Abstract

The present work includes a design and characteristics study of a controlling the wavelength of high power diode laser by thermoelectric cooler [TEC] . The work includes the operation of the [TEC] to control the temperature of the diode laser between ( 0- +30) °C by changing the resistance of thermistor. We can control a limited temperature of a diode laser by changing the phase cooling between hot and cold faces of the diode, this process can be attempted by comparator type [LM –311] .The theoretical results give a model for controlling the temperature with, the suitable wavelength.

العمل الحا لي يتضمن دراسة تصممية للسيطرة على الطول ألموجي الى نبضة اليزر المنبعثة من ليزر اشباة الموصلات ذات الطاقة العا لية بواسطة التبريد الكهروحراري . حيث يتضمن هذا العمل تشغيل المبرد الكهروحراري للسيطرة على درجة حرارة ليزر اشباة الموصلات بين 0-30 درجة مئوية, ويتم ذلك بواسة تغير مقاومة المقا وم الثيرميوني. ويمكن من خلال هذا العمل ايضأ الحصول على درجة حراره ثابتة لانتاج طول موجي ثابت وذلك بتغير طور المبرد الكهروحراري بين التبريد والتسخين, ويمكن ان تتم هذه العملية بواسطة المقارن نوع LM-311 . ومن خلال النتائج النظرية تظهر امكانيه عا لية للسيطرة على طول الموجى لنبضة الليزر من خلال هذا النوع من التبريد.

Abstract

Background & objectives: Ulcers in the mouth (recurrent aphthous stomatitis) are very common and may vary in size from very small to very large. The exact cause of mouth ulcers isn’t known but acidic foods and stress are thought to be factors, as well as local traumatic damage such as that caused vigorous toothbrushing. Although uncomfortable, small ulcers are tolerable, but large ulcers can last for up to 2 weeks and greatly affect the ability to eat, drink and even talk. This study aimed to evaluate the efficacy of treatment of recurrent aphthous stomatitis by application of 940 nm diode laser irradiation & its effect on the duration and pain sensation. Materials and methods: A total number of 32 patients having at least 2 ulcers for each. Those patients were enrolled in this study using a split mouth design. The study included two groups, The experimental (study group) consisted of thirty two ulcers ( in the 32 patients ) were subjected to 940 nm laser irradiation & control group comprised of thirty two ulcers ( in the same 32 patients ) left without laser irradiation. Pain scores were recorded before and after laser treatment. The patient subjected to follow-up visits until complete healing of the ulcers occurred. Results: There were immediate changes in the pain scores of the experimental group just after diode laser application. The duration of the control group lesions ranged between (7-14) days, while the lesion in the experimental group subsided with a range between (4-12) days. Conclusion:1. Single session of diode laser irradiation can be used to produce immediate, dramatic and sustained analgesic effect on RAS lesion.2. The lesion duration was significantly reduced to about 40% from that of control lesion

Abstract

The aim of study was a histopathological evaluation of the effects of low-level laser therapy (LLLT) such as diode laser on healing fracture of distal third of the radius in dogs, at the end of 1st,3rd and 6th week post operation .18 male adult street dogs were used to obtained transverse fracture at the distal third of the radius under highly aseptic technique, the anesthesia was done by using the combination of (15 mg/Kg.B.W ) ketamine hydrochloride,and (5 mg/Kg.B.W ) xylazine, the fractured limb fixed with (Gypson) as an external fixation devise, and window is obtained at the anterior surface of the radius at the fracture sites for direct irradiation of the laser. The experimental animals were divided into two equal groups each contain 9 dogs, the control group lift without any irradiation, while the treatment groups received daily single dose of laser (850 nm, for 5 minutes at 72 hours interval for 14th days). Histopathological finding at the end of the 1st week revealed early formation of irregular trabecular bone (immature woven bone), compare with the control group at the same period, then these trabecular bone became later more regular, dense and width at the end of the 3rd week in the treatment group, then quickly changed to lamellar bone in the end of the 6th wk P.O. in the treatment group compare with the control group. It concluded that The LLLT promote and accelerate fracture healing, in the distal third of the radius in dogs

Abstract

Background:A variety of benign soft tissue swellings can be found arising from oral mucosa, most of which are inflammatory hyperplasia and granuloma. Surgical diode lasers have been used in oral surgical procedures with beneficial and undoubted operative advantages and a better quality of the outpatient operation compared with electro- and coldblade techniques. Objective:To assess the effectiveness of diode laser 810 nm in the removal of pyogenic granuloma of oral cavity in repetitive pulsed mode.Methods:This study was conducted at the consultation clinic of the Maxillofacial Surgery Department in the Al-Imamein Al-Kadhiemein Medical City on 35 patients who suffered from oral pyogenic granuloma. The period of the study was from January 2013 to January 2015. The laser used in this study was diode laser 810 nm in repetitive pulsed mode. The maximum power was 5-8 watts. The pulse duration was 0.2-0.4 second and 0.2-0.4 second pulse interval. Excisional biopsies were sent for Histopathological study. Clinical examination was done and had included presence of swelling, infection, disturbance of function, pain and bleeding.Results:Patients with pyogenic granuloma were treated efficiently with repetitive mode of diode laser 810 nm. Minimal to moderate swelling, no disturbance of function, no infection and mild pain were observed postoperatively, no bleeding was seen neither intraoperatively nor postoperatively.Conclusion:Repetitive pulsed mode of diode laser 810 nm is efficient and safe in removal of pyogenic granuloma of oral cavity.Keywords:Diode laser, oral pyogenic granulomaCitation:Al-Alawi AS. Abstract

Objectives: The main objective of the current in vitro study is to compare the antimicrobial efficiency of diode laser 1064nm at different output powers and times with sodium hypochlorite at different concentrations and times against E. faecalis when used as root canal disinfectant. Materials and methods: 140 human extracted single-rooted teeth were decoronated to a length 14mm from the apical foramen to the cervical border of the root, then adjusted working length to 13mm, autoclaved and inoculated with a suspension of E. faecalis at a concentration 4×105 cfu and incubated at 37˚C for 24 hours. Samples were divided into 14 groups each group consisted of 10 prepared roots, sixty samples were irrigated with NaOCl solution at different concentrations and times. Sixty samples were irradiated at different powers at the total irradiation time 30 and 60 sec, the remainder twenty samples were control positive and negative groups. Results: All concentrations, powers and times for both diode laser 1064nm and NaOCl had significant antimicrobial effects against E. faecalis. 5.25% NaOCl at both 2 minutes and 5 minutes had the highest antimicrobial effect with no significant differences from 3W laser/10sec and 2.5W/10sec for (6 cycles) but significantly different from 3W/5sec (6 cycles). The lowest effect was 0.5% NaOCl/2min that significantly different from all other groups. Conclusion: Diode laser device seems to be highly suitable for killing E. faecalis as NaOCl irrigant in root canals if appropriate energy and irradiated time used

Abstract

Since the invention of laser in 1960, lasers have been developed and approved in many fields. Lasers can now be regarded as practical tools with unique properties that have been utilized effectively in several applications in fields of medical and biological sciences. six bacterial isolates were isolated from human samples of diabetic foot infections, which used for preparation of vaccines. The experiment was conducted on fifteen adult male rabbits, they were divided into three groups with 5 rabbits each. After irradiation of the bacterial suspensions with the diode laser for different exposure times and different frequencies, and the wavelengths used were (660, 820, 915 ) nm, the growth of bacterial isolates decreased until killed of bacteria at ( 40) min. The aim of the current study was to preparation of vaccines (live attenuated and killed) by irradiation of the bacteria by the low level diode laser. Wavelength (660) nm was more effective in killing the bacteria, and the variations were not significant between the live attenuated and the killed vaccine

Abstract

Abstract:Purpose: This is prospective study began in Jan. 2003 and concluded in April 2004, was undertaken to examine the benefits of 810 nanometer diode laser in treatment of four patient with bilateral vocal cord paralysis also to compare the results with conventional treatment Patients & Methods: 810 nm diode lasers 15 watt was used in these cases under general anesthesia, and induction of anesthetic drug done through tracheostomy tube in all patients. Results: All patients were decanulated “Tracheostomy tube removed”, the voice of all preserved within normal. Conclusion: Laser surgery in this case has more benefit and advantage than conventional methods even if the patient need more than on session of laser operation because of high success rate, less complication and easy technique. الملخص: تضمن هذا البحث دراسة سريريه لاستخدام 810 نانوميتر دايود ليزر في علاج شلل ثنائي الأوتار الصوتية أجريت لأربعة حالات في مركز مستشفى اليرموك التعليمي من الفترة كانون الثاني 2003 إلى نيسان 2004.الأسباب التي تؤدي إلى شلل الأوتار الصوتية والأكثر شيوعاً هي الضرر الذي يصيب العصب المغذي لعضلات الحبال الصوتية خلال عمليات الغدة الدرقية وكذلك أسباب أخرى منها الشدة الخارجية وإصابات الحنجرة وكذلك أسباب عصبية نتيجة تأثر تجويف الجمجمة بإصابات الشدة الخارجية أو أمراض أخرى كالنزف والأورام. الشكوى أو الأعراض التي تظهر على المريض هو انسداد الحنجرة وبالتالي ضيق التنفس الخانق والتي تستوجب عملية الفتحة الرغامية وهي عملية طارئة ومنقذة للحياة لغرض تامين التنفس. وقد استخدمت أكثر من طريقة في المعالجة بالليزر وكانت النتائج جيدة ومثيرة للاهتمام قياساً بالمعالجة بالطرق التقليدية وتظهر النتائج برفع الأنبوب ألرغامي والإمكان من التنفس من خلال المجرى التنفسي الطبيعي وكذلك المحافظة على نوعية الصوت بالحدود الطبيعية.

Abstract

Lasers can now be regarded as practical tools with unique properties that have been utilized effectively in several applications in fields of medical and biological sciences.Objectives: The aim of the current study was to preparation of vaccines (live attenuated and killed) by irradiation of the bacteria by the low level diode laser.Methods: six bacterial isolates were isolated from human samples of diabetic foot infections, which used for preparation of vaccines. The experiment was conducted on fifteen adult male rabbits; they were divided into three groups with 5 rabbits each. Blood samples were collected from the marginal ear vein of the rabbits after one month of the vaccination, for the purpose of measuring the concentration of the immunoglobulins which present in their serum, using Radial Immunodiffusion (RID) method by specialized kits (LTA-Italy).Results: After irradiation of the bacterial suspensions with the diode laser for different exposure times and different frequencies, and the wavelengths used were (660, 820, 915) nm, the growth of bacterial isolates decreased until killed of bacteria at 40 min. The results of IgA concentrations for the three groups were highly significant (P < 0.01) when comparing the attenuated with control group, while were significant (P < 0.05) between the killed vaccine and control groups, and not significant variations between the attenuated and killed groups,Conclusions: Wavelength (660) nm was more effective in killing the bacteria, and the variations were not significant between the live attenuated and the killed vaccine.

Abstract

Abstract: Background: Diode lasers are becoming popular in periodontal surgery due to their highly absorption by pigments such as melanin and hemoglobin their weak absorption by water and hydroxyapatite makes them safe to be used around dental hard tissues. Objective: The aim of the present study was to evaluate the efficiency of diode laser in performing gingivectomy in comparison to conventional scalpel technique in patients with chronic inflammatory enlargement. Materials and methods: Thirty patients were selected for this study. All of them required surgical treatment of gingival enlargements and were randomly divided into two groups: Control group (treated by scalpel and include sixteen patients) and study group (treated with diode laser 940nm and include fourteen patients). Data collected during and after surgery were compared and statically analyzed. Results: Fifteen male and fifteen female were included in this study with age mean of 26.2. Significant differences were observed in the Gingival Index and bleeding scores in laser group compared to the scalpel group, while no significant differences were found in swelling score and plaque index, The level of significance was set at P≤ 0.05.Conclusion: Diode laser 940nm are beneficial in achieving hemostasis with the improvement of gingival health compared to the scalpel technique.

الخلاصة : الهدف:الهدف من هذه الدراسة هو لتقييم كفاءة الدايود الليزر 940نانومتر في أداء عملية قطع اللثة بالمقارنة مع تقنية مشرط التقليدية للمرضى الذين يعانون من تضخم التهابي مزمن في اللثة المقدمة: ليزر الدايود أصبح ذا شعبية في جراحة اللثة نظرا لامتصاص الكبير من قبل ألاصباغ مثل الميلانين والهيموجلوبين مع قابلية امتصاص ضعيفة عن طريق الماء وهيدروكسي اباتيت مماجعلها آمنة للاستخدام في جميع أنحاء الأنسجة الصلبةللأسنان. المواد والطرق: تم اختيار ثلاثين مريضا لهذه الدراسة، عن العلاج الجراحي المطلوب من التوسعات اللثة، قسمت عشوائيا إلى مجموعتين. وتمت مقارنة مجموعة المراقبة (يعامل من قبل مشرط) ومجموعة الدراسة (تعامل مع دايود ليزر 940 نانومتر)، البيانات التي تم جمعها أثناء وبعد الجراحة وتحليلها بشكل ثابت. النتائج: أدرجت خمسة عشر ذكرا وخمسة عشر الأنثى في هذه الدراسة بمتوسط عمر26.2 وقد لوحظت فروق ذات دلالة إحصائية في مؤشر اللثة والنزيف في مجموعة ليزر مقارنة مع مجموعة مشرط في حين أن هناك لا توجد فروق ذات دلالة إحصائية في درجة التورم ومؤشر البلاك ، تم تعيين مستوى أهمية في P≤ 0.05. الاستنتاج: ليزر الدايود 940نانومتر مفيد في تحسين صحة اللثة مقارنة مع تقنية المشرط.

12.CO2 DIODE LASER HEMORRHOIDECTOMY: CLINICAL EXPERIENCE WITH 150 PATIENTS

Abstract

Background: Hemorrhoids are the most common benign anorectal conditions. About half of population are affected by hemorrhoids during their life time. Conventional surgical hemorrhoidectomy, which was the standard treatment for many decades are associated with remarkable and intolerable pain that may last up to 3 to 6 weeks, which makes more patients reluctant to surgery. Laser hemorrhoidectomy is relatively new promising modality with less postoperative pain and less complications with fast wound healing and early return to work.Objective: To assess the validity, feasibility and the outcomes of using the CO2 diode laser for treatment of symptomatic hemorrhoids.Methods: A prospective study conducted for the period from September 2013 to April 2015, included 150 patients (135 male and 15 female), with symptomatic first, second and third degree piles with age ranged from 24 to 83 years (mean 48.7 year) submitted to CO2 diode laser hemorrhoidectomy with either the coagulation mode or cutting mode of 30 Watt diode laser surgical machine. The procedures were done under local anesthesia as a day case surgery by single surgeon. Results: The results showed that the operative time ranges from 15 to 30 minutes (mean 23.5 minutes). Postoperative pain scores by visual analog scale (VAS) in the first, second and third postoperative days were 2.9, 2.1, 1.8 respectively. The pain decreased rapidly after the first week and reached to zero after 14 days postoperatively. The complications rate recorded in this work was 7.3%. All these complications were mild and can be dealt with conservatively; redo surgery was not required in any case. All patients were discharged 2 to 4 hours postoperatively and were followed for 3 to 6 months.Conclusion: Hemorrhoidectomy by CO2 diode laser is effective, and very quick outpatient procedure with very mild postoperative pain and low or negligible rate of complications. It associated with rapid healing and fast recovery that most patient can return to normal daily activity within 2 to 5 days.
