BCL2 and P53 immunoexpression in colorectal carcinoma

Abstract

Background and objective: The products of BCL2 and P53 genes are involved in the regulation of proliferation and apoptosis and have been associated with prognosis in several malignancies, including colorectal carcinoma. This study aimed to investigate the expression of antiapoptotic BCL2 protein in colorectal carcinoma and to examine its association with one of the important mediators of apoptosis (P53 protein) and with clinicopathological factors, and to assess the prognostic value of the combined BCL2 /P53 phenotypes by studying their relation with the two most important prognostic factors of grading and staging.Methods: A retrospective and prospective study was carried out from the period of August 2010 to June 2011. Sixty eight formalin-fixed, paraffin-embedded blocks of colorectal carcinoma cases were evaluated regarding BCL2 expression, P53 nuclear accumulation and their concomitant expression using immunohistochemistry by Dako Cytomation.LSAB + System-HRP.Results: The study shows that 29.41% of the colorectal tumors were positive for BCL2 protein expression and associated with earlier stage tumors (P = 0.021), while 72.06% were positive for P53 protein expression and associated with later stage tumors (P = 0.006) and male gender (P = 0.005). There was a trend toward an inverse correlation between BCL2 and P53 expression (P = 0.0013). Tumors that did not express detectable levels of BCL2 but exhibited nuclear accumulation of P53 were most common and associated with later stage tumors (P = 0.003).Conclusion: Concomitant assessment of both BCL2 and P53 status may be valuable in predicting the aggressiveness of tumors in patients with colorectal carcinomas.

Abstract

This study was conducted to estimate the expression of BCL2 oncoprotein in human colorectal carcinoma and to show its possible correlation to the clinic-pathological parameters. 50 patients of colorectal carcinoma studied in Diwania teaching hospital , out of 50 cases there were only 14 cases positive for bcl2 expression, (10%) of them with strong positive staining pattern, (12%) of them with moderate positive and (6%) with weak positive. Grade I showed (66.7) strong positive staining pattern, while Grade III showed (100%) weak positive staining pattern, also this study showed high expression of BCL2 in stage A,B than stage C,D which were with negative result. البحث عن التعبير المناعي النسيجي الكيميائي لجين أل bcl-2 كمؤشر للتغييرات الطارئة في الخلايا السرطانية لأورام القولون والمستقيم. تمت دراسة50عينة من سرطان القولون في مختبرات مستشفى الديوانية التعليمي. أظهرت الدراسة وجود ترابط في التعبير النسيجي لجين أل- bcl-2 مع مجموعات تمايز المرض ودرجة انتشاره وكان التعبير أكثر في العينات ذات التمايز القوي (الدرجة الأولى) والمجموعة غير المنتشرة ضمن جدار القولون والمستقيم عنه في العينات ذات التمايز الضعيف (الدرجة الثالثة) والمنتشرة خارج جدار القولون والمستقيم مما يدل على أن لهذا ألجين دورا اساسيآ في تقيم سرطان القولون والمستقيم المستقبلية وانتشاره وتحديد أهمية استجابته للعلاج.

Apoptosis and Necrosis Levels in Chemotherapy Treated Chronic Lymphocytic Leukemia Patients

Abstract

The results showed that CLL patients chemotherapy treatment increase the expression of Bcl2 gene (via measuring the phosphatidylserine -PS level) which increased apoptosis and necrosis percentage. It is also showed that the Necrotic cells (%) were found to be significantly higher in patients samples (1.07 ± 0.03) than in healthy (0.17 ± 0.02) with no significance in viable cells (%) (82.88% in patients and 86.8% in healthy). The apoptotic cells % were found to be significantly higher in patients (15.04 ± 1.03) than healthy (4.40 ± 0.5). On the other hand, late apoptosis % showed no significant results (0.639 % in patients and 0.803 % in healthy).

بينت النتائج أن المعالجة الكيميائية لمرضى ابيضاض الدم المزمن أدت الى زيادة تعبير المورث و ذلك من خلال قياس مستوى الفوسفوتايدل سيرين و الذي أدى الى زيادة نسبة الاستماتة و التنخر. بينت النتائج أيضا أن نسبة التنخر كانت عالية معنويا عند مجموعة المرضى (1.07 ± 0.03) مقارنة مع الاصحاء(0.17 ± 0.02) وعدم وجود فرق معنوي بينهما في حيوية الخلايا 86.8%) و 82.88% على التوالي ) . كما أن نسبة الخلايا المستماتة كانت عالية معنويا ايضا عند المرضى مقارنة (15.04 ± 1.03) مع الاصحاء(4.40 ± 0.5) بينما كانت نسبة الاستماتة المتأخرة غير معنوية بينهما ( 0.803 % و 0.639 % على التوالي ) .

Abstract

The results of the IHC showed there was positive expression for bcl2 in the control group with high score (++) high and this expression will decrease with the increase of the dose of the PC extract in the treated group .And the results of the ultrastructural examination by using TEM showed there was increase in the N/C ratio, well developed ER, increase number of variably sized mitochondria in the control group while the treated group revealed there was many vacuoles in the cytoplasm , plasma membrane blebbing and formation the apoptotic bodies. From our study we concluded that there was clear effect on the ultrastructural after using PC as anti- tumor.

وأظهرت نتائج IHC كان هناك تعبير إيجابي لbcl2 في السيطرة على المجموعة مع درجة عالية (++) مرتفعة وهذا التعبير ستنخفض مع زيادة جرعة من مستخلص الكمبيوتر في المجموعة المعالجة. ونتائج التركيبية وأظهر الفحص باستخدام تيم كان هناك زيادة في ن نسبة / C، ER متطورة، وزيادة عدد الميتوكوندريا الحجم بنسب مختلفة في المجموعة الضابطة في حين كشفت المجموعة التي تلقت العلاج هناك العديد من الفجوات في السيتوبلازم، blebbing غشاء البلازما وتشكيل الهيئات أفكارك . من دراستنا استنتجنا أن هناك تأثير واضح على التركيبية بعد استخدام الكمبيوتر كما المضادة للورم.

Abstract

ABSTRACT:BACKGROUND:Carriage of Helicobacter Pylori in the human stomach is associated with increased risk of peptic ulcerdisease, distal gastric adenocarcinoma and gastric B-cell mucosa associated lymphoid tissue lymphoma.OBJECTIVE:To study the immunohistochemical expression of bcl-2, as apoptosis makers in the gastric mucosa ofpatients infected with cagA Helicobacter Pylori demonstrated by insitu hybridization method.PATIENTS MATERIALS AND METHODS:Gastric antrum biopsies from 99 patients presented with dyspeptic symptoms (50 men, 49 women,median age 40) were analysed for the presence of H. pylori, and were classified according to updatedSydney system. Insitu hybridization technique was done to detect cagA H. pylori.Immunohistochemical expression of bcl-2 using (Avidin- Biotin method) was performed on paraffinembedded biopsy specimens.RESULTS:Forty four patients (44.44%) had H. pylori cagA positive starin. Atrophy of gastric mucosa was presentin 14 (14.14 %) patients. Intestinal metaplasia was present in 8 (8.08%) patients. The frequency ofatrophy and intestinal metaplasia were significantly higher in cagA H. pylori gastritis than non-cagA H.pylori gastritis (p=0.023 and 0.041respectively). Bcl2 expression was not significantly higher in H.pylori gastritis than non-H. pylori gastritis (p= 0.101). Bcl2 expression was significantly higher in thepresence of atrophy (p<0.001). Bcl2 expression was significantly higher in the presence of intestinalmetaplasia (p<0.001).CONCLUSION:The rate of apoptosis decreases when lesions (gastric atrophy and intestinal metaplasia) are present.

Abstract

Background: Colorectal cancer is a major cause of morbidity and mortality worldwide. Prognostic assessment influences the treatment of patients with colorectal cancer, including decisions about adjuvant therapy. Bcl2 over-expression is a genetic event associated with tumor progression and is a prognostic marker of this disease.Objective: Colorectal carcinoma is a major cause of morbidity and mortality worldwide. Bcl2 overexpression is a genetic event associated with tumor progression and is a prognostic marker for this disease. The aim of this study is to assess the expression of bcl2 in colorectal carcinoma and its correlation with other clinicopathological parameters.Methods: From January 2004- January 2005, thirty –five formalin fixed paraffin embedded tissue samples from patients with colorectal carcinoma were included in this study. Four-micrometer tissue sections were obtained for each case, two of them were stained by H&E and the diagnosis had been revised, and the other two were stained immunohistochemically by using avidin biotin alkaline phosphatase method for evaluating bcl2 expression. The presence of red cytoplasmic staining in less than 25% of tumor cells was considered a positive expression of bcl2.Statistical analysis of all the results were performed using Chi square test at level of significance alpha = 0.05 (P<0.05) regarded as statistically significant.Results: Bcl2 expression was significantly higher in low grade and early stage colorectal carcinoma .Non mucinous colorectal cancer showed more bcl2 expression than the mucinous type. An inverse correlation was found between bcl2 expression with the greatest diameter of the tumor and the lymph node status. Bcl2 expression was correlated neither with the age nor with the sex of the patient and the tumor location.Conclusion: Bcl2 over-expression correlates with many variables as low grade colorectal tumor, early stage, non mucinous type, small tumor size and negative lymph node status.

خلفية الدراسة: يعد سرطان القولون والمستقيم سبب رئيسي للفنائية والمرضية في جميع انحاء العالم . ان التقييم التكهني يؤثر على علاج سرطان القولون والمستقيم بضمنها القرارات حول العلاج المساعد .يعد التعبير المناعي للـ bcl2 هو حدث جيني مرتبط بتقدم الورم وهو مؤشر تكهني للمرض.هدف الدراسة: دراسة التعبير المناعي للـ bcl2 في سرطان القولون والمستقيم وبيان مدى علاقته بالعوامل المرضية والسريرية للورم.المواد وطرق العمل: للفترة من كانون الثاني 2004 – كانون الثاني 2005 تم اختيار 35 مقطع شمعي لمرضى مصابين بسرطان القولون استعاديا تغطي الفترة من كانون الثاني 2004 الى كانون الثاني 2005 وتم تحضير اربعة شرائح نسيجية بسمك 4 مايكروميتر, اثنان منها صبغت بصبغة الهيماتوكسيلين والايوسين وتم اعادة فحصها لغرض تشخيصها وتصنيفها نسيجيا والاثنان الاخران صبغت بطريقة التصبيغ الكيميائي النسيجي المناعي للكشف عن الـ bcl2 . ان وجود التفاعل السايتوبلازمي الاحمر في المقطع اعتبر تفاعلا ايجابيا . بدمج الشدة والكمية للصبغة المناعية فان الصبغة السايتوبلازمية الضعيفة مع كمية من الخلايا الورمية اقل من 25 % اعتبر ايجابيا للتعبير المناعي للـ bcl2 .تم التحليل الاحصائي لكل النتائج بأستعمال Chi square واعتبر ت قيمة P اقل من (0,05) كقيمة ذات مغزى او دلالة احصائيا.النتائــج: ان التعبير المناعي للـ bcl2 في الدرجة الدنيا كان ذا مغزى احصائيا . وجد ان تعبير ال bcl2 كان اكثر في الاورام عالية التمايز النسيجي (او ذات الدرجة الدنيا لتصنيف المرض ) وفي المراحل المبكرة للورم . اظهر سرطان القولون والمستقيم غير المخاطي الـ bcl2 اكثر من النوع المخاطي وكان هذا ذو قيمة او دلالة احصائيا. هناك علاقة معكوسة وجدت بين التعبير المناعي للـ bcl2 مع اكبر قطر للورم وحالة العقد اللمفاوية . ان التعبير المناعي للـbcl2 غيرمرتبط بعمر وجنس المريض وموقع الورم.الاستنتاج: ان ظهور بروتين bcl2 في سرطان القولون والمستقيم يترافق مع بعض عوامل المرض كالدرجة الدنيا لتصنيف المرض , المراحل المبكرة للمرض , النوع غير المخاطي لسرطان القولون والمستقيم , صغر حجم الورم والحالة السلبية للعقد اللمفاوية .

Abstract

background: Bcl2 is known to belong to a family of apoptosis regulatory geneproducts that may be death antagonist eg (bcl- 2, bcl-xl , mcl –l) or deathagonist ex (bax , bak , bcl -xs , bad) , The bcl- 2 oncoprotein inhibits apoptosisand is expressed by many tumors including carcinoma such as breast ,cervix and headand neck .Aims to identify bcl-2 oncogene product expression of oral carcinoma and itscorrelation to clinical features (exophytic , endophytic and mixed type) of oralcarcinomaPatients and methods Twenty four patients were gathered , 20 patients hadsquamous cell carcinoma and four patients with adenoid cystic carcinoma . Paraffinblocks were obtained of the study group , specimens of follicular lymphoma paraffinblocks were obtained from pathology department at teaching laboratory of medicalcity . Normal oral mucosa were obtained from healthy patients who had undergoneroutine oral surgery following informed consent of the patients .Immunohistochemical procedure of Bcl2 protein were applied .Results our study showed that 11 cases of the exophytic type of oral carcinomarepresented a positive bcl2 expression , 4 cases of the endophytic (ulcerative type)showed a positive immunoreactivity with bcl2 antibodies and 3 cases of mixed type(exophytic type and endophytic) showed a positive bcl2 expression . Statisticalanalysis showed non significant relationship between clinical features (exophytic ,endophytic , Mixed type) and bcl2 expression in oral carcinoma .Conclusion :- bcl-2 expression in normal oral mucosa and oral carcinoma ,bcl-2immunoreactivity was more correlated to mixed type , exophytic and rarely toendophytic oral carcinoma but all didn't reach a statistical significance .Keywords ,anti apoptotic gene product , bcl2 expression , oral carcinoma.

Association of t (14; 18) Chromosomal Translocation to Hodgkin’s Lymphoma in Iraqi Patients Using Multiplex PCR Technique

Abstract

Translocation t (14; 18) (q32;q21) is a common cytogenetic abnormality in B- cell lymphoma, especially in non-Hodgkin’s lymphoma, but it is rarely seen in Hodgkin’s lymphoma (HL). In the current study, t(14; 18) chromosomal translocation was detected in Hodgkin’s lymphoma of Iraqi patients by multiplex PCR with multiple primers and direct sequence by Sanger method with Agencourt® CleanSEQ® dye-terminator Removal Kit. DNA was extracted from 50 samples of paraffin- embedded tissue, 25 samples of blood by using QIAamp DNA Mini Kit (Qiagen /Germany) and 10 samples from reactive hyperplasia as control. The t (14, 18) was detected in 26(35%) cases that showed IGH/BCL2 gene rearrangement with a significant difference compared to control (P<0.01) or hyperplasia. The detected breakpoints represent as 5(19.2%) for major breakpoint MBR1, 4(15.38%) for MBR2 and 3(11.5%); 2(8%); 7(27%) for 3ʹMBR1, 3ʹMBR2 and 3ʹMBR4 respectively. Break points of MCR1, MCR2, 5ʹMCR were detected as 4(15.38%); 1(4%) and 2(8%)) respectively. On the other hand, no breakpoint was detected for 3ʹMBR3. Thus, this study reported an increased frequency of breakpoints in MBR regions in HL patients than mcr cluster and an increase in t (14; 18) translocation in adult’s age and in male gander. In conclusion t (14;18)(q32;q21) was detected in B- cell lymphoma by multiple primers for all three breakpoint regions, the MBR, 3ʹ MBR and mcr with multiplex PCR amplification

Abstract

Background:Cervical cancer is the fourth most common cancer affecting women worldwide. Immunohistochemical expression of several biomarkers; including those regulating apoptosis and angiogenesis; may help to distinguish reactive conditions from precancerous and cancerous lesions of the uterine cervix.Objective:To assess the IHC expressions of p53, bcl2 and CD34 in cervical intraepithelial neoplasias and carcinomas.Methods:A cross sectional study included a total of 127 formalin-fixed paraffin-embedded cervical tissue blocks; of which 22 cases were chronic cervicitis, 24 cases were low grade squamous intraepithelial lesion (LSIL), 28 cases were high grade squamous intraepithelial lesion (HSIL) and 53) cases were invasive cervical carcinomas. Sections from each block were immunohistochemically stained for p53, bcl2 and CD34.Results: p53 was not expressed in chronic cervicitis, with significant increase in its expression from LSIL through HSIL to carcinomas had been identified. A significantly higher IHC expression of p53 was observed in adenocarcinomas and adenosquamous carcinomas compared to squamous cell carcinomas. Bcl2 was expressed in all cases with non-significant differences. Regarding CD34 IHC expression; there was a significant increase in microvessel density (MVD) from chronic cervicitis through LSIL and HSIL to carcinomas. A significantly higher MVD was detected in adenosquamous carcinomas and adenocarcinomas, in poorly differentiated carcinomas and was significantly increasing with stage.Conclusions: p53 plays an important role in the progression of the severity of intraepithelial cervical lesions. MVD can be utilized as ancillary marker for the risk of malignant transformation of cervical intraepithelial lesion.Key words: LSIL, HSIL cervical carcinoma, p53, bcl2, CD34, MVD

Abstract

Background: Cancer is one of the major human problems and breast cancer is a significant health problem. In spite of the developed treatment methods, breast cancer cannot be treated very effectively. Therefore, the identification of novel genes that will have played a role in the treatment and diagnosis of breast cancer and the high interest of B-cell lymphoma (BCL2) family proteins are critical regulators of apoptosis).Objective: The current study aimed to investigate the relationship between the levels of BCL2, BAX, and B-cell integrate medium (BIM) proteins which the prognosis and apoptosis processes of the breast cell line.Patients and Methods: Collected tissue samples from normal and cancer tissue type, RNA isolated from the cell, then quantitation done, cDNA synthesized, cDNA quantitation and Real-time PCR was performed to see the level between two kinds of genes.Results: BCL2 protein level was significantly lessened in regulator molecule tissues of a cell line with breast cancer. In contrast to Bax protein, no significant difference was detected in the BIM expression level. Our result also showed that low BCL2 expression level is associated with significant clinical characteristics of the patient such as tumor grade, stage and breast cancer type.Conclusion: Findings of our study showed that BCL2 has a role in breast cancer formation and might be a novel biomarker for the diagnosis and treatment of breast cancer.
