ONABOTULINUM TOXIN-A FOR OVERACTIVE BLADDER

Abstract

Abstract The propose of this study is to evaluate the effect of Botulinum A intradetrusor injection on patients with overactive bladder refractory to oral anticholinergics and safety of the drug. Patients with overactive bladder (excluding patients with neurological or pathological causes) were enrolled in this study, those patients did not benefit of oral medications with anticholinergic effect, and they were assessed for history of their problem and kept a urinary diary for three days before the start of the procedure. They were also examined, and ultrasound for post voiding residue with urinary flow rate was kept in records. Injection of 200 IU of onabotulinom toxin-A in the detrusor muscle. The drug effect was studied one month after injection on patient’s symptoms, including day and night frequency, urgency and urge incontinence by keeping a three days’ urinary diary postoperatively. The maximum flow rate and the post voiding volume were also measured. Postoperative complications in the form of haematurea, urinary tract infection and retention of urine were recorded. There was a significant decrease in the number of voiding during both day and night from 10.7±1.68 SD, 2.09±0.93 SD to 4.407±1.394 SD, 1.257±0.752 SD respectively which is statistically significant difference (t (135) =6.377, P≤0.001), (t (135) =0.837, p≤0.001), this was associated with a reduction in the number of episodes of incontinence from 1.37±0.975 SD to 0.6815±0.676 SD. The postvoiding volume was increased remarkably. Complications includes urinary tract infection and retention of lower tract local effects only. In conclusion, the use of onabotulinom toxin-A in the treatment of refractory overactive bladder is effective in reducing patients’ symptoms and discomfort as well as being a safe option for treatment.

Abstract

ABSTRACT:BACKGROUND:We explored the effectiveness of enteric coated diclofenac sodiumfor patients with benign prostatic hyperplasia (BPH) complaining of nocturia. A total of 30 BPH patients aged 50-75 years were enrolled in the study. They each took a single 50-mg tablet of diclofenac sodium prior to sleeping at night for 14 days in addition to their BPH treatments In the questionnaire ,21 of 30 patients (70%) felt more satisfaction than previous treatments.OBJECTIVE:In this study, we attempted to investigate the role of enteric coated diclofenac sodium (voltaren) in the therapeutic management of BPH patients with nocturia.METHODS:Thirty patient with BPH ( 63.6± 6.56 years old) more than two episodes of nocturia per night were involved. These patients had received standard drug therapy. Although these patients had received standard drug therapy for more than half a year, they had still three or more episodes of nocturia. The patients took a single dose of 50 mg of diclofenac sodium enteric coated at night prior to sleep. Before and 2 week after the initiation of this therapy, the effects of this treatment were assessed by frequency volume chart and a questionnaire.RESULT:In the questionnaire ,21 of 30 patients (70%) felt more satisfaction than previous treatments. Patients were grouped into a diclofenac sodium-effective (n = 23) and ineffective groups (n = 7) based on the results of the frequency-volume chart. In the effective group, interestingly, night-time urine volume showed significant reduction (P < 0.001). On the other hand, the average single voided volume at night showed no significant change. There was a statistically significant difference in the night-time urine volume after treatment between groups (P < 0.05). In frequency volume chart, total void per day, total void per night, total urine volume per day, total night urine volume per day and single voided volume in the night before and after this treatment were 9.9±0.56and 10.1±0.69per day, 3.9±0.87 and 2.3±1.1 per night, 1500.2±106.9 and1508.7±107.3 mL per day, 580.2±136.9 and 350.4±169.3 mL per night, and 150±7.2 and 150.4±7.05mL, respectively, in a diclofenac sodium-effective group.CONCLUSION:Diclofenac sodium can be effective and useful for BPH patients with nocturia

Abstract

ABSTRACT:BACKGROUND:The overactive bladder (OAB) is extremely common. The treatment of refractory overactive bladder conditions has changed radically over the last decade. The efficacy of OAB treatment protocols in improving patient symptoms are not satisfactory, so new agents such as botulinum-A toxin must be investigated.OBJECTIVE:To evaluate the safety and efficacy of intradetrusor botulinum -A toxin injection in patients with idiopathic detrusor overactivity resistant to anticholinergic drugs.METHODS:From March 2010 to September 2011, 38 women with refractory idiopathic detrusor overactivity (IDO), from the urologic consultation department in Baghdad Medical City and Al-Ramadi teaching hospital, were included in this prospective study. Their age ranged from 45 to 70 years. Inclusion criteria were women with idiopathic detrusor overactivity not responding to different anticholenergic treatments. Patients with urinary tract infection, mixed incontinence, bladder stone, hematuria, neurogenic detrosur overactivity, high postvoiding residual urine volume (more than 50 ml) and history of bladder tumour were excluded from this study. Full clinical and urodynamic evaluations were done before intradetrusor injection of 300 units of botulinum-A toxin and at 2 weeks and 24 weeks after injection.RESULTS:Thirty six patients achieved urinary continence (94.7%). The age ranged from 45 to 70 years (mean 56.67±7.34). There was significant clinical improvement after botulinum toxin injection at both 2 weeks and 24 weeks of follow up as reflected by statistically significant decrease in the daytime frequency, nocturia and the incontinence episodes (p value <0.05).There was significant urodynamic improvement after botulinum toxin injection at both 2 weeks and 24 weeks of follow up as reflected by statistically significant increase in the maximum cystometric bladder capacity and the bladder compliance and by statistically significant decrease in the detrusor pressure at maximum flow rate (p value <0.05). Postvoiding residual urinary volume shows statistically significant increase at both 2 weeks and 24 weeks of follow up (p value <0.05)CONCLUSION:The use of intradetrusor injection of botulinum toxin-A (BTX A) in refractory idiopathic detrusor overactivity was well tolerated and demonstrated clinically meaningful and statistically significant improvement in the clinical and urodynamic parameters of the patients. The effect was durable for the period of study which is 6 months

Abstract

ABSTRACT: INTRODUCTION:Overactive bladder is a symptom basedsyndrome and defined by the InternationalContinence Society (ICS) as urgency, with orwithout urgency incontinence, usually withfrequency and nocturia, if there is no proveninfection or other obvious pathology (1). * Department of Urology, Al-Karama TeachingHospital; Baghdad. ** Department of Gynecology and Obstetrics,Ibn Al-Baladi Hospital; Baghdad. BACKGROUND: Overactive bladder (OAB) is a symptom based syndrome. These symptoms include: urgency, withor without urgency incontinence, usually with frequency and nocturia with no proven infection orother pathology. It is encountered in women visiting urodynamic units as part of mixed urinaryincontinence (MUI). It greatly affects physical and social activities. Incontinence is often the keysymptom for the assessment of OAB symptoms and the decision to treat patients. Antimuscarinicdrugs are the main agents used to treat this condition. Oxybutynin (immediate-release) 5 mg orallyis the main available form in our clinical practice. OBJECTIVE: The objective of this study is to assess the efficacy of Oxybutynin immediate-release (OXY-IR) 5mg oral tablets, in the treatment of women with overactive bladder, i.e. reduction in episodes ofurge incontinence, total incontinence, and micturition frequency. MATERIALS AND METHODS: Twenty-five patients, age range from 18-75 years, diagnosed as having overactive bladdersymptoms were treated with Oxybutynin immediate-release (IR) 5 mg tablets 2-3 times daily for 8weeks period. Homma OAB symptom score system was used for assessment of treatment efficacy.Cystometry was done in all patients to look for detrusor overactivity. RESULTS:After Oxybutynin treatments, all the parameters in micturition patterns improved significantly:frequency, nocturia, urgency (p value < 0.005). Homma overactive bladder symptoms scores alsoimproved following the treatment (p value <0.001). Cystometric examination revealed the presenceof detrusor overactivity in 20 of the 25 (80%) patients at presentation. This was reduced to 4/25(16%) following treatment. About two-thirds (64%) of the patients reported good to great benefitfrom the treatment and had a positive effect on their life concerning physical and social activity,sleep and feeling of well-being. CONCLUSION: The selective antimuscarinic agent Oxybutynin (OXY-IR) 5 mg 2-3 times orally is effective intreating overactive bladder symptoms in these women by reducing incontinence attacks, urgency,micturition times and nocturia; consequently, it improves their life quality. We recommenddevising a score system for OAB suiting Iraqi patients

Abstract

Background: The concept of using β3-agonists in the treatment of overactive bladder with less side effects than the currently used anticholinergics, has been introduced recently. Objectives: The aim of this paper to report the presence of β3-AR in the ovine detrusor muscle. Methods: Appropriate sets of experiments using isolated ovine detrusor strips preparation. Cumulative effects of selected β-AR agonists including BRL37344 (BRL) were obtained on carbachol in presence and absence of propranolol and the β3-antagonist SR59230A, and EFS contracted strips. Results: All agonists tested produced concentration-dependent relaxation, the rank order of their relaxing potency in the ovine detrusor muscle was isoprenaline (ISO)>BRL> dobutamine > salbutamol. Both ISO and BRL significantly suppressed 5Hz EFS contraction at a concentration had no significant effect on 40Hz EFS contraction. Conclusion: It is concluded that β3-ARs are present abundantly in the ovine detrusor and the activation of which by the β-AR agonists suppresses the non-voiding detrusor contraction

الخلفية : مفهوم استخدام مثيرات ألبيتا-3 في علاج المثانة المتزايدة النشاط بتأثيرات جانبية اقل من الأدوية المستخدمة في الوقت الحاضر (مضادات الكولين). الأهداف : الهدف من هذا البحث هو لإثبات وجود مستلمات ألبيتا-3 في عضلة المثانة الضأنية , من خلال مجموعة من التجارب باستخدام عضلة المثانة الضأنية المعزولة . النتائج : تم الحصول على تأثيرات تراكمية لمثيرات منتخبة لمستلمات بيتا الأدرينالية بضمتها أل (BRL37344) على العضلة المتقلصة بواسطة الكاربيكول , بوجود وعدم وجود البروبرانولول و مثبط ألبيتا-3 (SR59230A) , و التحفيز بالمجال الكهربائي. جميع المثيرات المستخدمة أنتجت إرخاء عضلي معتمد على التركيز , وكان الترتيب التسلسلي حسب قوة إرخاء عضلة المثانة الضأنية كالأتي: ايسوبرينالين (أيسو) > BRL> دوبيوتامين > سالبيوتامول. أيسو و BRL كلاهما يخمد التقلصات المتسببة بواسطة التحفيز بالمجال الكهربائي 5 هرتز عند تركيز ليس له تأثير مهم على التقلصات المحفزة بواسطة 40 هرتز. الاستنتاجات : نستنتج من ذلك إن مستلمات ألبيتا-3 موجودة وبغزارة في عضلة المثانة الضأنية وتحفيز هذه المستلمات بواسطة مثيرات ألبيتا يؤدي إلى إيقاف التقلصات اللامفرغة للمثانة .
