Abstract

Back ground: Spectral analyses of solutions have long been applied to various body fluids for the purpose of clinical study as well as research .Human saliva can be easily obtained by non invasive .In this study typical spectra (for UV and IR) of saliva of oral cancer Squamous cell carcinoma patients were determined under average conditions and evaluated in relation to the spectra of normal specimens.Materials and Methods: Seventeen patients of oral cancer Squamous cell carcinoma and seventeen age matched healthy subject were included in this study .Chewing - Stimulated Saliva was collected in plastic test tube and stored at -20° C. Bach of saliva samples were used for UV and IR measurements.Results: Many differences between the IR spectrum of saliva of oral Squamous cell carcinoma patients and IR spectrum of normal saliva.Conclusion: The results in this study were detected that the use of IR spectroscopy may be useful in the diagnosis of oral Squamous cell carcinoma by using saliva samples.

تم تعيين أطياف الأشعة فوق البنفسجية (UV) والأشعة تحت الحمراء (IR) في هذه الدراسة لنماذج لعاب مرضى سرطان الفم Oral Squamous Cell Carcinoma ونماذج اللعاب للأصحاء. ومن ثم مقارنة النتائج لغرض الحصول على الخط الخاص لكل منها ومن ثم الإستفادة منه لإغراض الدراسة السريرية. عينت العديد من الفروق بين الأشعة تحت الحمراء (IR) للعاب مرضى سرطان الفم وطيف لعاب الأصحاء.أقرت النتائج في هذه الدراسة إمكانية إستعمال مطيافية IR لأغراض التشخيص لمرض سرطان الفم باستخدام عينات اللعاب

Abstract

Background: Tumor markers assist in the field of oral cancer. The aim of this study is to determine whether Mg and Ca ions could serve as tumor markers.Materials and methods: A total sample of 53 individuals was studied that includes 33 patients with oral cancer and 20 well-matching control. Estimation of Mg and Ca levels in the lymphocytes of healthy individuals and patients, in the normal and diseased tissues of patients were performed using atomic absorption spectrophotometer.Results: The values of both elements in the lymphocytes of patients were exchangeable according to the histopathological diagnosis. They were generally elevated in comparison to the control. (Ca = (3.983±3.214mean ±SD), Mg = (2.598±2.364 mean ±SD). Decrease levels of either elements in the diseased tissues observed in comparison to normal tissues in both sexes but it was more obvious in females than males (P<0.001 for Ca, P<0.002 for Mg).Conclusion: Patients with oral cancer are immune suppressed and immune disturbance could be the cause of the results found in this work.

Abstract

Background: Trace elements like iron, zinc, magnesium, cobalt and manganese are some of these elements involved in antioxidant defense mechanism. Many authors observed direct association between trace elements deficiency and the cancer mortality. The study was conducted to measure levels of Zinc (Zn), Copper (Cu), Iron (Fe), Manganese (Mn), Cobalt (Co), Nickel (Ni), Chromium (Cr), Cadmium (Cd), and Magnesium (Mg) in saliva of 50 patients with histologically proved squamous cell carcinoma.Patient and Method: Unstimulated (resting) saliva was collected from oral cancer patients one day before surgical excision of the tumor and one day after surgery using plastic polyethylene tubes. The collected saliva was cold centrifuged at 2500 rpm for 10 minutes at 0-5°C.The centrifuged supernatants were stored frozen at (-20°C) until time of analysis using Atomic Absorption Spectrophotometer.Results: Saliva levels of Zn, Mn, Co and Ni were significantly higher in preoperative saliva of oral cancer patients when compared to the normal control. On the other hand, a highly significant reduction in the levels of Cu, Fe and Mg was observed in preoperative saliva of oral cancer patients. However, no significant changes were seen in saliva trace element levels when preoperative values were compared with postoperative values of same patients.Conclusion: Saliva may be employed alone for trace elements measurements or it can be used supplementary to serum test for confirmation of any finding. However, saliva has the advantage of easy collection without trauma to the patients.

Abstract

The qualified subjects for this study included 33 patients with benign and malignant oral tumors aged 15-75 years and 31 matched age and gender healthy subjects used as control. Proteins measurements included total protein, albumin, globulines in sera and saliva samples, and immunoglobulins (IgG, IgM, IgA) in sera samples of control and patients. Meanwhile, polyacrylamide gel electrophoresis (PAGE) was used to differentiate between protein patterns in both serum and saliva samples among the studied groups. The gel was also stained for glycoprotein to evaluate as well the changes in glycoprotein contents. For total protein, the results revealed a signifigant increase (P≤0.01) in both samples (serum and saliva) of patient group. Albumin concentration shows presence of a high significant decrease (P=0.001) in sera samples but a significant increase (P<0.05) in saliva of the patients group in comparison to that of control group. Immunoglobulins concentrations also show significant increase in patients group in comparison to control group. This study aimed to estimate salivary and serum proteins in oral cancer patients and in healthy control group to evaluate their role in diagnosis and prognosis of oral cancer

تضمنت المجموعة قيد الدراسة 33 مريضا (ذكور و اناث) مصابين بأورام الفم (الحميدة و الخبيثة)و باعمار 15-75 سنة, قورنت مع مجموعة السيطرةالمتكونة من 31 من الاشخاص الاصحاء (ذكور و اناث) و بنفس مديات الاعمار. تضمنت القياسات لمجاميع المرضى و الاصحاء مستوى البروتين الكلي و الالبومين و الكلوبيولينات في نماذج المصل و اللعاب, بينما تم قياس مستوى بروتينات المناعة (IgA, IgG, IgM ) في نماذج المصل فقط. في الوقت نفسه استخدمت تقنية الترحيل الكهربائي باستخدام هلام البولي اكريلامايد لتمييز البروتينات في نماذج المصل واللعاب للمجاميع قيد الدراسة بالاضافة الى متابعة متغيرات محتوى البروتينات السكرية في نماذج المصل واللعاب. اظهرت النتائج ومقارنة بمجموعة السيطرة (الاصحاء) وجود ارتفاع معنوي (P≤0.01 ) في مستوى البروتين الكلي لنماذج المصل و اللعاب, بينما اظهرت نتائج تركيز الالبومين انخفاضا معنويا (P=0.001 ) في نماذج المصل, و ارتفاعا معنويا (P<0.05 ) في نماذج اللعاب . اظهر تركيز بروتينات المناعة ارتفاعا معنويا (P<0.01 ) في مصل مجموعة المرضى مقارنة بمجموعة السيطرة(الاصحاء). تهدف هذه الدراسة الى قياس البروتينات في مصل ولعاب مرضى سرطان الفم و مقارنتها مع مجموعة السيطرة (الاصحاء) لتقدير دورها في تشخيص و تطور سرطان الفم.

Abstract

Background: The incidence of oral cancers is increasing all over the world. Early detection ofthis important publichealth matter makes them more amenable to treatment and allows the greatest chance of cure.The aim of thisstudy was to investigate the awareness and knowledge on oral cancer among final -year dental students in Iraq.Materials and methods: Questionnaires were delivered to 160 final–year dental students in the College of Dentistry inBaghdad. The questionnaire focused on the awareness/knowledge of oral cancer, earlyand common clinical signsand symptoms andassociated risk factors.Results: It was found that 87% of students were aware of oral cancer. The followings were recognized as signs andsymptoms of oral cancer: persistent ulcer and lymphadenopathy (71 %, 70%), followed by presence of white patch(63%). A satisfactory knowledge was observed on; smoking (86%), family history (84%), old age (63%), sun lightexposure (59%), smokeless tobacco (57%), alcohol consumption (55%) and Immunosuppression (54%).Consideringdilatory risk-factors, a satisfactory knowledge was identified on hot and spicy food (57%), but not on the otherfactors. Although satisfactory knowledge of dental risk-factors including premalignant disorders (84%) and previoushistory of oral cancer (70%) was observed, inadequate knowledge about other factors was observed.91% of futuredentists indicated that oral cancer can be cured when detected early.Conclusion: This study highlighted the need for more education in dental schools to enhance dental professionals’awareness and knowledge of oral cancer, aiding in early diagnosis and treatment of patients

Abstract

This study aimed to identify the changes in total protein in saliva and sera samples of patients with oral squamous cell carcinoma in comparison to those of healthy controls. These changes were followed using electrophoresis (PAGE). Meanwhile, determinations of albumin, globulin and albumin to globulin ratio were carried out on sera samples only.Two groups were the participants in the present study, 18 patients with Oral Squamous Cell Carcinoma (OSCC), and 20 ages and gender matched healthy controls.

هدفت هذه الدراسة إلى التعرف على التغيرات في البروتين الكلي في عينات لعاب وأمصال المرضى المصابين بسرطان الخلايا الحرشفية في الفم (n = 18 ) وذلك بالمقارنة مع الاصحاء المتطابقة (n = 20 ). وتم متابعة تغيرات هذه البروتينات باستخدام تقنية الترحيل الكهربائي الهلامي ( PAGE) لعينات اللعاب والامصال المرضى مقارنة مع مجموعة الاصحاء و في الوقت نفسه تم متابعة الألبيومين والجلوبيولين و نسبة الألبيومين إلى الجلوبيولين على عينات الأمصال فقط . وأظهرت النتائج انخفاض غير معنوي (p > 0.05) في تركيز البروتين الكلي في عينات أمصال المرضى المصابين بسرطان الخلايا الحرشفية في الفم في حين كان هناك انخفاض معنوي (P < 0.05) في الالبومين في عينات أمصال المرضى و زيادة معنوية (P < 0.05) في عينات أمصال المرضى لتركيز الجلوبيولين. كما وجد انخفاض معنوي (P < 0.05) في نسبة الألبومين إلى الجلوبيولين في مجموعة المرضى مقارنة مع ذلك مع مجموعة الاصحاء.تضمنت الدراسة مجموعتين من المشاركين 18 مريضا بسرطان الخلايا الحرشفية (OSCC)، و 20 من الاشخاص الاصحاء من كلا الجنسين.

Abstract

Background: The early detection of oral cancers gives the greatest chance of curing and the delay in presentationhas a significant impact on the associated morbidity and mortality. The lack of general dental practitioners (GDP)knowledge in early cancer detection has been shown to contribute to delays in referral and treatment. The aim ofthis study was to investigate the oral cancer awareness among a group of GDP by assessing their knowledge ofdetection and prevention of oral cancer.Materials and methods: A questionnaire based study was designed. The questionnaire was delivered to 200 GDPassessing oral examination method, knowledge of oral cancer risk factors, clinical appearance and the mostcommon site, point of referral and requests for further information.Results: A response rate of 69% was obtained. The GDP were less likely to examine the oral mucosa than other similarstudies in other countries, and also less likely to advice patient about risk factors. Smoking and alcohol usewereidentified as risk factors by 96.3% and 43.48% of the GDP respectively. Only 37.7% of GDP believed they hadsufficient knowledge of oral cancer, and more than 94% believed they need further information about oral cancer.Conclusion: This study highlights need for more education of the GDP on oral cancer

Abstract

AbstractSquamous cell carcinoma is the most common malignant tumor of oral mucosa.Oral cancer is one of the prevalent cancers of the body and is the most common causeof the death, in Iraq little is written regarding the occurrence of oral cancer.Clinical details of 129 (80 males and 49 females) patients with oral squamous cellcarcinoma were collected from the Institute and Hospital of Radiotherapy AndNuclear Medicine Center in Baghdad during the period 2003-2006.The aim of thecurrent study is to find out the prevalence of oral cancer in different governorates ofIraqi patients in relation to gender, age and site.The highest prevalence was observed in tongue 72 (55.81%) while the lowestprevalence was found in the floor of the mouth 5 (3.87%).The age groups 51–60 years were highly affected by squamous cell carcinoma45(34.88%), while the lowest affected age groups were <10 years and 11-20 years 1(0.77%).Baghdad governorate had the highest prevalence of oral squamous cell carcinoma53(41.08%) .

Abstract

Background: Cancers of the oral cavity and oropharynx represents approximately three percent of all malignancies in men and two percent of all malignancies in women in the United State.Squamous cell carcinoma, which arises from the oral mucosal lining, accounts for 90%- 95% of oral cancer.Objective: aim of this study is to detect oral cancers in Sulaimania and to know what is the most common type and its commonest location in comparism to other studies.Patients&Methods: During the period from 1st Jan. 2004 to 30th June 2009, all histopathological reports in Sulaimani city with diagnosis of oral cancers for patients attended and performing biopsies for suspected oral lesions were included.These biopsies were received, processed and embedded in paraffin, sectioning, then stained by Haematoxylin and Eosin stain.All collected information were analyzed according to age, sex, site and histopathological diagnosis. Results: Oral cancers were diagnosed in 82 out of 665 oral lesions (represented as 12.33% from the total number of oral lesion biopsies).Squamous cell carcinoma (SCC) was the most common type of the malignant neoplasm of oral cavity found in 46 cases (56.1% ).Most frequent cases found in the age distribution range between 50 -70 years with M: F ratio 1.2:1. The most common affected site was the lower lip(39%) followed by the tongue(20%).Malignant neoplasms of salivary glands included 16 cases (19.6%),female 10, male 6 with F:M ratio is 1.66:1.Conclusion: The most common oral cancer in sulaimania is SCC of oral mucosa affecting mainly the old age group, and males affected more than females, and the most common location is the lower lip. The second most common cancer is salivary gland carcinoma and commonest histological type is adenocarcinoma.

Expression of Ki-67 and p53 in Oral Squamous Epithelial Abnormalities

Abstract

Objective: We intended to study the cellular proliferation by means of Ki-67 labeling index and the protein product overexpression of p53 in a series of oral intraepithelial lesions and squamous cell carcinoma, to evaluate the potential association between their histologic grades and the expression of the two markers. The expression of these markers on sections taken from hyperplastic and normal mucosal epithelium was also evaluated in order to determine whether the combination of p53 and Ki-67 over-expression could be used as a diagnostic aid in evaluating oral biopsies.Materials and Methods: Archival biopsy specimens from 35 patients, with oral squamous cell carcinoma (19) and intraepithelial lesions (16) were retrieved from the histopathology departments of Erbil and Duhok cities, Iraq, between January and December 2010. From the same patients, 13 blocks with hyperplastic epithelium and 7 with normal mucosal epithelium were also retrieved. The study was done in Duhok Central Laboratory, Duhok, Iraq. Ki-67 labeling index and p53 overexpression was determined by immunohistochemistry on paraffin sections, using avidin–biotin technique. Results: Overall, the staining patterns for Ki-67 antigen and p53 were similar. High Ki-67 and p53 overexpression was observed in 52.6% and 63.2% of carcinoma cases respectively, in 62.5% and 56.3% of intraepithelial lesions respectively, in 23.1% and 15.4% of hyperplastic epithelium respectively, and none in benign epithelium. Both high Ki-67- labeling index and p53 overexpression were significantly higher in carcinomas and intraepithelial lesions than in hyperplastic epithelium and were associated with the grade of both carcinomas and intraepithelial lesions. Conclusions: Combination of p53 and Ki-67 overexpressions can be used as a specific marker for oral lesions that are probably at high risk for malignant transformation, their immunohistochemistry emerges as a clinically useful supplement for histopathological assessment of grading of oral squamous cell carcinoma and intraepithelial lesions.

الهدف: دراسة الانتشار الخلوي عن طريق مؤشرki-67 و التعبير المفرط للجين p53 في سلسلة من الآفات داخل ظهاري الفم وسرطان الخلايا الحرشفية، لتقييم مدى ارتباط محتمل بين درجاتهم النسيجية والمؤشرات الخلوية. كما تم تقييم التعبير عن هذه العلامات على المقاطع المأخوذة من مخاطية المفرطة التصنع والعادي من أجل تحديد ما إذا كان يمكن استخدام مزيج من p53, ki- 67 والإفراط في التعبير كوسيلة مساعدة التشخيص في تقييم خزعات عن طريق الفم. مواد وطرق: تم استرجاع المحفوظات لعينات الخزعة من 35 مرضى. الذين يعانون من سرطان الخلايا الحرشفية عن طريق الفم (19) والآفات داخل الظهاري (16) من امختبرات التشريح المرضي من مدينتى أربيل ودهوك ، ما بين يناير وديسمبر 2010. من المريض نفسه، كما تم مراجعة 13 عينة من ظهارة المفرطة التصنع و 7 مع مخاطية طبيعية. ان الدراسة اجريت في المختبر دهوك المركزي ، دهوك، العراق. كان تقيم ki-67 مؤشر وضع العلامات و التعبير المفرط للجين p53 بالدراسة النسيجية المناعية على نمادج البارافين، وذلك باستخدام تقنية أفيدين-البيوتين.النتائج: وعموما، كانت أنماط تلطيخ للمستضدki-67 وجين p53 متماثلة. لوحظ ارتفاع ki-67وجين p53 التعبير المفرط للجين في 52.6٪ و 63.2٪ من حالات سرطان على التوالي، وفي 62.5٪ و 56.3٪ من الآفات داخل الظهاري على التوالي، وفي 23.1٪ و 15.4٪ على التوالي من ظهارة المفرطة التصنع، وليس في الظهارة الحميدة. كان كل من ارتفاع ki-67والتعبير المفرط للجين p53 أعلى بشكل ملحوظ في سرطان الخلايا الظهارية والآفات مما كانت عليه في ظهارة المفرطة التصنع وترافقت مع درجة من كل من سرطان الخلايا الظهارية والآفات.الاستنتاجات: يمكن استخدام مزيج من p53 و التعبير المفرط للجين ki-67كعلامة محددة للآفات الفم التي هي على الأرجح الأكثر عرضة للتحول للورم الخبيث. الدراسة النسيجية المناعية اظهرت اهميه من الناحية السريرية لتقييم المرضية في الدرجات لسرطان لخلايا الحرشفية للفم والخلايا الظهارية الآفات.
