauze impregnated with Vaseline and glove fingers). Patients were compared for postoperative pain, headache, discomfort, bleeding, sleep difficulty, epiphora, septal hematoma on the day of the operation, 24 hours and 48 hours postoperatively. The collected data was analyzed by using SPSS v.17. The age of patients in this study ranged from 18 to 44 years with mean age of (29.6±6.2) years and most of them were between 28 to 37 years (48.9%). Among those 47 patients, 28 (59.6%) were males and 19 (40.4%) were females with male:female ratio was 1.47:1.Pain, headache, discomfort and bleeding was higher in patients in group two with anterior nasal packing materials especially during removal of the packing. there was significant reduction in the frequency of sleep disturbance, epiphora among the patients in group one without nasal packing, however there was no significant difference in septal hematoma between two groups.In conclusion, the plastic intranasal splints can be used alone without the need of anterior nasal packing materials after septoplasty as it can be considered as a type of packing methods that offers the advantages of elimination of pain and discomfort and reduce the complications of packing. The anterior nasal packing should be reserved only for selected cases.

Abstract

Objectives: To assess the characteristics of 195 cases with nasal foreign bodies regarding the sex , age , types , site and complain at presentation. Materials and Methods : One hundred ninety five patients with nasal foreign bodies , their age range of 1-12 years old visiting the private clinic from March 2010 – June 2015 retrospectively analyzed. Results : The nasal foreign bodies mostly presented with males , the most common age group from 1-6 years and the most common foreign bodies were beads and plastic toys. Conclusions : Nasal foreign bodies one of the most common otolaryngological emergencies in pediatric practice , although it is not life threatening , it still need special care to prevent complications.

Abstract

Background and objective: Nimodipineis effective in the treatment of various cerebrovascular impairment, but its clinical potential is limited due to several undesirable characteristics such as low bioavailability which caused by first pass effect in the liver and low aqueous solubility. The main purpose of this study was to prepare nimodipine-loaded nanoliposomes for intranasal delivery and performing in-vitro studies.Methods: The nimodipine loaded nanoliposome formula was prepared by ethanol-based proliposome method and characterized in term of particle size and size distribution, zeta potential, entrapment efficiency, and permeability studies. In addition, mucoadhesive nanoliposomes loaded nimodipine was prepared using chitosan as a mucoadhesive agent. Results: The in-vitro studies explored particle size increased (122.48nm±0.002) for chitosan coated formulation after sonication in comparison to non-coated formulations (114.09 nm ± 0.025). Also, the zeta potential was positive for the sonicated chitosan-coated formulation (5.286 mV ±0.341) while for non-coated formulations with chitosan were found to be negative (-1.317 mV ± 0.153). The entrapment efficiency (76.033 % ± 0.094 %) and drug permeability (2.53 μg/ml in the duration of 240 min) were significant for chitosan-coated liposome compared to other formulations.Conclusion: This study concludes that chitosan glutamate (PROTASAN® UPG213)coatednimodipine-loaded liposomes can be considered as a promising novel formulation for an efficient intranasal delivery of nimodipine. Additionally, changes in the size of liposomes and zeta potential confirmed the existence of a coating layer on the surface of liposome pellets. Chitosan was found to significantly enhance the drug entrapment and could also be considered as a permeability enhancer.

Abstract

ABSTRACT :BACKGROUND: Epistaxis is one of the commonest medical emergencies. It affects all age groups and both sexes. The causes may be local or systemic. Trauma is considered to be a major etiological factor. Various treatment protocols are utilized to control epistaxis depending upon the type, severity and cause of bleeding. OBJECTIVE:To show most etiological factures that causes epistaxis and who we manage each case by different method.MATERIALS AND METHODS : This descriptive study was designed to evaluate the etiology and efficacy of management of epistaxis.210 patient underwent prospective evaluation in the otolaryngology department of Al Diwaniyah Teaching Hospital . Standard principles where followed in the management.RESULTS: This study demonstrated a bimodal distribution with incidence peaks below25 years and above50 years of age. Males affected more than females (1:1.16) . Anterior nasal bleeding was noted in the majority of patients. Anterior nasal packing was the most effective method of controlling anterior epistaxis followed by chemical cautery. While posterior bleeding was controlled by posterior nasal packing and anterior nasal packing. The most common cause was found to be trauma followed by hypertension.CONCLUSION: Epistaxis affects all age groups and it has a bimodal age presentation, it affects both sexes equally. Anterior nasal bleeding is more common than posterior bleeding .Epistaxis may be controlled with chemical or electrical cautery if the bleeding point is visible. In case of failure to localize or access a bleeding point or profuse bleeding, anterior nasal packing can effectively control majority of epistaxis cases. Foley s catheter is a good option that can be used for posterior nasal packing. Gelfoam may be used for controlling epistaxis in cases of bleeding disorders, when there is mucosal ooze.

Abstract

AbstractBackground: Nasal septal deviation is a common cause for nasal obstruction and usually treated by surgery. Submucous resection of the septum (SMR) and septoplasty are the common procedures for the treatment of nasal septal deflection. Aims of the study: To audit the functional outcome of the patients undergoing submucous resection of the septum and septoplasty for deflected nasal septum.Materials and methods: 100 patients are complaining from symptomatic nasal obstruction due to nasal septal deviation. Their ages are ranged from 18-55 years. They were divided in to two groups, 60 of them were treated by SMR and 40 patients (caudal end and anterior deviation) were treated by septoplasty in Alzahra Teaching Hospital, Al- Kut / Iraq. From January 2009 to December 2011. Both surgeries were done under general anaesthesia with anterior nasal packs for 24 hours. For the patients with septoplasty a bilateral silastic splints left for 10 days. Follow up done in the outpatient department for 6 months to 1year.

المستخلصدراسة مستقبلية لمائة مريض يعانون من انسداد الأنف نتيجة انحراف الحاجز الأنفي تتراوح أعمارهم بين 18 و 55 سنة . عولجوا في مستشفى الزهراء التعليمي للفترة من ( كانون الثاني 2009 لغاية كانون الأول 2011 ) حيث قسموا إلى مجموعتين, 60 مريض أجريت لهم عملية تعديل الحاجز الأنفي بينما 40 مريض أجريت لهم عملية تقويم الحاجز الأنفي تحت التخدير العام . النتائج كانت استفادة ( إزالة انسداد الأنف ) 50/60 مريض (83,3%) بعد عملية تعديل الحاجز الأنفي مقابل 28/40 مريض (70%) بعد عملية تقويم الحاجز الأنفي علما بان إجمالي المضاعفات كانت متقاربة (33,3% مقابل 30% على التوالي ) . يستنتج من ذلك هو تفوق عملية تعديل الحاجز الأنفي بإزالة انسداد الأنف للمرضى الذين يعانون انحراف الحاجز الأنفي .
