Abstract

Al – Kindy Col Med J 2012; Vol. 8 No. 1 P: 63AbstractBackground : Shoulder pain is a common problem that can pose difficult diagnostic and therapeutic challenges for the family physician It is the third most common musculoskeletal complaint in the general population, and account for 5% of all general practitioners musculoskeletal consultsObjective: To determine the diagnostic performance of ultrasonography compared with the physical examination for detection of rotator cuff tears in painful shoulder syndrome.Method: Prospective study was done on seventy patients (48 male, 22 female), age ranged between 30-70 years (mean age 50 years), From February 2007 to July 2011, were subjected to comparative study in Al-Kindy teaching hospital with rotator cuff tears, including physical and ultrasonographic examination.Result: Ultrasound examination confirmed the presence of rotator cuff tear in 58 patients (82.8%), about two third of them below age 40 (65.7%) and male gender (68.6%).Conclusion: Ultrasonography which is non-invasive, safe; cheap with no known risk like radiation is a sensitive and accurate method of identifying patients with rotator cuff tears, and should be used wherever possible to improve diagnosis and treatment of painful shoulder disorders.Keyword: rotator cuff tear, physical examination and ultrasound

Abstract

Back ground: The shoulder joint is an elegant and complex piece of machinery. The rotator cuff tendons are keys to the healthy function of the shoulder and rotator-cuff tear is an especially painful injury and can happen at any age.Objectives; to evaluate the role of ultrasonography in the diagnosis of rotator cuff tear.Patients and method; From March 2013 to September 2015 forty eight patients were enrolled in a prospective comparative study and divided into full thickness, partial thickness tear or intact tendons according to clinical and ultrasonographic criteria and these results were assessed by surgery and results compared. Results: Sensitivity of clinical diagnosis was 95.3%., specifity 20% and predictive value 33.3% and sensitivity of ultrasonography was 100%, Specifity 37.5% and Predictive value of 100 %.Conclusions; Ultrasonography is well tolerated and cost-effective and useful test in diagnosis and detection of rotator cuff tear.

Abstract

Background: shoulder pain is a common problem in different age group, often diagnosed clinically but clinical diagnosis does not give accuracy alone unless to be confirmed by another investigations.Objective: Compare the role of ultrasound test in diagnosis of Rotator cuff tear with those clinically suspected patients.Patient and Method: The study started from 2008-2010. Thirty-three patients with shoulder pain were included in the study. The study is a prospective study was done in Al-Yermouk teaching hospital.Results: Complete tear is diagnosed clinically and Ultrasound study is useful to prove the diagnosis with sensitivity of (87.5%).Partial tear is diagnosed mainly clinically and Ultrasound study is not so useful in the diagnosis with the sensitivity of (56%) and thus those patients need for further investigations.Conclusion: Ultrasound is non-invasive, safe and gives high percentage of accuracy in diagnosis of Rotator cuff tear.Keywords: Rotator cuff tear, clinical diagnosis, ultrasound.

الخلفية:آلام مفصل الكتف من الامراض الشائعة والتي تصيب الاشخاص في مختلف الاعمار والتي تشخص سريريا وتحتاج الى فحوصات دقيقة لغرض الوصول الى التشخيص الصحيح.الهدف:مقارنة نتائج فحص المرضى المصابين بتمزق الكمه الدوارة في الكتف باستعمال السونار مع نتائج الفحص السريري.الطرق:تم تشخيص المرضى حسب التشخيص السريري المسبق حيث تم فحص 33 مريض في مستشفى اليرموك التعليمي, يشك في كونهم مصابين بتمزق الكمه الدواره. ومن هذا العدد اتضح ان ( ٨ مرضى مصابين بتمزق الاوتار التام و(٢٥( مريضاً كانوا مصابين بالتمزق الجزئي للأوتار سريريا.النتائج:وجد من خلال الدراسة أن التمزق التام للكُمّه الدواره يُشخص بالفحص السريري ويمكن تأكيد الفحص بوساطه السونار.ونسبه الدقه في التشخيص هي(٨٧٫٥٪).التمزق الجزئي للكُمّه الدواره يُشخص بالفحص السريري ولكن لا يمكن تأكيد الفحص بصورة مؤكده بالفحص بوساطه السونار.ونسبه الدقه في التشخيص هي(٥٦٪).الاستنتاج:تمزق أوتار الكُمّه الدواره الجزئي اكثر شيوعاً من التمزق التام والتمزق بنوعيه اكثر حدوثاً في كتف الجهه السائده واكثر في الرجال ويحدث في اعمار الاربعين سنه فما فوق.وقد تم استعمال جهاز السونار في فحص هؤلاء المرضى حيث يمتاز هذا الفحص بكونه فحص أمين ويعطي نسبة عالية من الدقة في تشخيص تمزق الكمه الدواره.مفاتيح الكلمات: تمزق الكمه الواره, التشخيص السريري, فحص السونار.
