Abstract

Background: Periodontitis (PD) is well-known chronic disease affecting the periodontal ligament and alveolar bone, Osteoarthritis (OA) is a chronic joint disease with compound reasons characterized by synovial inflammation, subchondral bone remodeling, also the formation of osteophytes, that cause cartilage degradation. Chronic periodontitis and osteoarthritis are considered widely prevalent diseases and related to tissue destruction due to chronic inflammation in general health and oral health. The aim of this study is todetermine the association of chronic periodontitis and osteoarthritits in patients by analysing tumor necrosis factor alpha TNFα and high sensitive c-reactive protein (hsCRP) in the serum.Materials and Method: A total of 80 patients of both sexes aged 35-50 years ,30 patients with osteoarthritis and moderate chronic periodontitis (CP+OA), 30 patients of both sexes aged 35-50 years with moderate chronic periodontitis alone (CP). And control group (C) of 20 healthy patients with healthy periodontium participated in this cross sectional study. we excluded the postmenopausal and pregnant woman from female patient and smoker patient also. All patients are free of medication and have good general health with no history of systemic disease. Participants with OA have documentation or radiographic imaging, consistent with degenerative arthritis in the absence of an inflammatory condition. Venous blood samples were drawn from each subject using pyrogen-free heparinized collection tubes. Tubes were centrifuged according to the manufacturer’s guidelines, then stored at -15°C till analyse. serum TNFα and hs CRP were measured using an enzyme-linked immunosorbent assay (ELISA).Results: A non-significant differences between studied group regarding serum level of hsCRP and TNF-α, as well as, the result revealed that the median of serum levels of hsCRP were little higher in chronic periodontitis with osteoarthritis group than chronic periodontitis group and control groups represent as 0.31, 0.29, and 0.24. Similarly, the medium serum level of TNFα was higher in chronic periodontitis with OAthan in other two groups (10.615, 10.533, and 9.682 respectively). The age and gender showed a non-significant difference between the groupsConclusion: The present study revealed that there’s a strong positive correlation between serum levels of hsCRP and TNFα in patients of all groups but there is no correlation between Osteoarthritis and chronic periodontitis.

Some Parameters of Inflammation & Oxidative Stress in Relation to the Risk of Type 2 Diabetes Mellitus

Abstract

Background Many clinical trials have indicated that lifestyle modification can delay or prevent the progression to type 2 diabetes in people with impaired glucose tolerance test (IGT). Detection of IGT requires a test which is inconvenient to screen for this condition in clinical practice or in the general population. Therefore, there is a need to search for additional significant risk factors for type 2 diabetes. Patients &methods Seventy two adults ≥ 40 years old (36 males and 36 females) were evaluated in this study. They were subjects who performed a brief 75-grams oral glucose tolerance test and were classified as having normal glucose tolerance (NGT group; 24 subjects), impaired glucose tolerance (IGT group; 24 subjects), or diabetes mellitus (DM group; 24 patients).In addition to hematocrit (PCV) and erythrocyte sedimentation rate (ESR), serum level of highly sensitive C-reactive protein (hsCRP) was measured. Also the serum levels of thiobarbituric acid-reactive substances (TBARS), iron, copper and ferric reducing ability of plasma (FRAP) were estimated in addition to erythrocyte glutathione (Ery-GSH) level. Results Both IGT group and DM group have a significantly high hsCRP mean level (2.59 ± 1.03 and 2.89 ± 0.90 respectively vs. 1.57 ± 1.40 mg /L in NGT group, P< 0.001), and a significantly decreased FRAP level (939.2 ± 157.4 and 961.5 ± 125.1 respectively vs. 1063 ± 104.5 µmole/L in NGT group, p< 0.01 ) as well as a significantly high TBARS and a significantly low Ery-GSH level in comparison with NGT group. The positive correlation between hsCRP and TBARS, although was statistically not significant, showed a step-wise increment from NGT, to IGT and to DM group (r = 0.01, 0.14 and 0.23 respectively). Conclusions IGT is associated with a state of low-grade inflammation and oxidative stress. Serum levels of hsCRP as a marker of inflammation and TBARS as a marker of oxidative stress may serve for identifying people at high risk for developing type 2 diabetes. Such people will be important targets for programs that are designed to prevent diabetes.

Evaluation of Interleukin 6 and high sensitive C-reactive protein in relation with Body Mass Index

Abstract

Background:Obesity is one of the largest public health problem worldwide. It is a multi-factorial chronic disease that increases mortality and the prevalence of heart-related diseases. Adipose tissue secretes large numbers of peptides that resemble cytokines. Interleukin 6 )IL-6( is one of those peptides that are released by adipose tissue. One of the major function of IL-6 is sending signal to liver to secrete C-reactive protein (CRP).Objective:To study the relationship between obesity and inflammation by: estimation of high sensitive C-reactive protein (hsCRP) and IL-6 concentrations in participants sera in relation with different body mass indices (BMI). Patients and Methods: This cross-sectional study was performed through the period of 3 months and 90 participants with the average range of age 25-40 years old were included, they were categorized into three groups ( 30 with BMI less than 25, 30 with BMI ranged between 25-29.9 and 30 with BMI more than 30).Results:The concentrations mean of hsCRP was significantly elevated among the three studied groups as a result of BMI increasing (0.945 mg/L, 1.694mg/L and 2.521mg/L respectively). There was a significant increasing in the mean of IL-6 in relation with increased BMI in all included groups (1.084mg/L, 1.802mg/L and 3.531 mg/L respectively). Linear correlation between the concentrations means of hsCRP and IL-6 were found in all studied groups.Conclusion:This study demonstrated a significant relationship between obesity and inflammatory marker (IL6 and hsCRP) .

خلفية الدراسة: تعتبر السمنة من اهم المشاكل الصحية الاجتماعية المنتشرة في جميع أنحاء العالم، وخاصة في البلدان الصناعية. وهو مرض مزمن متعدد العوامل يزيد من معدل الوفيات وانتشار الأمراض المرتبطة بالقلب. تفرز الأنسجة الدهنية أعدادا كبيرة من الببتيدات التي تشبه المدورات الخلوية . IL-6 هي واحدة من تلك الببتيدات التي يتم تحريرها من الأنسجة الدهنية. احد الوضائف الرئيسية لـ IL-6 هي إرسال إشارة إلى الكبد لإفراز CRP.اهداف الدراسة: هدفت هذه الدراسة الى معرفة العلاقة بين السمنة والالتهاب من خلال تقدير قيم تراكيز hsCRPوIL-6 في مصول المتبرعين وفقا لقيم كتلة الجسم BMI .المرضى والطرائق : استمرت الدراسة لمدة 3 اشهر واشتملت على 90 متبرع تراوحت اعمارهم 25-50 سنة وصنفوا الى 3 مجاميع حسب مقياس كتلة الجسم BMI : BMI<25 و BMIيتراوح بين 25-29.9 و 30 BMI≥.النتائج: اظهرت النتائج ارتفاعا معنويا في معدلات قيم تراكيز hsCRP في المجاميع الثلاثة بارتفاع قيم مقياس كتلة الجسم (0.94 mg/L و 1.694 mg/L و2.521 mg/L ) على التوالي ايضا ارتفعت معدلات IL-6 معنويا وفقيا لازدياد مقياس كتلة الجسم في المجاميع الثلاث ( mg/L 1.084 و 1.802 mg/L و 531 mg/L )على التوالي . اظهرت الدراسة وجود ارتباط خطي بين قيم hsCRPوIL-6.الاستنتاجات : توصلت هذه الدراسة الى وجود ترابط معوني بين السمنة والالتهاب.

High sensitive CRP as a predictor of Coronary Artery Disease Severity on Coronary Angiography

Abstract

Background: Inflammation in the arterial intima has the main role for the pathogenesis of atherosclerosis. Objective: To determine the correlation between serum hsCRP levels and severity of coronary artery disease in a sample of patients referring to Surgical specialty Hospital- Cardiac center in Erbil City 2018.Patients and Methods: In this cross sectional study 120 patients (78 male and 42 female patients) were studied who had undergone coronary angiography procedure among them 80 patients ( 57 male and 23 female ) had Coronary artery disease and the remain 40 patients had normal coronary angiography result. The blood samples from (120) patients were taken and transferred to the laboratory after clotting hsCRP was measured using cobas automated assay.Results: Here we detected a significant difference in the level of hsCRP between patients with coronary artery diseases and patients with normal coronary artery on angiography in addition to that fact the association for the severity of coronary artery disease and hsCRP level between single, two vessels and three vessels diseases, was statistically significant as well a finding which was comparable with other studies that detected a significant difference between hsCRP among patients with more severe coronary artery lesions.Conclusion: The study concluded that high level of hsCRP is a predictor of atherosclerosis and is wise to be done for patients as a risk factor and it is not only a predictor of coronary artery disease but it predicts it is severity as well.

خلفية الدراسة: ان وجود الالتهابات في الشرايين القلبية يكون لديها دور رئيسي و متسبب في تصلب الشرايين.اهداف الدراسة: لتحديد العلاقة بين ارتفاع مستوى بروتين hsCRP (بروتين التفاعلي عالي الحساسية) في مصل المرضى المصابين بتصلب الشريان التاجي و الذين اجري لهم عملية القسطرة التشخيصية لتلك الشرايين و ملاحظة شدة المرض من خلال اخذ عينات البحث من المرضى الذين يترددون إلى مستشفى التخصص الجراحي - مركز القلب في مدينة أربيل 2018.المرضى والطرائق: في هذه الدراسة المقطعية ، تم اجراء دراسة على 120 مريضاً (78 ذكورا و 42 اناثا ) ممن خضعوا لعملية القسطرة للأوعية التاجية بينهم 80 مريضاً (57 ذكور و 23 إناث) كانوا مصابون فعلا بمرض الشريان التاجي حسب نتائج قساطرهم و 40 منهم كانوا لديهم شرايين طبيعية. و من ثم تم أخذ عينات الدم من كل المرضى (120) و تم نقلها إلى المختبر واجراء فحص قياس بروتين hsCRP (بروتين التفاعلي عالي الحساسية) باستخدام جهاز الكوباكس للتحليل . النتائج: من خلال دراستنا اكتشفنا ان هناك اختلافًا كبيرًا في مستوى بروتين hsCRP (بروتين التفاعلي عالي الحساسية) بين المرضى الذين يعانون من انسداد الشريان التاجي والمرضى الذين كانت نتائج قساطرهم طبيعية حسب نتائج عملية القسطرة لديهم بالإضافة إلى تلك الحقيقة ارتباط شدة مرض الشريان التاجي حسب عدد الاوعية المتصلبة و بروتين hsCRP (بروتين التفاعلي عالي الحساسية) حيث وجدنا كلما زاد عدد الاوعية المتصلبة اي كلما ازداد شدة المرض ازداد ارتفاع مستوى بروتين hsCRP (بروتين التفاعلي عالي الحساسية) وهناك فرقا احصائيا واضحا. الاستنتاجات : خلاصة الدراسة إلى أن المستوى المرتفع من بروتين hsCRP (بروتين التفاعلي عالي الحساسية) هو مؤشر على تصلب الشرايين وأنه من الحكمة أن يتم اجراء ذلك الفحص للمرضى الذين لديهم عوامل خطورة اخرى و بالنسبة للمرضى المصابون بتصلب الشرايين كعامل مؤشر خطر وليس مجرد عامل تنبؤ بوجود مرض الشريان التاجي ولكنه يتنبأ بشدة قساوة المرض ايضا.
