Abstract

Expressed Emotion (EE) is an established factor in short term relapse in schizophrenia.Research on Expressed Emotion (EE) developed out of studies of the impact of family members on patients with schizophrenia, and has provided us with a measure of relatives' emotional attitudes towards patients. This study aimed to find out the relation between families high & low emotional expression and schizophrenic patients relapse. The study population consisted (60) patients diagnosed with schizophrenia and their householders, EE of the householders was rated by using Comberwell Family Interview. Results revealed that high criticism (CR) was associated with higher rate of readmission an longer hospitalization compared with low criticism. The result supported the value of CR as a prognostic indicator of the course of schizophrenia.

يعتبر التعبير العاطفي من العوامل التي نشأت وساعدت على انتكاست مريض الفصام خلال فترة قصيرة. ان الدراسات حول التعبير العاطفي تطورت ليس فقط الى دراسة تأثير اعضاء الاسره على مريض الفصام وانما تطورت الى قياس مواقف افراد اسرة المريض العاطفية واثرها على المرضى الذين يعانون من مرض الفصام، وقد وفرت لنا قدرا من المقاييس لهذه المواقف العاطفية لأقارب المرضى. تهدف هذه الدراسة إلى معرفة العلاقة بين الأسر عاليةالتعبير العاطفي ومنخفضة التعبير العاطفي مع انتكاسة مرضى الفصام. وتألفت عينة الدراسة من (60) من المرضى الذين تم تشخيصهم بانفصام العقل مع أرباب اسرهم، وتم قياس التعبير العاطفي لدى أرباب الأسر باستخدام مقابلة كامبيروول للعوائل. وكشفت النتائج على ان الانتقادات عالية الحدة الموجه الى المريض تؤدي الى طول فترة بقاء المريض في المستشفى وكذلك ارتفاع معدلات الانتكاسة بعكس الانتقادات منخفضة الحدة هذه النتائج اعطت اهمية لاعتبار حدة الانتقادات كمؤشر للتنبؤ بمسار مرض الفصام

Causes and Prevention of Missing a Diagnosis and Late Management of Acute Appendicitis

Abstract

A delay in diagnosis and late management of appendicitis with appendectomy is still high with subsequent some serious sequelae. This study is to identify and prevent a causes of delay in diagnosis of appendicitis. Aprospective cross section study includes 308 appendectomies performed in a Hilla teaching hospital from January 2013 to July 2015 by same surgical team; 143 (46%) male and 165 (54%) female. Delayed diagnoses was considered when the duration of pain was more than 48 hours, or operative finding show obvious features of delay, like perforation, gangrenous appendix, abscess collection or mass formation. Average duration of symptoms for delayed patients was 96 hours and for early diagnosed was 29 hours, Reasons fordelay were sought and divided into two groups; the first is patient behavioral reason, and the second is medical assessment reason. Twenty one percent of appendectomised patients show delayed management despite more than 48 hours abdominal pain (3-10 days, average 4 days), 69%of them was male, and, 31% female. Average duration of hospitalization in a delayed cases was 3.5 days, while average duration of admission for early treated cases was 1.3 day. Mortality rate was zero. As a reason for delay in diagnosis; patient assessment was the main reason for delay diagnosis (44 patients, 68%); while patient behavior cause was 32% (21 patients). The patient assessment is the main cause of delay in diagnosis and should be improved by examining patients with abdominal pain by expert surgical team considering atypical presentation with proper follow up and investigations.Quality improving program is advised to reduce missed diagnosis of appendicitis.

Abstract

With an emphasis on the pattern of curative tourism knew and practiced in the days of the Romans, where the therapeutic resorts and curative springs, baths and widespread, in addition to the Pharaonic history that mention to the knowledge of the ancient Egyptians to this pattern, Nowadays with the increasing of the progress of civilization and technology in recent years and doubled pressure of life and different aspects of pollution and congestion have emerged many of the diseases of modern psychotherapy, anxiety and nervous tension, heart disease, etc., thereby increasing the need for tourism activity in general and especially for curative tourism, whether medical or therapeutic or preventive. Therefore, countries of the world are racing to attract tourism and therapeutics tourists from all over the world, relying on what it characterized at both the medical possibilities and natural resources with therapeutic properties. The aim of this study is to analyze the current state of curative tourism in general in Saudi Arabia, and study the effect of the quality of medical services provided on marketing and growth of curative tourism coming to the Kingdom of the application on the city of Riyadh. The study relied on a set of scientific references and studies, research and official reports and opinions tourist competent and medically related to the field of curative tourism, in addition to a field study based on the descriptive method was based on a questionnaire a simple random sample of patients who received medical treatment institutions in the city of Riyadh. Came out research with a number of results was the most important possession of the Kingdom of Saudi Arabia to the elements of curative tourism in all its forms and a private hospital, medical, however, that despite the efforts at the official level of the General Authority for Tourism and Antiquities, there is still a lack of marketing of this type needs to review in the coming years. The research recommended that the adoption of medical institutions in the Kingdom of the policy of development and marketing of curative tourism, and increasing forms of cooperation and coordination between the government and private agencies involved in curative tourism
