Hypomagnesemia in Iraqi Diabetic Patients and Healthy Controls: An Exploratory study

Abstract

BACKGROUND: Diabetes mellitus has been suggested to be the most common metabolic disorder associated with magnesium deficiency, and because available data suggest that adverse outcomes are associated with hypomagnesemia, it is prudent that routine surveillance for hypomagnesemia be done and the condition be treated whenever possible.AIM OF THE STUDY:To explore the serum Mg concentrations of diabetic patients and healthy controls in our locality.MEHTODS: One hundred and forty four diabetic patients (22 with type I and 122 with type II diabetes mellitus) recruited from the outpatient diabetes clinic at the Specialized Center For Endocrine Diseases-Baghdad (62 patients), National Diabetes Center-Al Mustansiria University (20 patients), and from private endocrinologic practice clinics in Baghdad (62 patients), during the period from 1st October 2005 to 30th April 2006. Ninety non-diabetic healthy controls matched for age and sex were participated in this study. Exclusion criteria for both groups included diarrhea and loop diuretics. None were taking Mg supplements. Level of Mg and Albumin were determined spectrophotometerically in the same serum samples.RESULTS:Mean serum Mg concentrations of the diabetics was significantly lower than in controls (p<0.001). Serum albumin was not a significant predictor of serum Mg neither among diabetic patients (r = 0.005) nor control subjects (r = 0.139). In 88.9% of the diabetic patients and 11.1% of the control subjects serum Mg concentrations were below the normal reference range of 0.70 mmol/L. The prevalence is increased when hypomagnesemia is defined by a reference limit of 0.75 mmol/L (98.6% and 28.9 % for diabetics and controls, respectively), a further higher prevalence among controls (91.1 %) was noted when 0.80 mmol/L is adopted as the lower normal limit. All diabetics (100%) and controls (100%) were identified as hypomagnesemic with a lower reference limit of 0.90 mmol/L.CONCLUSION:The tremendous hot climate of ours, can be suggested as an influential cause for increased Mg losses, and may provide a coherent explanation for the exceedingly high prevalence of hypomagnesemia observed in diabetic and control subjects participated in this study. Which therefore may suggest an inevitable requirement for magnesium supplementation to avert hypomagnesemia, not only among diabetics, but as well for controls, particularly through the hot summer episode

Abstract

ABSTRACT:BACKGROUND:Chronic renal failure is defined as an irreversible deterioration in renal function which classicallydevelops over a period of months or years. renal transplantation is the treatment of choice for endstage renal failure and during surgery the maintenance and restoration of intravascular volume areessential tasks to achieve sufficient organ function in renal transplants.OBJECTIVE:To study the comparison between normal saline 0.9% with glucose 5 % and normal saline 0.9% ,glucose 5 % with albumin in renal transplantation.METHODS:The study data from 50 patients 47 patients underwent living donor kidney transplants three wereexcluded because they are diabetics .twenty three patients received normal saline 0.9% , glucosesaline 5%,and twenty four received normal saline 0.9 with glucose 5% and albumin 100 milliliter .RESULT:There was no statistical difference between the two groups in the primary measurement of outcomemeasure in the urine output in the 1st day and the serum creatinine in the 1stand 3rd day .CONCLUSION:Although statistically there was no difference in the two groups, it might be its useful to combine acolloid with a crystalloid in the fluid management regimen to improve microcirculation, oxygenperfusion and to ovoid large volumes of crystalloid.

Abstract

This study was designated to evaluate some external and internal characteristics of quail eggs reared in Basrah city. Ten mature female quails were used in the present study. All birds were reared under similar environmental, managerial and hygienic conditions. To assess the egg quality parameters, a total of 100 eggs were randomly collected for 20 days.The values related to egg weight, shape index, shell weight, shell ratio , shell thickness, egg length, Egg width, Shell index and shell surface area (cm2) are found respectively as 7.041, 79.590, 0.973, 14.193, 0.307, 2.825, 2.245, 13.044 and 8.205. The internal traits as albumen index, albumen height, albumen weight, albumen ratio, albumen width, Albumen length, albumin average, yolk diameter, yolk height, yolk weight, yolk index, yolk ratio and Yolk : Albumen ratio are found respectively as 12.056, 3.204, 2.5, 35.515, 2.308, 3.058, 2.683, 2.195, 3.983, 2.591, 17.601, 36.746 and 1.120.

صممت هذه التجربة لتقييم بعض المواصفات الخارجية والداخلية لبيض طائر السمان المربى في محافظة البصرة. عشر من إناث السمان استخدمت في هذه التجربة. جميع الطيور خضعت لظروف متشابهة في الأجواء والرعاية والتربية. مائة بيضة جمعت عشوائيا خلال عشرون يوما لإجراء الاختبار. سجلت القياسات الخارجية كوزن البيض, دليل الحجم , وزن قشرة البيض, نسبة القشرة , سمك القشرة, طول البيضة, عرض البيضة, دليل القشرة والمساحة السطحية للقشرة (سم2) فكانت كالأتي: 7.041, 79.590, 0.973, 14.193, 0.307, 2.825, 2.245, 13.044و .8.205 أما القياسات الداخلية مثل دليل الألبومين, ارتفاع الألبومين, وزن الألبومين, نسبة الألبومين, عرض الألبومين, طول الألبومين, معدل الألبومين, قطر الصفار (المح), ارتفاع الصفار, وزن الصفار, دليل الصفار, نسبة الصفار ونسبة الصفار- الألبومين فكانت 12.056 ،3.204، 2.5، 35.515،2.308 ، 3.058،2.683، 2.195، 3.983، 2.591، 17.601،36.746 و 1.120.

Abstract

background: Burn patients are unique in that not only do they reduce albuminsynthesis in response to trauma, but also they experience accelerated albuminlosses into their wound dressings and soft tissue.Methods: Serum albumin has been determined in 26 children with age rangedbetween (l-14) years and with total body surface area of burn (11-60%) during theirinitial few weeks of care. Their serum albumin levels were estimated at second day,seventh day and fourteenth day after burn. Intravenous albumin was administered byinfusion of 1-2 g/kg/d when plasma Albumin fell bellow 2 g/100ml. or bellows 2.5g/100ml in the presence of enteral feeding intolerance or pulmonary dysfunction,their further results were excluded.Results: The mean plasma Albumin was 3.062 g/100ml) at day 2 , 2.952 g/100mlat day 7 and 2.45 g/100ml at day 14 . The relationship between the surface area ofburn and albumin concentration was not significant, (r = - 0.1111406). The valueof albumin was found to be 0.752 g/dl under the normal average value (p< 0.005).Conclusion: Hypoalbuminemia is usual in pediatric burned patients especiallyafter 7 -14 day of the accident, the period of high risk of septic complications.Keywords: Serum albumin, Hypoalbuminemia, Burn

Abstract

A total of 114 confirmed cases of visceral leishmaniasis which is diagnosed by bone marrow aspiration at Basrah and Thi-Qar children hospitals were described, most of them are less than two years old. The majority of the patients suffered from leukopenia (97%) with increase in lymphocyte and monocyte numbers. Common biochemical features of the patients bloods show an increase in bilirubin. Liver enzymes (ALT and ATS) increasing in severity cases.

تم وصف 114 حالة طفل مصاب بالحمى السوداء DAT والمؤكد تشخيصهم بواسطة فحص نخاع العظم. وجد إن 97% من الأطفال ينخفض لديهم معدل عدد كريات الدم البيض مع وجود زيادة في المونوسايت والخلايا اللمفية مقارنة بالعينات الضابطة. لوحظ في النتائج أيضا زيادة في بعض المؤشرات الكيموحياتية كما في ارتفاع البليروبين، كما أظهرت الدراسة زيادة في بعض إنزيمات الكبد في بعض حالات الإصابة الشديدة.

Abstract

ABSTRACT:BACKGROUND:Hypoalbuminemia is considered as poor prognostic factor in patients with certain chronic diseases, such as cancer and end-stage renal failure. Low serum albumin is common in patients with heart failure; nevertheless, the relationship between serum albumin and heart failure prognosis has not been well verified.OBJECTIVE:To elucidate the effect of serum albumin level on prognosis of patients with systolic heart failure.METHODS:This study included 250 patients with systolic heart failure who were admitted to Baghdad teaching hospital between February 2008 and April 2010. Patients were divided into groups based on presence of hypoalbuminemia (≤3.4 g/dL).RESULTS:The mean age of patients was 55 ± 14 years and the mean left ventricular ejection fraction [LVEF] was 28 ± 11 %). The mean serum albumin was 3.9 ± 0.7 g/dL; 27.2 % of patients had hypoalbuminemia. Patients with and without low serum albumin levels were similar in age, cause of heart failure, and ejection fraction. Patients with hypoalbuminemia had higher New York Heart Association (NYHA) class, higher serum urea, creatinine and C-reactive protein levels but lower levels of sodium, hemoglobin, and cholesterol. In patients with body mass index (BMI) < 25 kg/m2, 26% had hypoalbuminemia compared to 20% in those with BMI ≥25 kg/m2 (P ≤ 0.01). One-year survival was 64.71% in patients with and 85.72% in those without hypoalbuminemia (P < 0.001). Risk-adjusted hazard ratio for 1-year mortality was 1.9 (1.5-2.4).CONCLUSION:Hypoalbuminemia is common in patients with heart failure and is independently associated with increased mortality.

Abstract

Abstract:Objectives: to study the impact of increased thyroid secretion on levels of serum cholesterol and albumin in serum women.Methodology: a study in applied AL-Sader educational hospital for the period from March 2011 to August 2011 including the study the measuring of concentration of cholesterol and albumin in serum blood of (53) women patients of hyperthyroidism. The researcher used statistical analysis of data.Results: the search results showed significant decrease in the rate of albumin concentration compared to control group (p < 0.001). And a decrease in cholesterol concentration compared with the control group (p < 0.001).Recommendations: the study recommends compensation decrease blood cholesterol and with increased thyroid secretion by addressing the complementary and healthy food appropriate for each situation.

الخلاصة:الأهداف : لمعرفة تأثير زيادة إفراز الغدة الدرقية على مستويات الكولسترول والألبومين في مصل النساء.منهجية البحث : أجريت دراسة تطبيقية في مستشفى الصدر التعليمي للفترة من آذار 2011 إلى آب 2011 حيث شملت الدراسة قياس تركيز الألبومين والكولسترول في مصل دم(53) من النساء المريضات بمرض فرط الغدة الدرقية. واستخدم الباحث التحليل الإحصائي للبيانات المأخوذة.النتائج :أظهرت نتائج البحث نقصان معنوي في معدل ألألبومين مقارنة مع مجموعة السيطرة (p<0.001). ونقصان معنوي في معدل الكولسترول مقارنة مع مجموعة السيطرة(p<0.001) .التوصيات : توصي الدراسة بتعويض نقصان الألبومين والكولسترول في الدم ممن لديهم زيادة إفراز الغدة الدرقية بتناول المتممات الغذائية والغذاء الصحي المناسب لكل حالة.

Abstract

Objectives: to study the impact of decreased thyroid secretion on level of serum cholesterol and albumin in serum women.Methodology: a study in applied AL-sader educational hospital for the period from April 2013 to August 2013 including the study the measuring of concentration of cholesterol and albumin in serum blood of (65) women patient of hypothyroidism .the researcher of used statistical analysis of data.Results: the search results showed significant increased in the rate of cholesterol compared to control group (p<0.05) and non-significant in the rate of albumin compared to control group (p<0.05). Recommendation: the study recommends to decreased the high rate of cholesterol with patient of hypothyroidism by taking thyroxin to erect the thyroid gland to normal action. Conclusion: thyroid hormones regulated serum cholesterol and albumin. The decreased serum thyroxin is attributable to the presence of an abnormal human serum cholesterol.

الهدف :لمعرفة تأثير نقصان إفراز الغدة الدرقية على مستوى الكولسترول وألالبومين في مصل النساء.المنهجية:أجريت دراسة تطبيقية في مستشفى الصدر التعليمي للفترة من نيسان 2013 إلى أب 2013 حيث شملت الدراسة قياس تركيز الألبومين والكولسترول في مصل دم (65) من النساء المريضات بمرض نقصان إفراز الغدة الدرقية. واستخدم الباحث التحليل الإحصائي للبيانات المأخوذة.النتائج:أظهرت نتائج البحث زيادة معنوية في معدل الكولسترول مقارنة مع مجموعة السيطرة (P<0.05 ) ولم تظهر أي فرق معنوي في معدل الألبومين مع مجموعة السيطرة P<0.05) ).الاستنتاجات:هرمونات الغدة الدرقية تؤثر على معدل الكولسترول والألبومين حيث ان نقصان الثايروكسين يؤدي الى تغيير معدل الكولسترول في الانسان.التوصيات:توصي الدراسة بتقليل نسبة الكولسترول في الدم ممن لديهم نقص إفراز الغدة الدرقية بواسطة استخدام الثايروكسين للمرضى بنقص إفراز الغدة الدرقية لإرجاع الغدة الدرقية للعمل بشكل طبيعي.

Abstract

Background: Cherry extract has a wide range of biological activity. In the present investigation, two oral doses 0.04&0.08 mg/kg/day of cherry stick (ethanol extract) in albino male mice resulted in a significant decrease in concentration of serum total cholesterol concentration, triglyceride and total protein, albumin and globulin.
Objective: we investigated the impact of biological activities of cherry stick extract on serum cholesterol, triglyceride, total protein, albumin and globulin in white albino male mice.
Design: This study include three groups of white albino male mice. The first group (G1) was a control group comprised of 5 animals. The second group (G2) consisted of 5 animals treated with 1ml of cherry stick extract at concentration of 0.04 mg/kg/day for two weeks after treatment, while the third group (G3) consists of 5 animals treated with 1ml of cherry stick extract at concentration of 0.08 mg/kg/day for two weeks with treated animals.
Results: there was a statistically significant reducing effect on serum total cholesterol, triglyceride, total protein and albumin P<0.001 at 0.04 mg/kg/day and 0.08 mg/kg/day for all biochemical parameters. Globulin was markedly decreased P< 0.01 in comparison with control but no significant change in ratio of albumin to globulin.Conclusion: Extract of cherry stick exerts at low dose a remarkable effect on serum cholesterol, triglyceride and total protein.
