Abstract

SUMMARY:BACKGROUND:Hodgkin disease is a lymphoid tumor that accounts for less than (1%) of all DeNovo neoplasms occurring every year worldwide.OBJECTIVE:The aims of the study was to assess the expression of immunohistochemical markers namely (CD20,CD15) in malignant cells of Hodgkin's disease.METHODS:Over the period extending from (January 2006-March 2008), specimens form 31 cases of lymphocyte predominance Hodgkin lymphoma (Rye's classification) as formalin fixed, paraffin embedded tissue blocks from excisional lymph node biopsies. For each case, five representative sections were prepared. One stained with hematoxyline and eosine (HxE), and four other sections (on positively charged slides) prepared for IHC procedures using CD15, and CD20. For the evaluation of markers expression, a semiquantitative evaluation system was used to register the staining intensity and the numbers of positive cells.RESULTS:The most common site of lymph node biopsy was the cervical lymph node in (61.2%) of cases. Immunohistochemically, CD20 was expressed in (12.9%) of cases, while CD15 was expressed in (77.4%) of cases.CONCLUSION:This study has shown that HD with a nodular growth pattern and a lymphocyte-rich background encompasses 2 entities with distinct morphologic, phenotypical features. Therefore, the precise classification of such case requires a combination of conventional histology and immunohistology with distinct panel of antibodies.

Abstract

Background: Hodgkin lymphoma (HL) is a malignant process involving the lympho-reticular system that accounts for 6% of childhood cancers. Infectious agents may be involved, such as human herpes virus 6, cytomegalovirus, and Epstein-Barr virus (EBV). Infection with EBV confers a 4-fold higher risk of developing HL.Objective: To have an idea about the mode of presentation, pathological subtypes, treatment applied and outcome in one center in our country.Methods:A retrospective study was done whereby twenty cases were collected from the Pediatric Oncology Clinic in Al-Imamein AL-Kadhimein Medical City over a period of ten years from the first of January 2007 – end of December 2016. Information was taken from the patient’s records in the Pediatric Oncology Clinic including age at presentation, sex, physical finding, histopathological subtypes, staging , treatment applied and outcome.Results: Among the studied group, 14 cases (70%) were males and 6 cases (30%) were females, male: female ratio equal to 2.3:1, rang of age was between 5-12 years with a peak age at presentation was between 5-7 years, 12 cases (60%). The initial presentation was an enlarged cervical lymph node in 19 of them (95%), histopathologically, most of the patients had mixed cellularity subtype, 9 cases (45%), stage II and stage III comprise the majority of cases 9 cases (45%), 6 cases (30%) respectively, B symptoms were reported in 7 cases (35%). Chemotherapy was the mainstay of treatment with good response and remission, 17 cases (85%). Three cases refractory to treatment (15%) with no response, no death was reported.Conclusion: Male predominance was noticed with younger age group at presentation. The majority presented with enlarged cervical lymph node. Although mixed cellularity was encountered in most of the patients, the response to chemotherapy was good. Rituximab can be used along with chemotherapy for refractory cases and for lymphocyte predominant Hodgkin lymphoma.Keywords: Hodgkin Disease , ChildrenCitation: Abbas SA, Ali RH. Treatment of Forty Adult Patients with Hodgkin Disease; Baghdad Teaching Hospital Experience

Abstract

Background: Hodgkin disease was the first cancer in which the curative potential of combination chemotherapy was demonstrated. The affected patients are often young and there is a great potential for adding years of productive life by giving curative therapy even when the disease is advanced. Objective: to describe the experience of the hematology unit,Baghdad Teaching Hospital, in the management of 40 adult patients with Hodgkin disease.Patients and Methods: a retrospective cohort study of forty adult Iraqi patients with Hodgkin disease between 2005 and 2013 in the hematology unit. Patients were treated initially with 6-8 cycles of ABVD chemotherapy protocol (doxorubicine+ bleomycin+ vinblastin+ dacarbazine) , nine patients received additional involved field radiotherapy for residual masses or bulky disease. Overall survival and progression free survivals were estimated using Kaplan Meier survival plot.Results: The mean age was 28.6±12.88 years with females forming 61.5% of patients, mean duration of follow up was 27.9± 20.6 months. Staging showed that 55% and 27.5% had stage II and III respectively. B symptoms were found in 72.5% patients , bulky disease in 42.5% patients. Complete Response+ Complete Response undetermined was seen in 85% of cases. First Relapse occurred in 14%, and death in 7.5% of the patients. The 8 year overall survival and progression free survival were 82% and 50% respectively while the mean overall survival and progression free survival times were 84.7 and 59.9 months respectively.Conclusion: The results of the treatment of adult patients with Hodgkin disease in our unit is rather comparable to the results from other studies. المقدمة: مرض هوجكن كَانَ السرطانَ الأولَ الذي تم الشفاء فيه بالعلاج الكيمياوي. إنّ المرضى المتأثّرينَ هم شباب في أغلب الأحيان وهناك إمكانية عظيمة لإضافة سَنَواتِ مِنْ الحياةِ المُنْتِجةِ بإعْطاء العلاجِ الشافيِ حتى عندما يكون المرضِ متقدّمُا.الهدف: لوَصْف تجربةِ شعبة امراض الدمّ في مستشفى بغداد التعليمي في علاج 40 مريضِ بالغينِ مصابين بمرضِ هوجكن.المرضى والطرائق: دراسة مجموعةِ ذات أثر رجعيِ شملت أربعين مريضِا عراقيا من البالغينِ المصابين بمرضِ هوجكن بين 2005 و 2013 في شعبة امراض الدمّ. المرضى عولجوا أولياً بنظامِ علاج ABVD الكيمياوي (doxorubicine + bleomycin+ vinblastin+ dacarbazine)، كما إستلمَ تسعة مرضى علاج بالأشعةَ الحقلِية .تم تحليل النتائج احصائيا. النَتائِج: كان متوسط العُمر 28.6 سنةَ .الإناث تُشكّلُ 61.5 % مِنْ المرضى، كان متوسط مدة المتابعة 27.9 شهرَ. كما وجد بأن 55 % و27.5 % من المرضى كَانَ عِنْدَهُم المرحلةُ الثّانية والثّالثه من المرض على التوالي.وجدت أعراض ب في 72.5 % من المرضى، كما كان المرض ضخما في 42.5 % من المرضى. تم الحصول على الهوادة الكامله + الهوادة الكامله غير المقرّره في 85 % مِنْ الحالاتِ. الانتكاسة الأولى حَدثتْ في 14 %، وحدثت الوفاة في 7.5 % من المرضى. كانت نسبه بقاء المرضى احياء ونسبة بقاءهم بدون انتكاسة لـمدة 8 سنوات هي 82 % و50 % على التوالي بينما كان متوسط فترتيهما هي 84.7 و 59.9 شهرَا على التوالي.الخاتمة: إنّ نَتائِجَ معالجةِ المرضى البالغينِ بمرضِ هوجكن في شعبة امراض الدم مقارنةُ بالنَتائِجِ مِنْ الدِراساتِ الأخرى متقاربة الى حد مامفتاح الكلمات: مرض هوجكن، ABVD، بقاء.
