Abstract

Background: It has been accepted that in patients with CSOM, conductive deafness results from mechanical break-down of sound conduction ,whilist the cochlea remains unaffected.Objectives: Our study aims to prove a sensorineural component in hearing loss in patients suffering from CSOM , which necissates early detection as it increases in severity with time.Methods: A prospective study was done on 64 patients, all were diagnosed as having unilateral CSOM, their ages ranged from 18-40 years to exclude presbyacusis as a cause of sensorineural deafness.These patients were divided into 4 groups according to the duration of discharge and further divided into 3 groups according to the pathology (simple perforation, granulation tissue or cholesteatoma).Results: 62 patients showed some degree of sensorineural deafness in the affected ear accounting for 96.8% of the total number of patients,While only 2 patients escaped the phenomenon. Worsening of bone conduction thresholds was noticed with increased duration of the disease.The highest loss was in patients with severe pathology (cholesteatoma) and least in those with simple perforation.Conclusion: This study indicates that the degree of sensorineural hearing loss in patients with CSOM relates with the duration and complications of the disease which is contrary to the traditionally accepted belief of the physicians that cochlea remains unaffected in CSOM.Key words: Cochlea, sensorineural deafness, CSOM

Abstract

Background: Patients with chronic kidney disease have different grades of sensorineural deafness . Objective: To study the incidence of sensorineural hearing loss and possible contributing factors in patients with chronic kidney disease. Methods: A total of 100 patients with chronic kidney disease were studied. All of them were males. 92 of them were on regular haemodialysis programme. Only 8 patients were on conservative management the age range of the study patients was 18-40 year patients were divided into three groups according to age. All patients were assessed clinically and were evaluated by audiometry , and analysis was made on bone conduction threshold .The mean follow up period was 28 weeks . Results: 36 patients (36 %) showed sensorineural hearing loss .The incidence of sensorineural deafness was found to increase with the advancing age and duration of chronic kidney disease but not directly proportional to the number of haemodialysis sessions .The number of haemodialysis sessions did not show increase in the degree of sensorineural deafness . Conclusion: Patients with chronic kidney disease have sensorineural deafness of some degree which should be assessed and evaluated to halt its progression.

Abstract

BackgroundChronic suppurative otitis media is a common disease affecting young age group, it is typically a persistent disease, often capable of causing severe destruction and irreversible sequelae and clinically manifest with deafness and discharge.ObjectivesTo evaluate the degree of sensorineural hearing loss component in patients with chronic suppurative otitis media with and without pathological sequelae, which necessitate early detection as it increase in severity with time.MethodsA prospective study of 140 patients with unilateral chronic suppurative otitis media was evaluated otologically and audiologically for bone conduction hearing loss in the diseased ear throughout one year. The patients mean age was 26.5±0.5 years ranging from 10-45 years and the mean duration of the illness were 6.9±2.5 years ranging from 1-30 years.ResultsEighty-eight and a half percent of the patients with unilateral chronic suppurative otitis media had some loss of cochlear function.ConclusionMost patients had a difference of less than 10 db in bone conduction between diseased and uninvolved ears. The range difference between diseased and uninvolved ears was wider at the higher frequencies, related mainly to the duration of the disease and its pathological sequelae such as cholesteatoma and or granulation tissue.Key wordSensorineural, otitis media, round window, cholesteatoma

Abstract

Objectives : A prospective study conducted in Basra General Hospital from April 2008 to March 2009 to find the cause - effect relationship between chronic suppurative otitis media and sensorineural hearing loss . Method : Hearing thresholds of hundred patients with chronic suppurative otitis media were determined using Pure Tone Audiometry. All subjects were less than 40 years old (to exclude Age related hearing loss) , unilateral cases selected (so as the contralateral healthy ear served as a control) . All of them presented criteria of evaluation which exclude the other possible causes of sensorineural hearing loss like : chronic medical illnesses , otological surgeries , hereditary causes , exposure to noise and exposure to systemic ototoxic drugs . Results : The average loss in bone conduction threshold was determined between the affected and healthy ear in relation to the disease duration and in relation to the extent of the pathology . Conclusion : We found that the longer the duration of the disease and the more complicated the pathology, the more the loss in bone conduction . This loss is mostly at the higher frequencies .

Abstract

Background: The auditory system is a potential target of the damaging effect of diabetes mellitus. Objective: To assess the clinical impact of diabetes mellitus on hearing. Patients and Methods: This is a case control study in which we assess the hearing of 60 diabetic patients of both types (type 1 and type 2 diabetes) regarding (age, gender, type, duration and control) of diabetes mellitus and compare it with 60 normal (age and gender match) control group. Thorough history and full ENT examination done for both diabetic patients and control group to exclude those with other causes of sensorineural hearing loss. Pure tone audiometry were done for all patients and normal control group and (5cc) of blood were taken from diabetic patients for assessment of HbA1c level.Results: This study shows that there is significant increase (p value <0.00004) of prevalence sensorineural hearing loss among diabetic patient compared with normal (age and gender match) control group. The sensorineural hearing loss was mostly for high frequency (P value at 4000Hz was ˂0.0001 which is significant), and the loss is proportionate with the duration of diabetes and is more in uncontrolled diabetes than controlled group(according to their HbA1c level) and the P value was 0.0082 (significant). There is no significant effect of type of diabetes mellitus on hearing loss (P value was 0.4526 which is not significant). Conclusion: High frequency sensorineural hearing impairment is one of the complications of diabetes mellitus.Recommendation: regular hearing assessment being a part of follow up program of diabetic patients.

الهدف : لتقييم تاثيرالسكري على السمع الحسي العصبيالمنهجية : هذه الدراسة هي دراسة مقطعية من خلالها أجرينا تقييم سمعي ل 60 من مرضى السكري من كلا نوعي السكري (نوع1 ونوع2) للفترة من حزيران 2012 إلى كانون الثاني 2013 شملت مراجعي عيادة السكري والعيادة الخارجية للأذن والأنف والحنجرة في مدينة الاماميين الكاظميين (ع) وقد تم مقارنتهم بأقرانهم من الأشخاص الغير مصابين بالسكري الذين يما ثلونهم في الأعمار والأجناس. خضع كل المرضى بالسكري لفحص شامل وأخذ تاريخ مرضي دقيق وكذلك فحص الأشخاص الغير مصابين بالسكري لاستبعاد أولائك الذين لديهم أسباب أخرى لفقدان السمع مثل ( تأريخ عائلي لفقدان السمع المبكر أو تعرض لضجيج شديد أو أدوية معروفة بسميتها للجهاز السمعي وكذلك لاكتشاف أي عارض أو علامة لمرض آخر في الأذن).تم اجراء تخطيط سمعي لكل مرضى السكري والأشخاص الغير مصابين بالسكري وكذلك أخذت عينة من دم كلاهما لقياس نسبة السكر التراكمي في الدمHbA1c) ).النتائج : هذه الدراسة بينت بأن هناك زيادة مهمة في انتشار فقدان السمع الحسي العصبي بين مرضى السكري عند مقارنتهم مع أقرانهم الغير مصابين بالسكري (P value يساوي اقل من 0.00004 أي انه ذو أهمية) وان فقدان السمع غالبا كان للترددات العالية حيث ان ال (P value كان 0.0794 عند التردد 500Hz أي انه ليس ذو أهمية) وكان (عند التردد 4000Hz اقل من 0.0001 أي انه ذو أهمية).كذلك فان الدراسة بينت بان فقدان السمع يتناسب طرديا مع مدة تواجدالمرض وانه أكثر في المرضى الغير مسيطرين على نسبة السكر في الدم بصورة جيدة وذلك عن طريق قياس نسبة HbA1c . كذلك أوضحت هذه الدراسة بأنه لا يوجد تأثير مهم لنوع داء السكري(نوع1 او نوع2) على فقدان السمع الحسي العصبي.الاستنتاجات : فقدان السمع الحسي العصبي في الترددات العليا هو احد مضاعفات داء السكري.التوصيات : الفحوصات الدورية المنتظمة للعتبة السمعية (pure tone audiometry) هي جزء من متطلبات متابعة مرضى السكري.

Abstract

Background: Sensorineural hearing loss is a common sequel of chronic suppurative otitis media and the round window membrane is currently being considered as a major route for noxious agent to pass from the middle ear cavity to the cochlea.Objective: To evaluate the incidence and severity of sensorineural hearing loss in patients with chronic suppurative otitis media.Methods: Prospective study on 93 patients from January 2010 to December 2014 in the AL-Karama Teaching Hospital.The patients were complaining of unilateral chronic suppurative otitis media they were grouped according to the pathology to active, inactive mucosal disease and active, inactive squamous epithelial chronic otitis media, based on clinical otoscopical examination. Audiological studies were done in Specialized Surgery Hospital/ Science Center of Otorhinolaryngology Hearing and Speech Unit. Result: Forty-four percent of the patients with unilatral chronic suppurative otitis media had some loss of cochlear function. The range of difference between diseased and uninvolved ears was wider at the higher frequencies and related to the activity and duration of the disease.Conclusion: Chronic suppurative otitis media may associated with mixed rather than purely conductive hearing loss.

الخلفية: ان الصمم العصبي الناتج من مضاعفات الاصابة بالتهاب الاذن الوسطى المزمن، ودخول المواد السامة من الاذن الوسطى الى الاذن الداخلية (القوقعة). الهدف من الدراسة: تقييم النسبة وشدة الصمم العصبي في المرضى المصابين بالتهاب الاذن الوسطى المزمن.طريقة البحث: أجريت هذه الدراسة على 93 مريضا في مستشفى الكرامة التعليمي للفترة من شهر كانون الثاني 2010 الى كانون الاول 2014, المرضى مصابين بالتهاب الاذن الوسطى المزمن وحيد الجانب وتم تقسيم المجاميع حسب نوع المرض، اجري تخطيط السمع في مركز علوم الانف والاذن والحنجرة -وحدة السمع والتخاطب في بغداد. النتائج: كانت نسبة حدوث الصمم العصبي 44% وان هناك علاقة وثيقة بين الصمم العصبي وحدوثه في الترددات العالية والفترة الزمنية للمرض.
