Abstract

Osseous metaplasia of the endometrium is an uncommon clinical and pathological disorder. It is a very rare cause of infertility and induced by abortion. Hysteroscopic examination revealed multiple bony specules and a resectoscopic excision of the bony specules was performed. Histopathological examination reveals bony trabeculae adjacent to the endometrial glands & stroma.Case presentation: 34-year-old woman with osseous metaplasia of the endometrium is described, in which the diagnosis was suspected clinically and confirmed histopathologically. The clinical, pathogenesis and pathological features are briefly reviewed.

Abstract

ABSTRACT:BACKGROUND:Hysteroscopy have gained general acceptance as a method for the investigation of infertility. The main advantage of hysteroscopy is the capability of inspecting the uterine cavity. Transvaginal sonography (TVS) is a noninvasive modality that provide excellent imaging of the uterus and of endometrial abnormalities. The present study was designed to evaluate the use of TVS as the initial diagnostic procedure before hysteroscopy for detecting intrauterine disorders.METHODS:Seventy-eight infertile women were examined by TVS and diagnostic hysteroscopy in the late follicular phase of the menstrual cycle and the results were compared.RESULTS:The uterine cavity abnormalities were present in 19.2% of the infertile females involved in the study. The Transvaginal sonography had 87.5% sensitivity, 100% specificity for detecting endometrial polyps while it had 100% sensitivity and 100% specificity for detecting other intrauterine cavity abnormalities including intrauterine adhesions, uterine septae and submucosal myomas.CONCLUSION:Examination of the uterine cavity is an integral part of any thorough evaluation of an infertile woman. Transvaginal sonography, when performed during the follicular phase, can detect most uterine cavity abnormalities. It was an accurate tool in the identification of intrauterine adhesions, uterine septae and submucosal fibroids.

Abstract

Summary:Background: To compare the diagnostic value of hysteroscopy with conventional curettage and to evaluate the sensitivity of both methods to detect intrauterine endometrial pathology in patients with abnormal uterine bleeding.patients and Methods: This prospective study carried on 100 patients underwent diagnostic hysteroscopy as well as dilatation and curettage for abnormal uterine bleeding in two teaching hospitals, Al Yarmouk and Al Kadhmiya Teaching hospital / Baghdad from the period of Jan. 2002 to Dec. 2003, endometrial specimens were sent for separate histological study, the sensitivity of both methods were assessed according to the operative and histological findings. Results: High sensitivity and positive predictive values of hysteroscopy and directed taken biopsy for almost all pathological causes of uterine bleeding except for atrophic endometrium (66.7%) and hyperplasia (33.1%). The sensitivity of dilatation and curettage is very low compared with hysteroscopy.Conclusion: Hysteroscopy was more sensitive than dilatation and curettage in detecting endometrial polyp, submucuse fibroid, carcinoma but less sensitive than dilatation and curettage in detecting endometrial hyperplasia and atrophic endometrial . Hysteroscopy with directed biopsy taken provided high sensitivity in detecting pathological state of the endometrium.Key words: Hysteroscopy, dilatation and curettage, abnormal uterine bleeding

Abstract

To evaluate the diagnostic accuracy of saline infusion sonography (SIS) compared with hysteroscopy in patient presented with abnormal uterine bleeding. Prospective ,comparative study. This study carried in Al Yarmouk Teaching Hospital /Baghdad from the period Jan 2002 to Dec. 2002 on 50 women referred to the outpatients clinic for abnormal uterine bleeding all were underwent saline infusion sonography followed by hysteroscopy ,and the results was compared with endometrial biopsy. Sensitivity and specificity of each test were assessed. SIS was more sensitive (100% versus 88.9%) and specific (96.9% versus 94.4%) than hysteroscopy in detecting causes of uterine bleeding. SIS is good as hysteroscopy in detecting local cause of abnormal uterine bleeding , and this can replace diagnostic hysteroscopy in many patients with abnormal uterine bleeding, it is safe procedure, rarely associated with infection or complications.

لتقييم دقة التشخيص بالموجات فوق الصوتية من ضخ المياه المالحة (SIS) مقارنة مع الرحم في المريض قدم مع نزيف الرحم غير طبيعي. المحتملين، ودراسة مقارنة. هذه الدراسة نفذت في مستشفى اليرموك التعليمي / بغداد خلال الفترة من يناير 2002 إلى ديسمبر 2002 على 50 امرأة أشار إلى عيادة المرضى الخارجيين لوقف نزيف رحمي غير طبيعي وخضع جميع المالحة بالموجات فوق الصوتية ضخ تليها الرحم، وتمت مقارنة النتائج مع خزعة بطانة الرحم. وقد تم تقييم حساسية وخصوصية كل تجربة. وكانت الهيئة العامة للاستعلامات أكثر حساسية (100٪ مقابل 88.9٪)، ومحددة (96.9٪ مقابل 94.4٪) من الرحم في الكشف عن أسباب نزيف الرحم. الهيئة العامة للاستعلامات جيدة كما الرحم في الكشف عن سبب محلي من نزيف الرحم غير طبيعي، وهذا يمكن أن تحل محل الرحم التشخيص في العديد من المرضى مع نزيف الرحم غير طبيعي، هو إجراء آمن، ونادرا ما يرتبط مع عدوى أو مضاعفات.
