The value of serological markers (CA19-9 & CCSA4) in the screening and prognosis of colon cancer

Abstract

Background:- Colonic cancer is a very common disease world-wide being fourth most common cancer characterized by abnormal proliferation of the inner wall of colon at beginning then taking full colon wall thickness then to surrounding lymph nodes and tissues and finally metastasis.Objective:- To evaluate the efficacy of serum CA19-9 and CCSA-4 levels in the screening and prognosis of colonic cancer and their validity for this.Patients and methods:-This study was applied on 35 patients with colonic cancer, 35 patients with benign polyps and 16 negative controls. All individuals were subjected to blood sampling for measuring their serum CA19-9 and CCSA-4 using ELISA technique.Results:-Our study shows that colonic cancer patients presented at ages between 53-82 years of age (mean 68.5). Serum levels of CCSA-4 were significantly elevated in those patients with advancing stages (C&D) compared with stages (A&B) and lower levels found in patients undergone surgical removal of tumor or received chemotherapy.Also a positive relation found between CCSA-4 with alcohol intake and smoking. Less significant levels and relations found with CA19-9.Conclusion:-Serum CCSA-4 is sensitive and specific indicator for diagnosis, prognosis and screening of colonic cancer.Keywords:-Colonic cancer, tumor marker, CA19-9, CCSA-4.

Detection of H.pylori in a Group of Iraqi Patients with Colonic Cancer (Histological and Molecular Study)

Abstract

ABSTRACT:BACKGROUND: H. pylori is a one of the gastrointestinal organism which infects more than half the population of the world. H.pylori had been recognized as a class I human carcinogen by the International Agency for Research on Cancer. Recently H. pylori were detected in the mucosa of normal colon. Certain studies suggested a possible relationship between CagA (Cytotoxin Associated Gene A) positive H.pylori and Colonic Cancer while other studies did not find causal relationship between CagA positive H.pylori and Colonic Cancer. OBJECTIVE: We studied and isolated H.pylori from at or near by the site of lesion of Colonic Cancer and it is found to assess a possible relationship between CagA positive H.pylori and Colonic Cancer.PATIENTS AND METHODS: Study involved 30 patients of colonoscopically and Histopathologicaly determined Colonic Cancer (CC group) compared with patients with normal colon (n=90) as a control group (NC group). Patients divided into <20 years age group, 20-40 years age group and >40 years age group. Endoscopic specimens were tested for Biopsy Urease Test (BUT) and Hematoxylin & Eosin (H&E) method for detection of H. pylori also the detection of CagA mRNA using In Situ Hybridization technique with a biotin labeled probe.RESULTS: There was 66.7% H.pylori positive in patients with Colonic Cancer (by BUT) and 60% by (H&E) compared to 33.3% (by BUT) and 26.7% (H&E) in patients with normal colon (Control). A comparison between NC&CC groups regarding H.pylori infection using H&E method revealed significant difference (P=0.001).Using In Situ Hybridization technique, there was 66.7% CagA positive H.pylori in CC group in correspondence to 25% in NC group. CagA positive H.pylori infection between CC and NC groups revealed significant difference (P=0.008)CONCLUSION: H.pylori detected at or near by the site of lesion of Colonic Cancer. In addition it was found that there is a statistical relationship between CagA positive H.pylori infection and Colonic Cancer which suggested a possible role of CagA positive H.pylori in Colonic Cancer.

Abstract

Background: Colorectal carcinoma is common in Northwest Europe, North America, and other Anglo-Saxon areas, while it decreases in number in Africa, Asia, and some parts of South America,There are many immunohistochemical markers react to colonic tissue, the large majority of colorectal carcinomas are positive for mucin stains. Colorectal adenocarcinomas are invariably positive for cytokeratin (CK), Reactivity for CEA is also the rule; as a matter of fact, failure to detect CEA in an adenocarcinoma of makes a colo-rectal site of origin seems to be unlikely, and many other markers that could claimed in colorectal tumors, a one marker that may has a role in staining colorectal tumors is HepPar-1 which is a monoclonal antibody that reacts to an as yet unidentified cytoplasmic marker of normal and neoplastic hepatocytes, which could be expressed in neoplastic or non-neoplastic colorectal tissue.Objectives: to see the expression of HepPar-1 (cytoplasmic marker of normal and neoplastic hepatocytes) in colorectal cancer.Patients and methods: Fifty-eight cases (49 with colorectal carcinoma, 9 cases on non-malignant colorectal tissues) male and females depending on records, the study is conducted in GIT specialized hospital in medical city / department of histopathology – Baghdad city during period from 1/10/2016 to 1/4/2017.Results: cases were studied, the positivity in our study of the cases was (8.2%), and none of the control "non-neoplastic" cases express this marker. No significant statistical correlation was found between HepPar-1 expression and the tumor grade, site, age or sex, (P values >0.05).Conclusion: HepPar-1 can be expressed in tumors including 8.2% of colorectal carcinoma in this study; HepPar-1 is not expressed in normal colorectal tissue.Keywords: HepPar-1, colonic cancer, rectal cancer, liver secondary tumors

الخلفية: يعتبر سرطان القولون ذو انتشار كثير في مناطق شمال اوربا, واميركا الشمالية وبريطانيا العظمى ولكن بنسب قليلة في افريقيا واسيا واميركا الجنوبية. هنالك الكثير من المعلمات المناعية التي تتفاعل مع انسجة القولون, فان كثير من سرطان القلون والمستقيم يتفاعل مع (mucin), كذلك يتفاعل سرطان القولون والمستقيم مع (cytokeratins) , وايضا مع CARCINOEMBRYONIC ANTIGEN (CEA).وهناك الكثير من المعلمات الخرى التي يمكن ان تستضهر في القولون والمستقيم , واحدة من المعلمات التي من الممكن له دور في التفاعل مع انسجة اورام القولون والمستقيم هو المعلم المناعي (HepPar 1) , حيث انه جسم مضاد احادي النسخة يتفاعل مع جسم مستضد غير معرف في سايتوبلازم خلايا الكبد الطبيعية والسرطانية وقد يضهر تفاعلا مع انسجة القولون والمستقيم سواء السرطانية او الغير سرطانية.الهدف: لمعرفة مدى استضهار المعلم المناعي (HepPar 1) في سرطان القولون والمستقيم .الاسلوب : ثمان وخمسين حالة (49 منها سرطان القولون والمستقيم و9 منها لنسيج القولون والمستقيم لغير الاورام السرطانية) للاعوام 2014, 2015 و 2016 تشمل الذكور والاناث وتعتمد المستخرجات على السجلات الموجودة , الدراسة اجريت في مستشفى الجهاز الهضمي في مدينة الطب في بغداد قسم النسيج المرضي من تاريخ 1102016 الى تاريخ 142017النتائج : ثمان وخمسون حالة ( تسع واربعون منها سرطان القواون والمستقيم وتسع حالات منها لخزعات للقولون والمستقيم لحالات غير السرطان) تم دراستها , الاستضهار الايجابي كان بنسبة (8.2%) للانسجة السرطانية , بينما كانت الانسجة الغير سرطانية ليس هناك استضهار للمعلم المناعي المذكور , ولا توجد علاقة احصائية بين المعلم المناعي (HepPar-1) وبين درجة التورم وموقع السرطان والعمر والجنس وذلك لان قيمة P اكثر من 0.05.الخلاصة : بالرغم من اعتبار HepPar-1 كملعم اورام للكبد سواء النسيج الطبيعي او السرطاني , الا انه يمكن استضهاره في انسجة سرطانية اخرى ومثالا لها هذه الدراسة التي بينت نسبته في سرطان المستقيم والقولون بنسبة (8.2%) بينما لم يظهر في النسيج الغير سرطاني للمستقيم والقولون.مفاتيح الكلمات: HepPar-1 , سرطان القولون , سرطان المستقيم , سرطان الكبد الثانوي.
