Incidence of Epstein Barr Virus infection in newly diagnosed non-Hodgkin lymphoma in the national center of hematology-single center study

Abstract

Background: Non Hodgkin lymphoma (NHL) is a diverse group of disorders both in their original history and in response to therapy .The appearance of Epstein-Barr infection (EBV) in Non-Hodgkin’s lymphoma (NHL) could be distinguished by enzyme link immune sorbent assay by detecting IgM and IgG. The role of EBV as causative agent for non-Hodgkin lymphoma has been supported by identification of large amounts of latent membrane protein 1 (LMP-1) outflow in tumors. The aim: of this study was to determine the incidence of EBV infection in patient with newly diagnosed patients with non-Hodgkin lymphoma. Materials and Methods: This study was done at the Department of clinical hematology, the national center of hematology (NCH), Baghdad, Iraq from August 2012 to November 2013. It is a prospective study. A total of 56 patients with age ranged from 18 – 56 years were included. They were diagnosed with NHL and then classified to subtypes by studying histopathology and immunohistochemistry. Assessment of EBV IgM and IgG were done by ELISA technique. A second group comprised of 22 patients with non-malignant condition regarded as control group with matched sex and age. Statistical analysis was performed utilizing SPSS version 17.0.Result: Mean age of the patients was 51.44±15.93 years. Thirty-six (70.4%) were males and 20 (29.6%) were females. Seven (12.5%) out of 56 cases were positive for EBV–IgM, two cases (28.5%) were follicular lymphoma, 5 cases (71.5%) were diffuse large B cell lymphomas. For the EBV-IgG 12 patients (21.4%) were negative while majority 44 patients (78.6%) were positive, in comparison to control group the result was statistically not significant.Conclusion: This study showed that the age, sex and clinical characteristics of Iraqi patients with NHL are different from those of surrounding countries and. The incidence of EBV is high in NHL. The most frequent subtype of NHL is diffuse large B cell lymphoma.

خلفية: ورم الغدد اللمفاويية اللاهوجكني هو سرطان الغدد الليمفاوية و هو مجموعة متنوعة من الاعراض التي تصيب الاشخاص و تتنوع سواء في تاريخ المرض الأصلي والاستجابة للعلاج .ان ظهور و اكتشاف الإصابة بابشتاين بار ( EBV ) في ورم الغدد اللمفاويية اللاهوجكني يمكن تمييزها عن طريق فحص بتقنية الاليزا حيث يتم اكتشاف الغلوبولين المناعي.. وقد تم دعم دور EBV كمسبب للمرض أو في نشوء ورم الغدد اللمفاويية اللاهوجكني عن طريق تحديد كميات كبيرة من البروتين الغشاء الكامنة 1 ( LMP- 1 )..الهدف: كان الهدف من هذه الدراسة تحديد معدل عدوى انتشار فيروس ابشتاين بار EBV في المرضى المشخصين حديثا بمرض ورم الغدد اللمفاويية اللاهوجكني في المركز الوطني لامراض الدم في بغداد.المواد و الطرق: أجريت هذه الدراسة في قسم أمراض الدم السريرية، في المركز الوطني لأمراض الدم ( NCH ) ، بغداد، العراق من أغسطس 2012 إلى نوفمبر 2013. وكانت دراسة استطلاعية . ما مجموعه 56 من المرضى الذين يعانون من ورم الغدد اللمفاويية اللاهوجكني تراوحت اعمارهم ما بين 18-56 عاما. تم التشخيص والتصنيف إلى أنواع فرعية من خلال دراسة التشريح المرضي و المناعية. وقد أجريت تقييم EBV الغلوبولين المناعي بواسطة تقنية .ELISA المجموعة الثانية تتكون من 22 مريض مشخصين بحالة غير خبيثة وتعتبر المجموعة الضابطة المتطابقة مع الجنس والعمرلمرضى ورم الغدد اللمفاوية. تم إجراء التحليل الإحصائي باستخدام التكيف SPSSالنتائج: كان متوسط عمر المرضى 51.44 ± 15.93 عاما. 36 مريضا ( 70.4 ٪ ) كانوا من الذكور و 20 مريضا ( 29.6 ٪ ) من الإناث . سبعة ( 12.5 ٪ ) من أصل 56 حالة كانت إيجابية لل EBV - الغلوبولين المناعي ، و 2 من الحالات ( 28.5 ٪ ) كان تشخيصها لمفوما جريبية ، 5 حالات ( 71.5 ٪ ) تم تشخيصا بالأورام اللمفاوية ذات خلية كبيرة . كانت نتيجة EBV IgG- لاثنا عشر مريضا ( 21.4 ٪ ) سلبية في حين كانت الغالبية 44 مريضا ( 78.6 ٪ ) إيجابية ، و بالمقارنة مع المجموعة الضابطة كانت النتيجة إحصائيا غير ذات دلالة .الاستنتاج : ان عدوى انتشار فيروس ابشتاين بارEBV في مرض ورم الغدد اللمفاويية اللاهوجكني هو شائع مع نسبة انتشار تساوي 12.5 ٪ و بصورة عامة فهي شائعة في النوع المعروف ب ورم اللمفاوي ذات الخلية الكبيرة (ب). بينما غالبية المرضى مع NHL تحمل EBV IgG .

Abstract

Background: Breast cancer is the most common malignancy affecting females worldwide. The association of Epstein-Barr virus (EBV) with this cancer is a long-standing interest to this field.Aim: to investigate the presence of EBV in breast tumor tissue in relation to age.Patients and Methods: Paraffin-embedded tissue blocks from 45 female patients with breast tumors (ranged in age from 28 to 85 years) were retrieved. The cases were grouped into two categories: group (A): included 30 cases with breast carcinoma and group (B): included 15 cases with benign breast diseases as a control group .The expression of EBV protein was examined immunohistochemically. Results: Twelve (40%) of the 30 breast cancer cases (group A) were reported as positive for EBV expression and significantly higher in patients less than 40 years age (P= 0.026). Conclusion: the results of the current study might refer to the significance of EBV expression in breast tissue of young patients with breast cancer.

Abstract

ABSTRACT:BACKGROUND:Epstein-Barr virus (EBV) was ubiquitous Herpes virus that had a role in the development ofundifferentiated carcinoma of the nasopharynx, Burkett’s lymphoma, acute infectious mononucleosisand other lymphoprolifrative disorders.METHODS:Thirty Iraqi patients with nasopharangeal carcinoma were referred to Oncology Unit in Al-KadhemiaTeaching Hospital from 1992-1994. Sera of those patients were tested for the presence of antibodiesagainst Epstein-Barr virus nuclear and early antigens using indirect immunoflourescence test.Cellular immunity for those patients was tested for the CD4+, CD8+, CD4/CD8 ratio, T-cells % andB -cells %. Their results were compared with twenty-two normal apparently normal individuals.RESULTS:Antibodies to Epstein-Barr virus nuclear and early antigens were detected in nasopharangealcarcinoma Iraqi patients and not in the control group. There was significant difference between twogroups in CD8+ cells, T- cells % and B- cells % and there was no significant differences between twogroups in CD4+ cells, CD4/CD8 ratio.DISCUSSION:EBV infection was stopped by T- cells immune response that was capable of eliminating virusinfected cells and virus neutralizing antibodies against nuclear and early antigens which prevent thespread of infection. Lymphocytes were predominantly CD8+ cytotoxic T lymphocytes, whichrecognize and destroy EBV infected cells.RECOMMENDATIONS:Other antibodies to viral capsid antigens (IgG, IgA and IgM). Other methods must be used other thanindirect immunoflourescense test like western blot method and enzyme linked immune sorbent assay(ELISA).

Abstract

Summary Background :To detect and study prevalence of Epstein Barr – virus in human serum of thalassemic patients in Mosul –Iraq.Patients and Methods The presence of IgM – antibodies has been examined in a group of 104 (63 males and 41 females) consecutive thalassemic patient who received blood transfusion in Mosul . Results Overall 18.3% were found to be anti – EBV positive and 81.7% were negative and Only 6.0 (20.7%) from those positive were also have anti lgM for Cytomegalovirus CMV. Conclusion (s): Epstein – Barr –virus infection was diagnosed by serological screening in 19(18.3%) thalasemic patients in Mosul using ELISA.

الغاية :- للتحري ودراسة انتشار الأجسام المضادة لفيروس ابشتاين- بار في مصول مرضى الثلاسيميا في الموصل- العراق.المرضى وطرائق العمل:- تم التحري عن وجود الكلوبيولينات نوع أم في مجموعة من 104(63ذكرو41 أنثى) في مرضى يعانون الثلاسيميا والذين يستلمون الدم في الموصل.النتائج:- اعطى18.3 % منهم أجساما مضادة ضد فيروس ابشتاين- بار ولم تلاحظ في81.7% منهم .وأعطى فقط من الذين كانوا يمتلكون هذه الأجسام أيضا أجسام مضادة لفيروس التشوهات الخلقيةCMV .الاستنتاج:- شخصت الاصابه بفيروس ابشتاين-بار بالتحري مصليا في19(18.3%) من مرضى الثلاسيميا.

Abstract

Background: Epstein Barr Virus (EBV) contamination has been involved in pathogenesis of numerous forms of carcinomas, which includes gastric most carcinoma nasopharyngeal carcinoma, and bladder most carcinoma. It has been recently linked with cancer of the breast. The aim of this study is to estimating the relationship among Epstein Barr virus encoded minor piece of RNA (EBER) with tumors of the breast. 40 patients with breast cancer had been regained from the Pathology laboratory of AL-Sadder Medical City in Najaf AL-Ashraf Governorate/Iraq. Scientific records were investigated of the medical information and formalin permanent, paraffin implanted tumor tissue have been observed via Chromogenic in situ hybridization (CISH) method to the discovery of the protein of virus EBER. The manifestation of EBER in most breast tissue in this study became 50% (11 from 22), in which tough association became observed amongst the communication from EBER and sufferers with cancer of the breast. Even as not create considerable variances between ISH terms of EBER with kind of cancer, age, lymph node metastasis and grade. Depedned on the outcomes of the present study, Epstein Barr virus performs a prime part in the pathogenesis of most breast cancers.

Abstract

ABSTRACT:BACKGROUND:Infectious mononucleosis is caused by the ubiquitous Epstein-Barr virus .It is a common condition usually adolescents and young adults. Most cases are mild to moderate in severity with full recovery taking place over several weeks. More sever cases and unusual complications occasionally occur.AIMS OF THE STUDY:Determining the role of theTh1and Th2 in infectious mononucleosis –like syndrome by measuring IL-10.METHODS:Enzyme immunoassay for Quantitative Determination of human Interleukin-10 in serum.RESULTS:of cytokines showed a significant increase in IL-10 in patients with EBV infection (122.3±34.97)pg/ml. In non EBV infection the level of IL-10 was also increase (74.24±18.84)pg/ml when compared with the healthy control (2.2±0.86) pg/ml. There was high significant difference between them (p<0.001).CONCLUSION:High levels of IL-10 may be referred to a TH1/TH2 response in infectious mononucleosis like- syndrome in comparison with healthy control group.

DETECTION OF EPSTEIN BARR VIRUS IN RENAL TRANSPLANT RECIPIENTS: TWO CENTERS STUDY

Abstract

Background:Viruses are among the most common causes of opportunistic infections after transplantation. The risk for viral infection is a function of the specific virus encountered and the intensity of immune suppression used to prevent graft rejection.Epstein-Barr virus infection has also been implicated as co-factor in acute and chronic rejection syndromes.Objective:Detection of Epstein-Barr viremia in renal transplant recipients.Methods:Fifty seven (57) renal transplant recipients were enrolled in this study. Plasma samples were taken from all renal transplant subjects. Screening of Epstein-Barr virus was first done by serology viamono spot test, then, viral DNA of Epstein-Barr viruswas extracted from 200 µl plasma samples and Epstein-Barr virus DNA was detected and measured by Taqman quantitative real-time PCR.Results:19/57 (33 %) of renal transplant subjects had Epstein-Barr virus viremia and the viral load ranged from 7100 to 16.165 copies/ml. Serology of all RT subjects showed negative heterophil antibody except for one patient had positive hetrophil antibody.Conclusion:The current study showed that Epstein-Barr virus might be considered as an important cause of renal impairment and allograft loss in renaltransplant subjects. And Epstein-Barr virus seems associated with post transplantation renal impairment and/or kidney rejection. Real-time PCR is a very sensitive and specific method for the detection of Epstein-Barr viremia in renal transplant subjects.Key words:Epstein-Barr virus, Renal transplantation, real-time PCR

Abstract

Gastric carcinoma is the most prevalent cancer related deaths worldwide today . The current study was carried out to shed the light on Epstein Barr virus, Tumor suppressor gene p53 associated gastric cancer of some Iraqi patients. According to insitu hybridization technique , this study was determined EBV and p53 gene correlates with some aspects of gastric cancer patients like age, gender , histological grade and stage of the tumor . Fourty six biopsies were obtained from patients with adenocarcinoma who had undergone gastrectomy. The tissue sections were collected during the period between September 2010 until June 2013. Epstein Barr virus was detected insitu hybridization in 18 patients (39%) out of 46. Whereas, the positive results of relation between p53 oncogene and gastric adenocarcinoma patients were detected in 30 (65.2%) out of 46. Out of 46 patients , this study included 32 male and 14 female with mean age 54 ranged between 30-72 years. The histological types included 18 well, 4 moderate and 24 poor differentiated respectively . Most of cases 32 (69.5%) falling in stage I-II and the remaining 14 were in the stage III and IV. The positive results revealed that EBV correlate in highly significant association with each of age, gender, grade and stage of the tumor , also expression of tumor suppressor gene p53 was correlation with all patients aspects in highly significant association.

Abstract

Background: Many newer studies reported that Epstein- Barr virus (EBV) hasassociation with chronic lymphocytic leukemia (CLL). The average age of presentation isof patients with CLL is between 65 and 70 years with male to female ratio is 2:1. Notably,several studies have reported that expression of Epstein Barr encoding RNA (EBERs) isassociated with progressive or accelerated clinical courses. This type of RNA increasesthe level of IL-8 and IL-10 in serum of newly diagnosis of CLL patients.Objectives: the aims of study were to determine the interleukins 10 and 8 level in newlydiagnosed CLL patients and determine the incidence of EBV infection in patient of CLL.Materials and Methods: A prospective study conducted at department of clinicalhematology in the national center of hematology in Baghdad, Iraq from January 2013 toJanuary 2014. It included thirty samples of formalin-fixed, paraffin-embedded tissue ofbone marrow aspirates samples and blood from newly diagnosis B-CLL. They werediagnosed with CLL according to Binet criteria. The detection of EBV encoded RNAs(EBER1, EBER2) and also detection for the level of Interleukin (8 and 10) in the serum ofCLL patients were done by in situ hybridization technique.Results: Histopathological study revealed that all the controls were negative for EBERsand 46.7% were found to be EBERs positive. There was a correlation between positiveEBERs and tumors stage and also EBERs and IL-10 and with IL-8.Conclusion: The highest incidence of CLL occurs in the age group 40-80 years old andmales are more liable than females (male/female: 70/30).In situ hybridization technique issuccessful method to detect of EBV and positive EBERs. IL-8 is highly significant inCLL patients and correlates with EBERs and LMP1. There is a correlation between IL-10EBERs and LMP1 in CLL patients.
