Abstract

The objective of the current study was to evaluate the healing of experimentally severed Achilles tendons of the dogs repaired by Kessler suture pattern alone or with autologous peritoneal scaffold. A total of twelve healthy, neurologically and orthopedically local breed dogs were used in this study, which were randomly allocated into two equal groups (6 of each). The 1st group as a control, while the 2nd as a treated group. In both groups the Achilles tendon of one hind limb was completely severed transversely, in 1st group the tendon was repaired by Kessler suture pattern using silk No.3, while in the 2nd group a combination of Kessler suture and autogenous peritoneal scaffold was applied to reinforce the tendon repair. The site of surgery was evaluated clinically, while gross and histological study was performed at 15, 30 and 60 days post-operative days. Results was revealed that autologous peritoneum graft has a remarkable effects on Achilles tendon healing represented by improving fibroblastic proliferation, reduce adhesion, increase tendon strength and improve tendon function when compared with the control group. We concluded that peritoneal graft application has valuable and beneficial effects on treatment of ruptured Achilles tendon in dogs when utilizing it in conjunction with the traditional procedures.

هدفت الدراسه الحاليه لتقييم التئام وتر اكيلس الممزق تجريبيا والمعالج بالخياطه بتقنيه كسلر لوحدها او مع استخدام سقالة البريتون الذاتيه في الكلاب استخدم 12حيوانا محليا خاليا من الاصابات العصبيه والعظميه .قسمت الحيوانات عشوائيا الى مجموعتين متساويتين في كل مجموعه 6 حيوانات .المجموعه الاولى مجموعه السيطره والمجموعه الثانيه مجموعه المعالجه. في كلتا المجموعتين تم قطع الوتر بشكل تام ثم خياطته بطريقه كسلر مع تثبيت سقالة البريتون الذاتيه لتدعيم الوتر في المجموعه الثانيه .تم تقييم موضع العمليه سريريا اما الفحص النسجي المرضي فقد تم اخذ خزعات لفترات 15و30و60يوما بعد العمليه . اظهرت النتائج ان استخدام رقع البريتون الذاتيه لها تاثير واضح على التئام وتر اكيلس وكان جليا من خلال تكوين الارومات الليفيه وتقليل الالتصاق وزياده شد الوتر وقوته وتحسين وظيفته مقارنة بمجموعة السيطره نستنتج من ذلك ان استخدام رقع البريتون الذاتيه ذات اهميه وتاثير فاعل في علاج وتر اكيلس الممزق تجريبيا في الكلاب اذا استخدم مقترنا مع الخياطه التقليديه

Abstract

Auricular reconstruction represents a meticulous reconstructive and aesthetic problem to theplastic surgeon. One of the greatest challenges in facial plastic surgery is total earreconstruction. The ability to reconstruct a fully satisfactory complete external ear has forcenturies been an elusive goal. The most promising field of advances with the hope ofeventually clinical utility lies in the realm of bioengineering with cultured cartilage.To date, no perfect material has been found to substitute for the sharply elastic cartilagenormally present in ear, the matter of total auricular reconstruction remain very complex,Prosthetic restoration is not favored by most but does remain available option for manypatients. Tissue engineers have waited to create a precise three dimensional auricularreconstruction neocartilage.Ten patients were operated upon for auricular reconstruction in Basrah Alsader TeachingHospital. The operation had 2 stages: First stage, creation and implantation of frameworkcartilage graft, which harvested from the 6th ,7th & 8th ribs. The second stage, was detachment ofthe constructed auricles ear lobe transposition and post auricular sulcus was defined byseparating the constructed auricle from the head covering the under surface with a thick splitthickness skin graft.The success rate was 90% with good and satisfactory result except in one case; there wasinfection with loss of the cartilage.

COLUMELLAR STRUT GRAFT IN TIP RHINOPLASTY, IS IT OF BENEFIT?

Abstract

Abstract Strut graft is an important, commonly used method to increase nasal tip projection and rotation. This study discuss its benefit by prospective analysis of a group of patients in which strut graft was used and compare it with a control group using digitalized photographs. The aim of this study is to evaluate the benefit of columellar strut graft and its effect on nasal projection and rotation using digital images. Thirty two patients were treated with external rhinoplasty. Using their photographs, we analyzed the projection and rotation of the nose before and after operation. The patients were classified into two groups: group A included 18 patients who have strut and group B included 14 patients without strut. The analysis of the photos of the two groups was done with a computer program. By using Goode method, nasal tip projection decreases from 0.63 to 0.62 for patients using the strut, while in patients without strut nasal tip projection decreases from 0.64 to 0.61. Nasal tip rotation slightly increased in patients with strut graft from 99 to 99.5 degrees, while in those without the graft, nasal tip rotation markedly increased from 95 to 103 degrees. In conclusion, external rhinoplasty decreases nasal tip projection and the use of strut graft is unnecessary in increasing nasal tip projection but it helps in preserving the projection and slightly increasing nasal tip rotation. Abstract

The biological calcium hydroxyapatite was isolated and purified from local cow bones in laboratories of the University of Mosul. The chemical analysis of the product shows that the purity of the biological calcium hydroxyapatite is 99%. The biological calcium hydroxyapatite was prepared locally as bone graft substitute from cheap unlimited resource. The biological calcium hydroxyapatite was implanted around induced fracture of forearm in 4 white male rabbits. The forearm fracture united thirty days after the experiment. The biological calcium hydroxyapatite was implanted inside the medullary cavity of the femur and around fracture ends in another 4 male white rabbits. The implanted biological calcium hydroxyapatite was replaced by callus around the bone and in medullary cavity of bone without any chronic inflammatory changes or foreign body reaction. We conclude that biological calcium hydroxyapatite can be used as bone graft substitute.

Abstract

ABSTRACT:BACKGROUND:Peyronies disease is an acquired fibrous plaque induced penile deformity,it has two phases;acute with painful erection and chronic with minimal pain but persistant penile curvature.Many conservative and surgical treatment modalities have been described.Plaque excision and dermal grafting has given promising results.OBJECTIVE:To evaluate the efficacy of plaque excision and dermal grafting in peyronies disease.PATIENTS AND METHODS:A prospective study was conducted on ten patients with chronic peyronies disease.All the patient underwent surgical excision of fibrotic plaque and dermal grafting.All the patients were evaluated postoperatively through regular visits for six to eight months to detect any recurrence or erectile dysfunction.RESULTS:The disease was more prevalent between ages of 40-55 years.All the plaques were located dorsally on the penile shaft except one which was dorsolateral.Five patients were complaining of erectile dysfunction preoperatively,two of them improved after surgery.Postoperative recurrence of fibrotic pluque involved only one patient.CONCLUSION:Fibrotic plaque excision and dermis grafting is an effective method on the long term with minimal associated complications.

Abstract

Objective: to evaluate the results of (Modification of Russe method) in treatment of nonunion fracture scaphoid bone by bone graft with external splintage (plaster of paris cast (pop ).Methods:Prospective study done on 26 patients (24 male, 2 female), age range between 25-42 years (mean age 34 years), fracture site at middle 1/3 with minimal displacements with no carpal bone or radial bone injury, technique of Matte- Russe method (explore the bone through volar approach using bone graft from iliac crest (cortico-cancellous peg plus cancellus bone) with thumb spica for 90 days with period of follow up 12-18 months.Results: out of 26 patients treated by this method , 23 patients (88.5%) union was achieved radiologically by the end of 3rd month &progressed over the next 3 months, range of motion of the wrist & power of grip increased gradually became almost comparable to the normal side by the end of sixth month , 5-7 degrees loss of dorsiflexion was residual in 6 patients . Three patients (11.5%) did not show union radiolgically after sixth postoperative month, their local symptoms were better than before surgery ,they were not keen to have another operation .No complications were noticed, wound healed well ,patient tolerance was goodConclusion: the use of cancellous bone graft with plaster of paris (p.o.p.) splint is less demanding surgically& less coasty and resulted in good results(88.5% healed well ).we recommend this treatment as a method for treating nonunion of carpal scaphoid bone .Key wards: nonunion, scaphoid , bone graft , plaster of paris

Abstract

Background: Augmentation of the craniomaxillofacial region is required for many aesthetic and reconstructiveprocedures. Many bone grafts substitutes have been developed .These products differ in their osteoconductive andosteoinductive properties .The use of bone graft substitutes offers the ability to lessen the possible morbidity of theharvest site inautograft. Aragonitic calcium carbonate has proved to have biocompatible properties as bone graftsubstitute. This study aimed to evaluate the histopathological effects of aragonitic calcium carbonate inexperimentally induced bone defect in rabbit's mandible.Material & method: Twenty five adult male healthy rabbits were used in current study. Two holes were prepared inthe rabbit's mandible, first hole remain empty as control, the second filled with aragonitic calcium carbonate.Samples were taken after 2, 6,8,12 and 24 weeks. Histopathological evaluation include; examination of new boneformation and inflammatory response, in both control and experimental groups.Results: At 2 weeks, there was no new bone formation in both defects with moderate amount of inflammation .At6,12 and 24 weeks the amount of new bone formation in implant group was more than that formed in control group( statistically highly significant correlation, p<0.01). At 8 weeks, less bone amount in implant group .There weremarked degradation of implant particles at 6 weeks.Conclusions: ACC is suitable bone graft substitute and induce new bone formation by acting as bioactive,osteoconductive bone graft when come in contact with bone.

Abstract

Background: In Maxillofacial surgeries, iliac bone graft has traditionally been harvested to provide osteoconductive,osteoinductive and osteogenic components to initiate bony formation. There are multiple types of procedures forharvesting iliac bone graft, either by surgical exposure procedure for procurement bone graft bicortical, unicortical,tricortical or by using Fritsch bone harvesting (trephine) system or by Spine-Tech grinding harvester technique. Thisstudy compares procedures for harvesting iliac bone graft by using sub-crestal window technique procedure andtrap-door iliac crest bone graft procedure for different purposes like alveolar cleft, bone resection due to tumor,reconstruction of avulsed facial bone due to trauma by bullet injury or RTA, confirming that donor site morbidity ofthe sub-crestal window technique is more significant than trap-door technique.Patients and methods: In this study (40 patients / 22♂:18♀) underwent harvesting of iliac bone graft with age range(3.5-65 years) in Al-Kadhmyea Teaching Hospital (2009-2012) for different purposes like alveolar cleft, reconstructionof mandible after tumor resection and reconstruction of maxillofacial defects due to massive blast or bullet injury.Results and Conclusions: The results indicate that major complications from ICBG harvest are uncommon, but minorcomplications are common. The findings suggest that donor site morbidity of the sub-crestal window technique issignificantly less than trapdoor technique. Making the sub-crestal window technique is the method of choice forharvesting large amounts of bone graft

Abstract

Objective: The study aims to assess the early and late complications that occur in patients with cardiovascular-coronary grafts as well as find a relationship with some demographic specifications for them.Methodology: A descriptive study - analysis conducted to assess the complications early and late that occur in patients with graft artery to coronary heart in the intensive care unit of Hospital Ibn Al-Bitar Teaching Hospital, surgery specialist education in the city of Baghdad for the period from the first of September / 2009 until the twentieth of May / 2010. Chosen sample object-and consisting of (50) sick "after the exit of the process through the (48) hours the first in the intensive care unit phase evaluation of the first and the second phase in the outpatient clinic after discharge from the hospital and when their review periodical for the purpose of screening and follow-up after obtaining official approvals fundamentalist institutions health and to decide when, with patients in advance. Data was collected using the questionnaires and filled through the interview. The data were analyzed using descriptive statistical methods (frequencies and percentages) and inferential statistical analysis (mean and correlation coefficient).Results: The results indicate that most of the sample are male, smokers, people with high blood pressure in addition to the strong relationship between educational level and incidence of complications early for the process of grafting veins coronary heart Kertjav heart muscle, lack of fluids, difficulty breathing, infection of wounds on the chest. Conclusion: The study concluded that the most of the sample are male, smokers, people with high blood pressure in addition to the strong relationship between educational level and incidence of complications early for the process of grafting veins coronary heart. Recommendation: The study recommended an educational program to provide a video or manuals containing instructions to be followed by patients pre and postoperative in order to avoid complications.

الهدف: تهدف الدراسة الى تقييم المضاعفات المبكرة والمتأخـــــــــــرة التـــــــي تحدث عند مرضى ترقيع الشــــــرايين القلبية التاجية وكذلك ايجاد العلاقة مع بعض المواصفات الديموغرافية لهم. المنهجية: دراسة وصفية - تحليلية أجريت لتقييم المضاعفات المبكرة والمتأخرة التي تحدث عند مرضى ترقيع الشرايين القلبية التاجية في وحدة العناية المركزة لمستشفى ابن البيطار التعليمي ومستشفى الجراحات التخصصية التعليمي في مدينة بغداد للفترة من الاول من ايلول / 2009 ولغاية العشرون من مايس /2010. اختيرت عينة غرضية والمتكونة من(50) مريضا" بعد خروجهم من العملية خلال ال (48) ساعة الاولى في وحدة العناية المركزة كمرحلة تقييم اولى والمرحلة الثانية في العيادة الخارجية بعد خروجهم من المستشفى وعند مراجعتهم الدورية لغرض الفحص والمتابعة بعد استحصال الموافقات الرسمية الاصولية من المؤسسات الصحية وتحديد الموعد مع المرضى مسبقا". تم جمع المعلومات بأستخدام الاستمارات الاستبيانية وملؤها عن طريق المقابلة. تم تحليل البيانات بأستخدام الوسائل الاحصائية الوصـــفية ( التكرارات و النسب المئـوية) والتحـــليل الاحصائي الاستنتاجــــي ( الوسط الحسابي ومعامل الارتباط ).النتائج: اشارت النتائج الى ان معظم افراد العينة هم من الذكور ,المدخنين, والمصابين بأرتفاع ضغط الدم بالأضافة الى وجودعلاقة قوية بين المستوى التعليمي وحدوث المضاعفات المبكرة لعملية ترقيع الاوردة التاجية للقلب كأرتجاف عضلة القلب , قلة السوائل , صعوبة التنفس وخمج الجروح عن الصدر.التوصيات: أوصت الدراسة بتقديم برنامج تثقيفي فيديوي او كتيبات تتضمن الأرشادات والتعليمات الواجب اتباعها من قبل المرضى قبل العملية وبعدها لتلافي حدوث المضاعفات.

Abstract

Background:The reconstruction of facial deformity has been of paramount clinical concern for many years and one of the most difficult and challenging tasks facing the maxillofacial surgeon. The ultimate goal in the treatment is the relief of suffering, restoration of function of jaw, restoration of speech, regain of the normal looking contour and improvement in the quality of life.Objective:To obtain more knowledge on autogenous on lay bone graft behavior in different facial defects and to evaluate the lateral and medial surgical approaches to the iliac crest.Methods:A prospective study was conducted during the period from January 2009 to January 2012 on 20 patients with facial defects, in the orbit, zygoma, maxilla and mandible. The causes of defects were trauma, odontogenic tumors and alveolar cleft. Types of bone graft used were either block cortico-cancellous or chips cancellous bone, the block was either monocortical or bicortical bone graft.Results:Complications associated with donor site harvesting procedure included pain 5% and gait disturbance 5%. Failure of bone graft was observed in 3 patients (15%) while the rest, 17 patients, (85%) ended with functional and esthetic successful graft. Causes of failure were due to sequestration (5%), inflammation due to osteomesh (5%) and soft tissue breakdown (5%).Conclusion:Iliac crest graft has evolved as a safe, well accepted procedure, with relatively low morbidity that can be used for a wide variety of maxillofacial procedures.Keywords:Iliac crest, bone graft, facial defects
