Effects of Ramadan fasting on body weight and metabolic profile

Abstract

Background and objective: Fasting during the Ramadan month is a religious duty and it is obligatory for all healthy adult Muslims. The aim of this study was to investigate the effects of fasting during Ramadan in different times and place on body weight, blood sugar, serum lipids and blood pressure.Methods: Fifty three healthy adult fasting volunteers were included in the study. Peripheral venous blood samples were taken three days before Ramadan, at the end of the first week, and at the end of the fourth week of fasting. The last blood sample was taken one week after the end of Ramadan. Serum total cholesterol, HDL and LDL, triglycerides and glucose were measured. Vital signs and body mass index were taken by one of the researchers.Results: Thirty three (62.3%) volunteers were males, 93.4% were below 40 years of age. Weight changed significantly during Ramadan. Mean systolic blood pressure decreased by 11 mmHg while diastolic blood pressure decreased by 9 mmHg (P <0.001). Fasting blood sugar decreased by 14.96 mg/dl (P <0.001). Plasma lipids; cholesterol decreased by 19.3 mg/dl (P <0.027), LDL by 23 mg/dl (P <0.001), triglyceride by 44 mg/dl (P <0.003) but the HDL increased by 5.4 mg/dl (P <0.002) during Ramadan fasting. Before Ramadan, 19% of participants had abnormal serum cholesterol and 37% had abnormal LDL level, while after Ramadan all had normal lipid levels (P <0.001). The proportion of participants with abnormal HDL decreased from 94% before Ramadan to 50% after Ramadan (P <0.001).Conclusion: Ramadan affects the body physiology and lowers the weight, serum lipids, blood pressure and blood glucose levels.

Abstract

Background and objective: Cerebrovascular diseases are attacks of sudden neurological deficits (motor, sensory or cerebellar). There are a lot of risk factors for stroke like age, diabetes, hypertension, smoking, hyperlipidemia, cardiac diseases and others. This study aimed to show if there is increasing rate of cerebrovascular events during Ramadan in relation to fasting in our city during the summer season.Methods: This case-control study was carried out in Rizgary Teaching Hospital, Erbil- Iraq from 1st of August to 30th of August 2011. Patients were included in this study if they were middle aged and elderly patients having clinical and radiological features of stroke, another group of in-patients was selected as a control group. Chi square test and logistic regression analyses were used to show the association between stroke and fasting.Results: A sample of 60 patients and 60 control cases were included in this study. Fasting was significant risk factor for stroke in our studied sample, 66.7% of the cases were fasting compared with 40% of the control group (P = 0.03). Hyperlipidemia and history of ischemic heart disease found also to be associated with stroke (P = 0.017 and 0.011, respectively). Logistic regression analysis showed that only fasting and hypercholesterolemia were independent risks factors in causing stroke in our studied sample.Conclusion: In Erbil, where the summer is too hot and the daytime (fasting hours) is long, fasting during Ramadan was found to be an independent risk factor for stroke, and specifically ischemic stroke.

Abstract

Background: Knowledge about the clinical problems during the fast of Ramadan is important in order to opens the way to further research aimed at discovering the significance of Ramadan fasting in patients with heart disease.Patients and Methods: Eighty-six outpatients with heart disease with intention to fast were studied in the month of Ramadan 2010 (1430 H) at the Ibn Al-bitar Hospital. Detailed clinical and biochemical assessments were performed within 3 days before the start of Ramadan and then on the last day of Ramadan.Results: There were 54 (62.8%) males and 32 (37.2%) females with a mean age of 56.3 years (range, 17-84 ). Forty-six patients (53%) had coronary artery disease, 23 patients (27%) had valvular heart disease, 13 patients (15%) had congestive heart failure and 4 patients (5%) were treated for arrhythmia. Sixty-two patients (72%) were in New York Heart Association (NYHA) Class I, 18 patients (21%) in Class II, and 6 patients (7%) were in Class III. Seventy-four patients (86%) managed to fast during the entire Ramadan, 9 patients (10.4%) missed the fasting for up to 7 days, and 3 patients (3.5%) could not fast. There were no significant changes in the NYHA Class (p=0.12). No significant changes occurred in any of the hematological or biochemical parameters during the fasting of Ramadan.Conclusion: The effects of fasting during Ramadan on stable patients with cardiac disease are minimal. The majority of patients with stable cardiac disease can fast during Ramadan without significant detrimental effects.Key words: Ramadan, fasting, cardiac disease, biochemical effects

Abstract

ABSTRACT:BACKGROUND:Ramadan fasting is one of the pillaris of islam. The fasting time is about 12-19 hours depending on the season in which Ramadan falls and the geographic position of the country. It is often a subject of discussion nether or not Ramadan fasting confer any harmful effect on the body.OBJECTIVES:The aim of this study was to evaluate the effect of Ramadan fasting on some clinical, biochemical and Immunological aspects of the healthy and type two Diabetes mellitus patients.METHODS:The study was performed on (30) healthy fasting and (30) patients with type two diabetes mellitus in the month of Ramadan (2005). Blood samples were obtained on the second and fourth week's of Ramadan and were analyzed for fasting blood sugar, urea, uric acid, lipid profile, Immunoglobulins and complement component concentration, in addition, lymphocyte was separated to study the lymphocyte transformation assay in vitro compared with healthy non-fasting.RESULTS:There was slight elevation in the HDL-C, B. urea, S. Triglyceride, VLDL, S. uric acid and slight reduction in LDL-C at the end of fasting. There was slight reduction but the difference was non-significant (P>0.05) in the value of immunoglobulins (IgA, IgG, IgM) and (C3, C4) also, there was no difference in the sensitization rate of lymphocyte transformation in both healthy and type two diabetic patients compared with healthy non-fasting control.CONCLUSION:Slight elevation of HDL-C and slight elevation of B. urea, S. uric acid, S. Triglyceride, VLDL and there is non significant changes in Immunoglobulins and complement concentration and sensitization rate of lymphocyte transformation. Thus, Ramadan is safe for type two diabetic patients with the proper education of diabetic management.

Abstract

ABSTRACT:BACKGROUND:Fasting during the month of Ramadan is one of the five holy pillars in the Islamic faith and is an important annual ritual practiced by all Muslims. This involves fasting from dawn to dusk, followed by a break of fast at designated times. The length of fasting varies from ten to 19 hours, depending on the season in which the fasting month of Ramadan falls and the geographical locations.OBJECTIVE:1st to evaluate the relation between Ramadan fasting & duodenal ulcer perforations & 2nd to asses the risk factors of this complications .PATIENTS & METHODS :This is a prospective study in Baghdad Teaching Hospital – Medical City . Data from 231 consecutive patients with perforated duodenal ulcer from the period of October 2007 to December 2010 were collected & analyzed & all the patients were managed surgically after resuscitation in emergency unit .RESULTS :(96/231, 41.6%) of patients with perforated duodenal ulcer during Ramadan vs. (135/231, 58.4%) during the rest of the years . (96/490, 19.5%) of patients with perforated duodenal ulcer from the total number of all patients with acute abdomeninal surgeries during Ramadan. 43 % of patients smoking & 47 % of patients had a previous history of acid peptic disease in Ramadan. 54 % of patients were in between (20 to 40) years age group and the male to female ratio was 6 :1 . 15% patients of perforated duodenal ulcer had history of regular use of non steroidal anti inflammatory drugs.. Erect chest x-ray demonstrated gas under the diaphragm in 86% . Only 2 patients had a previous surgery of duodenal ulcer perforation .CONCLUSION:This study suggests that the incidence of duodenal ulcer perforation is relatively high in Ramadan among the people, who are fasting & have predisposing factors ( smoking , history of acid peptic disease , non steroidal anti-inflammatory drugs ) and need special precaution during this month

Abstract

Fasting during Ramadan is a religious duty for all healthy adult muslims, fasting is abstention from food and drink from sunset to down.The study was conducted on the undergraduate students and officers of the College of Medicine, College of Pharmacy and College of Engineering ,University of Tikrit.Twenty five apparently healthy students volunteered participated in the study. Their ages ranging from (18-50) years. Serum lipid profile were measured for this group.The investigations were done in the laboratories of Tikrit Teaching Hospital from Oct .to Nov.2004. Average duration of the fasting was about 12 hours and maximum ambient temperature ranged from 10-12 C. The study was conducted in three trails. The first trial during a period extending up to one week before Ramadan fasting, the second trail during the 7,8,9th days of Ramadan fasting, the third trail during the 7,8,9th days of Ramadan fasting ,the third trail during 27,28,29 th days of Ramadan fasting. During each trail five ml of blood was collected from vein by a disposable syringe and centrifuged (centrifuge 1500 r/min) and serum kept in deep freeze at -20C for biochemical analysis( lipid profile).The instrument used for the determination of lipid profile is the spectrophotometer .There was a non significant decrease in concentration of serum cholesterol at the beginning of Ramadan as compared to pre-ramadan values and there was a non significant increase in concentration of serum cholesterol at the end of Ramadan as compared to pre-ramadan values.There was a non significant decrease in concentration of serum triglycerides at the beginning of Ramadan as compared to pre-Ramadan values and there was a significant decrease in concentration of serum triglyceride at the end of Ramadan as compared to pre-Ramadan values.There was a non significant decrease in concentration of serum VLDL at the beginning of Ramadan as compared to pre-Ramadan values and there was a significant decrease in concentration of VLDL at the end of Ramadan as compared to pre-Ramadan values.There was a non significant decrease in concentration of serum LDL at the beginning of Ramadan as compared to pre-ramadan values and there was a significant increase in concentration of serum LDL at the end of Ramadan as compared to pre-Ramadan values.There was a significant decrease in concentration of serum HDL at the beginning of Ramadan as compared to pre-ramadan values and there was a significant decrease in concentration of serum HDL at the end of Ramadan as compared to pre-Ramadan values.There was a non significant decrease in body weight at the end of Ramadan as compared to pre-Ramadan values

الصوم في رمضان هو واجب ديني لكل المسلمين الاصحاء البالغين,الصوم يعني البقاء بدون اكل وشرب من شروق الشمس الى غروبها.الدراسة اجريت على طلاب وموظفي كلية الطب,كلية الهندسة وكلية الصيدلة في جامعة تكريت.شملت هذه الدراسة 25متطوعا وتم قياس مستوئ الكولسترول,الدهون الثلاثية,البروتينات الدهنية الواطئة الكثافة جدا,البروتينات الدهنية الواطئة الكثافة,البروتينات الدهنية العالية الكثافة,وزن الجسم.الدراسة اجريت في مختبر مستشفى تكريت التعليمي للفترة من شهر تشرين الاول الى شهر تشرين الثاني لسنة 2004.تم جمع العينات في ثلاث زيارات,الزيارة الاولى كانت قبل رمضان باسبوع,الزيارة الثانية كانت خلال الايام السابع والثامن والتاسع من رمضان,والزيارة الثالثة كانت خلال الايام السابع والعشرين والثامن والعشرين والتاسع والعشرين من رمضان.حيث قمنا بدراسة اثر الصيام في رمضان على مستوى الدهون والبروتين الدهني في الدم وعلى وزن الجسم.في هذه الدراسة كان هناك انخفاض غير معنوي في تركيز الكولسترول في بداية رمضان من 5.08(mmol/l) الى 4.94(mmol/l)بالمقارنة الى ما قبل رمضان وكان هناك زيادة معنوية في تركيز الكولسترول في نهاية رمضان رمضان من 5.08(mmol/l) الى 5.52(mmol/l) بالمقارنة الى ما قبل رمضان.كان هناك انخفاض غير معنوي في تركيز الدهون الثلاثية في بداية رمضان من 1.91(mmol/l) الى 1.80(mmol/l) بالمقارنة الى ما قبل رمضان وكان هناك انخفاض معنوي في تركيز الدهون الثلاثية في نهاية رمضان من 1.91 الى 1.76(mmol/l) بالمقارنه الى ما قبل رمضان.كان هناك انخفاض غير معنوي في تركيز البروتينات الدهنية الواطئة الكثافة جدا في بداية رمضان من 0.88(mmol/l) الى 0.82(mmol/l) بالمقارنة الى ما قبل رمضان وكان هناك انخفاض معنوي في تركيز البروتينات الدهنية الواطئة الكثافة جدا في نهاية رمضان من 0.88(mmol/l) الىبالمقارنة 0.81(mmol/l) الى ما قبل رمضان.كان هناك انخفاض غير معنوي في وزن الجسم في نهاية رمضان بالمقارنة الى ما قبل رمضان.

Abstract

Background: Ramadan fasting exposes diabetic patients to glycemic exertion (mainly post prandial hyperglycemia in the night) and to metabolic dysregulation. Objectives: To evaluate the effect of Ramadan fasting on metabolic parameters of type 2 diabetic patients.Patients & Method: Twenty eight type 2 diabetic patients on oral antidiabetic drugs attending National Diabetes Center ( NDC ) at October 2006 were investigated before and during Ramadan fasting ( 1st, 2nd, 4th, week )of fasting for the following parameters ( FPG; PPG; Lipid profile, urea and HbA1c).changes in the dose of medication were done during fasting. Results: During Ramadan fasting the FPG increased to 173.4±25.4mg/dl and PPG show no changes at 183.3±43.5mg/dl. While HbA1c increased at the end of Ramadan to8.95±1.55%.Both Total cholesterol and LDL- c level decreased to the 218.2±53.9mg/dl and 121.5±19.9 mg/dl respectively.TG elevated non significantly and HDL–c level increased at end of Ramadan.Conclusion: There is an improvement in most of metabolic control during Ramadan. Fasting Ramadan for type 2 diabetic patients will not expose them to an improper metabolic imbalance especially if they were followed up, education about diet and self monitoring were done.Key words: Ramadan fasting; FPG; PPG; TC; LDL-c; HDL-c; TG.

مقدمة البحث: إن صيام شهر رمضان يعرض مرضى السكري إلى اختلاف في السيطرة على مستوى السكر ( خصوصاً ارتفاع في مستوى السكري بعد فترة الافطار ليلاً ) وأيضا يؤثر على عدم انتظام العمليات الايضية في الجسم والتي تتفاقم بعد انتهاء شهر رمضان.هدف البحث: لدراسة تأثير فترة صيام شهر رمضان على مؤثرات العمليات الايضية لدى مرض السكري نمط 2.طريقة البحث : تمت الدراسة على مرض السكري نمط 2 الذين يراجعون المركز الوطني للسكري خلال شهر تشرين أول 2006 وتمت دراسة (28) مريض مصاب بداء السكري نمط 2 يعالجون بواسطة خافضات السكر الفموية وتم اخذ عينات من الدم قبل شهر رمضان وأعيد فحص عينات أخرى من الدم من الأسبوع الأول والثاني والرابع من شهر رمضان. وأجريت تغيرات ضرورية على جرعة ووقت تناول العلاج خلال شهر رمضان و أجريت فحوصات مستوى الدهون والخضاب السكري قبل شهر رمضان وفي نهاية الشهر.النتائـــج: إن مستوى السكر في الدم قبل الافطار خلال صيام شهر رمضان يرتفع بينما مستوى السكر بعد الافطار يبدي عدم تغيير في المستوى بينما مستوى الخضاب السكري يزداد في نهاية فترة الصيام. كما إن مستوى الدهون من دهون الكولسترول واطئة الكثافة تبدي انخفاض ملحوظ بينما هناك زيادة في مستوى دهون عالية الكثافة في نهاية شهر رمضان.الاستنتاج: هناك تحسن في اغلب مؤشرات العمليات الايضية خلال فترة صيام شهر رمضان. إن صيام شهر رمضان عند المصابين بداء السكري نمط 2لا يعرضهم إلى تغيرات في توازن العمليات الايضية إذا كانت هناك متابعة مستمرة وتثقيف حول طبيعة ونوع الطعام خلال فترة الصيام والمتابعة الذاتية من قبل المريض.

Abstract

Ramadan, the 9th month of Islamic calendar ranging for 29-30 days in duration, during which Muslims are fasting . It was demonstrated that most of the asthmatics Muslim in our regions did not consider asthma to be a problem to Ramadan, and they pass in fasting by arranging their treatment as use the controller inhaler on Iftar and Sohoor time & stop taking the reliever inhaler in the day time. the Aim is to study the effect of Ramadan fasting on the clinical, spirometric figures of patients with stable asthma with treatment modification . Methods: A prospective cohort study was performed before, during and after Ramadan fasting on well-controlled asthmatic patients as they were classified according to the treatment types as:group 1: on inhaled budesonide + formoterol fixed single device (symbicort, Astra Zeneca, sewed) + on need short acting B adrenergic agonist. group 2 : short acting B adrenergic agonist on needed alonegroup 3: interleukins antagonists tablets Once daily (10 mg montelukast Na tablet) + short acting B adrenergic agonist on needed. Results found that fasting seen to have detrimental effects on fasting in hot weather as FEV1, FEF in 50% and asthma control test score show statistically significant differences before & after fasting but patient on inhaled symbicort with treatment modification show better outcome.

Abstract

Aim: This clinical study is to investigate the effect of fasting (Sawm) Ramadan on thecount of streptococcus mutans in plaque surrounding fixed orthodontic appliances.Materials and Methods: Fifteen patients with orthodontic appliances wereparticipated in this study. Plaque specimens were collected from those patients attwo occasions, during Rmadan fasting (Sawm) and after they break there fastingafter Ramadan. The plaque Streptococcus mutans number of colony-formingunits (CFU) was analyzed by using Dentocult SM Strip mutans test.Results: This study demonstrated highly significant difference in Streptococcusmutans (CFU) counts in plaque between the values obtained from fasting patientsduring Ramadan and when they are breaking the fast after Ramadan.Conclusions: during Ramadan fasting, persons change timing and types of their foodand beverages consuming as well as teeth cleaning habits. Therefore orthodontistshould be aware of their patients’ oral hygiene during Ramadan by promoting andmaintaining satisfactory teeth cleaning during fasting (Sawm) and Iftar hours.
