ROLE OF VISFATIN IN THE PATHOGENESIS OF GESTATIONAL DIABETES MELLITUS

Abstract

Background:The recently discovered adipocytokine visfatin has insulin-like properties. It lowers blood glucose and improves insulin sensitivity; however, clinical data on visfatin are limited.Objective:To evaluate the role of visfatin in GDM (gestational diabetes mellitus), we determined visfatin levels in women with GDM and healthy pregnant women.Methods:A total of 60 women were evaluated: 30 women with gestational diabetes mellitus and 30 healthy pregnant women to serve as control subjects. Serum visfatin concentrations were analyzed using an enzyme-linked immunosorbent assay the study was done in Al-Yarmouk Teaching Hospital during the period from November 2010 to March 2011.Results:Serum visfatin concentrations were significantly lower in the gestational diabetes mellitus group (0.27±0.1 ng/ml) than in the healthy control group (1.37±0.25ng/ml) (P=0.0001). Conclusions:Our results show that there are decreased concentrations of serum visfatin in gestational diabetes mellitus subjects and this may indicate that visfatin plays a role in the pathogenesis of gestational diabetes mellitus .However; further experiments are needed to clarify this role.Key Words:Visfatin, Gestational Diabetes Mellitus

Abstract

Background: Visfatin is a novel adipokine that mainly secreted by visceral adipose tissue, had an important role ininflammation and immune system. Creatine Kinase (CK) which is an enzyme that is involved in energy metabolism,found in large amounts in myocardium, brain and skeletal tissues.This study is carried out To evaluate the periodontal health status of the study groups (chronic periodontitis andchronic periodontitis with coronary atherosclerosis) and control groups, to measure the salivary levels of visfatin andCreatine Kinase in these groups and compare between them, and to determine the correlations between salivaryvisfatin and Creatine Kinase levels with the periodontal parameters in the three groups.Materials and Methods: eighty participants, males and females were recruited in this study with age ranged from (30-60) years, they were divided into three groups: the first study group was the Chronic periodontitis group (n=30), thesecond study group was chronic periodontitis and coronary atherosclerosis (n=30) and the control group(n=20)which was healthy systemically with healthy periodontium. Periodontal health status was determined by measuringplaque index(PLI),gingival index (GI), probing pocket depth(PPD), bleeding on probing (BOP) and clinicalattachment level (CAL),salivary samples were taken from each participants, salivary visfatin levels were determinedby enzyme-linked immune-sorbent assay(ELISA) while the activity of salivary Creatine Kinase was determinedspectrometrically by using the International Federation of the Clinical Chemistry (IFCC) method on Hitachi 911Automatic analyzer.Results: The results of the study showed that the mean values of PLI, GI, visfatin, Creatine Kinase and the percentagesof sites according to PPD scores, CAL scores, BOP were higher in the second study group with chronic periodontitisand coronary atherosclerosis than in the other groups with highly significant differences between the groups at(P≤0.01). Also by using Pearson Correlation Coefficient, salivary visfatin levels were correlated positively with allclinical periodontal parameters with a strong and positive correlation between salivary visfatin levels and CAL scoresand PPD scores. Salivary Creatine Kinase levels were correlated positively with all clinical periodontal parameters witha strong and positive correlation between its levels and mean values of GI and percentages of BOP.Conclusion: The present study showed that salivary visfatin can be used as a marker for the development ofcoronary atherosclerosis and its levels are associated with the degree of periodontal destruction and showed thatCreatine Kinase may be used as a marker for coronary atherosclerosis and chronic periodontitis.

Abstract

Background: Obesity is highly associated with insulin resistance and increased risk of type 2 diabetes and cardiovascular disease. Visfatin is adipocytokines that is highly expressed in visceral fat. Its expression in visceral fat is hyperglycemia in insulin deficient diabetes. Objective: To study the visfatin level in type 2 diabetes mellitus patients and its relation with obesity and insulin resistance.Patients and methods: This study was done in national diabetes center(NDC),AL -Mustansria University and AL –Yarmok Hospital Teaching ; on a total (120) individuals age range from (33-60) years. ( 80) patient type 2 diabetes nephropathy, (40) healthy controls . The enrolled patients were divided to three groups according to BMI ,data collection about age, sex ,WHR, BMI and blood samples to estimate serum visfatin levels, fasting plasma glucose(FPG), glycated hemoglobin(HbA1c), lipid profile (TC,TG,HDL-C,and LDL-C), fasting C-Peptide ,and fasting insulin level Furthermore insulin resistance parameters calculated from Homeostasis Model Assessment 2- insulin Resistance were calculated using HOMA2-Calculator software downloaded freely .Results: The mean FPG,HbA1c, C-Peptide level, fasting insulin level ,HOMA2IR and visfatin level show statistically significantly increased in diabetic patients when compared with control subjects. Serum visfatin level was significantly increased with BMI increased groups. Visfatin level was increased in HOMA2-IR =>3 when compared with HOMA2-IR <3 in diabetic patients. Visfatin level show significant positive coorelation with BMI, WHR,FPG, HbA1c, fasting insulin level , HOMA2-IR , TC,TG,and LDL-C in diabetic patients when compared with control subjects ,and negative correlation between serum visfatin level and HDL-C in diabetic patients when compared with control subjects . Conclusion: Obesity and Insulin resistance play an important role in the pathophysiology of type 2 diabetes mellitus. Elevated visfatin level in diabetic patients with obesity features of insulin resistance are believed to result in distance clinical characteristics and complications such as atherosclerosis.

Abstract

Background:Visfatin is adipokine originally discovered as a pre- B-cell colony enhancing factor, is expressed by amniotic epithelium,cytotrophoblast,and deciduas and is over expressed when fetal membrane are exposed to mechanical stress or inflammatory stimuli.Objective: To determine whether preterm labour is associated with changes in maternal plasma circulating C-reactive protein (CRP) and visfatin concentrations.Study design: This is a case control study.Setting: The department of Gynecology and Obstetrics in the college of medicine in Kufa University.Materials and Method: The patients included in this study were divided into three groups control group who are preterm not in labor(50) and cases group who are preterm in labor(50) and full term group in labor(50). Parameters include age, gestational age, general urine examination, liver function test, renal function test, white blood count, hemoglobin, c-reactive protein (CRP), and visfatin.Results: There is a significant increment in the level of C-reactive protein and visfatin between cases and control groups .Conclusion: The visfatin and c-reactive protein may be involved in the pathogenesis of preterm labor.

Abstract

Psoriasis is a common inflammatory autoimmune skin disease that affects 2–4% of the world population. T-cells infiltration, interleukins and cytokines including adipocytokines like visfatin, high molecular weight- adiponectin (HMW-adiponectin) and retinol binding protein-4 (RBP-4), together with oxidized low density lipoprotein (ox-LDL) have been implicated in psoriasis pathogenesis. A total of 86 subjects (43 with Psoriasis and 43 healthy controls) were enrolled in this study. Their mean age was (41± 14.37) for patients, and (35.81± 18.66) for the control group. The study conducted in dermatology outpatient clinic in Merjan Teaching Hospital in Al –Hilla City and investigations were done in laboratory units of the hospital through the period from June 2013 to September 2014. The sera obtained from blood were used to determine the level of serum visfatin, HMW-adiponectin, RBP-4 and oxidized-LDL by enzyme-linked immunosorbent assay (ELISA). Assessment of psoriasis disease activity and methotrexate efficacy was done by psoriasis area and severity index (PASI) score.Results of this study showed a significant increase (p<0.05) in serum visfatin, HMW- adiponectin, and oxidized-LDL concentrations, and a significant decrease (p<0.05) in retinol binding protein-4 (RBP-4) concentrations in sera of patients with chronic plaque psoriasis when compared with control group. There was a significant correlation between oxidized-LDL and visfatin, HMW-adiponectin and retinol binding protein-4 (r = 0.81, 0.68, -0.77) (p value < 0.001) respectively .Finally, the impact of systemic methotrexate on above adipocytokines and oxidized-LDL was determined by significant reduction in the levels of visfatin, HMW-adiponectin and ox-LDL and a significant increase in levels of RBP – 4 after treatment and by a significant mean difference in PASI score before and after treatment in psoriatic lesions. Results obtained by this study indicate that some adipocytokines including serum visfatin, HMW–adiponectin and RBP-4 levels, in addition to oxidized-LDL measured in patients with chronic plaque psoriasis were closely associated with disease severity and could be used to assess the successful treatment.To the best of our knowledge, this is the first study in Iraq to evaluate serum levels of these adipokines and Ox – LDL in patients with psoriasis and study the effect of methotrexate treatment on these parameters.

Abstract

ABSTRACT:BACKGROUND: Thyroid hormones have a crucial physiological role to maintain balance of metabolism of body. Deficiency of iron can produce hypothyroidism. Hypothyroidism causes several lipid metabolism disorders. Visfatin and leptin are adipocytokines and have been suggested as important regulators in patients with thyroid dysfunction.OBJECTIVE: The aim of the present study was to estimate the circulating levels of lipids, iron, visfatin, and leptin in newly diagnosed hypothyroid patients. PATIENTS AND METHODS: The current study was conducted in medical city teaching hospital on 45 newly diagnosed cases of hypothyroidism aged 35-50 years and compared with 45 healthy in¬dividuals as control group during the period from June 2016 to January 2017. This study investigated the serum concentrations of lipid profile, some iron status parameters and hematological parameters, total- and free-, triiodothyronine, tetraiodothyronine, thyroid stimulating hormone, visfatin, and leptin in hypothyroid patients and compared them with the controls.RESULTS: There was a significant increased in serum total cholesterol and triacylglycerol in hypothyroid patients compared to the control, (P=0.001). There was a significant decreased in hemoglobin, serum iron, and ferritin while a significant increased in total iron binding capacity in hypothyroid patients compared to the control, (P≤0.05). Means value of thyroid stimulating hormone levels were significantly increased in hypothyroid patients compared to the control group. On the other hand, the levels of total- and free-, tetraiodothyronine, free triiodothyronine were significantly decreased in hypothyroid patients as compared to the control group. There was a significant increased in visfatin and leptin concentrations in hypothyroid patients compared to the control group, (P≤0.05).CONCLUSION: These data suggest that thyroid hormone insufficiency may lead to deficiency of iron and lipids abnormalities. Thus, estimation of iron and related parameters with lipids may be quite useful during the diagnosis and treatment of hypothyroid patients. Also, alterations in thyroid status change serum visfatin and leptin in hypothyroid patients. It is advisable to routinely investigate them for early detection allowing its early management.

Abstract

Background: Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathies in women of the reproductive age group. Objective: Thestudy aimed to assess the relation of serum visfatin level in women with polycystic ovarian syndrome. Materials and Methods: A cross‑sectionalstudy was carried out in Kirkuk city from January 10, 2018, to June 15, 2018. The number of polycystic ovary syndrome (PCOS) women under studywas 60 women whose ages were between 15 and 45 years old. These patients admitted to obstetrics and gynecology unit at Azadi teaching hospital.The control group who were matched to the patients studied included 30 individuals. Results: The study showed that there is the significant differencebetween PCOS women and the control group concerning visfatin level and the highest mean of visfatin was occurred in PCOS women (13.60 ± 7.98vs. 7.73 ± 1.22). There was significant relation between body mass index and visfatin level among PCOS women in this study. The highest rate ofPCOS women had irregular menstrual cycle. The study showed that most PCOS women included in the study had hirsutism. The

Abstract

Background and objective: Women with polycystic ovary syndrome serve as a model to estimate the functions of insulin resistance and chronic hyperinsulinemia on serum level of insulin resistance markers. This study aimed to determine the association of the serum visfatin, resistin levels with insulin resistance in patients with polycystic ovary syndrome, and its correlations with various parameters.Methods: This case-control study was performed at the College of Pharmacy, Hawler Medical University in the period between May 2017 and July 2018. It included 100 patients and 100 age matched healthy control group was also enrolled for comparing purposes.Results: Patients had significantly elevated serum visfatin ( 2.46 vs. 1.08 ng/ml), resistin (3.87 vs. 2.46ng/ml), and insulin (17.22 vs. 8.1 µlU/ml) levels than controls. They also had significant elevations in insulin resistance (2.41 vs. 1.92) and β insulin function (94.45 vs. 86.98). There was no significant variation in insulin sensitivity (88.77 vs. 92.08). Conclusion: There was an association between resistin, visfatin, and polycystic ovary syndrome-related insulin resistance. Many recent proteins are now considered as potential new markers of insulin resistance in polycystic ovary syndrome. There was a strong connection between visfatin and resistin with insulin resistance in polycystic ovary syndrome pathophysiology.

Abstract

Background: Acute myocardial infarction continues to be a major health concern. It contributes to morbidity and may end fatally. Objective: To evaluate Visfatin, apelin melatonin, and interleukin-6 levels in patients with acute myocardial. Patients and Methods: The study included 30 AMI patients (group :I)and 30 as healthy control group(group: II). Visfatin, apelin melatonin, interleukin-6 levels were assessed. Results: Significantly higher levels of serum apelin, IL-6, cholesterol, and triglyceride, while significantly lower levels of serum melatonin, visfatin , and HDL were observed in the cases than in the controls. Receiver operator characteristic curve investigation shown the levels of melatonin and apelin are the best biomarkers differentiating subjects with AMI.(AUC=0.997; 95% CI: 0.933 to 1.000, cutoff value ≤ pg/ml) ], (AUC =0.952; 95% CI: 0.863 to 0.990, cutoff value >3.21 pg/ml respectively) ]. Conclusion: This study shows a significantly increase in apelin, IL-6 while a significantly decrease melatonin, visfatin in the circulation of G:I as compared with G:II. The results obtained in the present study indicate that serum melatonin; apelin might play an important pathogenic role not only in the occurrence but also in the severity of AMI. The circulating level of melatonin provides highly specific biomarker for AMI more than apelin

خلفية الدراسة: احتشاء عضلة القلب الحاد لا يزال مصدر قلق للصحة. وهو يساهم في الاعتلال ويمكن أن ينتهي بالموت. اهداف الدراسة: هدفت هذه الدراسة لتقييم فيسفاتين، الميلاتونين أبيلين، ومستويات انترلوكين -6 في المرضى الذين يعانون من احتشاء عضلة القلب الحاد.المرضى والطرائق : شملت الدراسة 30 مريضا من المرضى الذين يعانون من احتشاء عضلة القلب الحاد (المجموعة: I) و 30 كمجموعة السيطرة (المجموعة: إي). فيسفاتين، الميلاتونين أبيلين، تم تقييم مستويات انترلوكين -6.النتائج: اظهرت النتائج زياده معنويه في مستويات أبيلين، انترلوكين --6، الكوليسترول، والدهون الثلاثية في المصل ، في حين لوحظت نقصان معنوي في مستويات الميلاتونين المصل، و فيسفاتين، و أظهر ROC تحقيق منحنى مميزة مستويات الميلاتونين وأبيلين هي أفضل المؤشرات الحيوية في المجموعة الأولى بالمقارنة مع المجموعة الثانية. الاستنتاجات : أظهرت هذه الدراسة زيادة كبيرة في أبيلين، ، انترلوكين -6 في حين انخفاضا كبيرا الميلاتونين، فيسفاتين في المجموعة الأولى بالمقارنة مع المجموعة الثانية: إي. النتائج التي تم الحصول عليها في هذه الدراسة تشير إلى الميلاتونين وأبيلين قد تلعب دورا هاما المسببة للأمراض ليس فقط في حدوث ولكن أيضا في شدة المرض.
