Abstract

SUMMARY:BACKGROUND:Hodgkin disease is a lymphoid tumor that accounts for less than (1%) of all DeNovo neoplasms occurring every year worldwide.OBJECTIVE:The aims of the study was to assess the expression of immunohistochemical markers namely (CD20,CD15) in malignant cells of Hodgkin's disease.METHODS:Over the period extending from (January 2006-March 2008), specimens form 31 cases of lymphocyte predominance Hodgkin lymphoma (Rye's classification) as formalin fixed, paraffin embedded tissue blocks from excisional lymph node biopsies. For each case, five representative sections were prepared. One stained with hematoxyline and eosine (HxE), and four other sections (on positively charged slides) prepared for IHC procedures using CD15, and CD20. For the evaluation of markers expression, a semiquantitative evaluation system was used to register the staining intensity and the numbers of positive cells.RESULTS:The most common site of lymph node biopsy was the cervical lymph node in (61.2%) of cases. Immunohistochemically, CD20 was expressed in (12.9%) of cases, while CD15 was expressed in (77.4%) of cases.CONCLUSION:This study has shown that HD with a nodular growth pattern and a lymphocyte-rich background encompasses 2 entities with distinct morphologic, phenotypical features. Therefore, the precise classification of such case requires a combination of conventional histology and immunohistology with distinct panel of antibodies.

Abstract

Background: Exposure to microwaves radiation from microwave oven may be harmful for users especially for the onewho have highest contact with microwave oven. Because the body is electrochemical in nature, any force thatdisrupts or changes human electrochemical events will affect the physiology of the body by destabilization andinterruption of many chemical body substance including growth factors.The insulin-like growth factors (IGFs) are afamily of mitogenic proteins that control growth, differentiation, and the maintenance of differentiated function innumerous tissues. It fulfils an important role in growth and development of teeth, mandible, maxillae, and tongue.Platelet derived growth factors (PDGF) are proteins that regulate cell growth and division. In particular, it plays asignificant role in blood vessel formation (angiogenesis). It seems that IGF and PDGF share in much tissuedevelopmental process. Therefore they included in the present study in correlation to tooth growth & development.This study Illustrates the expression of insulin like growth factor and platelet derived growth factor by dental cells ofrat embryos at periods of gestation 16th,18thday intrauterine life(I.U.L) and one day neonatal life.Materials and Methods: Animal model: Thirty-six female rats were used in this study .Starting from zero days (time ofgestation that recorded) the pregnant rats were divided into three groups. Group A serve as a control, groups Bexposed to microwave oven radiation For (15 /minutes; 5/min /hour for 3 hours continuously) daily and C exposed toEMF radiation for (45 minutes ;15 min /hour for3hours continuously)daily starting from zero day of gestation till the lastday. The embryo of rats at 16thday and 18th day of intrauterine life and one day old rat (new born rat) were studiedimmunohistochemically for localized of platelet derived growth factor (PDGF) and insulin growth factor (IGF) markers.Results:The results showed that experimental group (B) exposed to short duration of radiation (5/ min.) stimulates thedevelopment of tooth germ and faster tooth growing in comparison to control with immunohistochemical resultsshow strong to moderate intense stain for positive expression of growth factors(PDGF,IGF) by dental tissue.For longexposure period of radiation( group C) , it showed retardation in the tooth growth withimmunohistochemical findingsrecord weak to negative intense stain for the expression of growth factors(PDGF,IGF) by dental tissue.Conclusion: Exposure to microwave (oven) radiation during pregnancy may play a role in the expression of IGF andPDGF by cells of tooth germ thatinfluence on cell differentiation and physiological activity of specialized dental cells,depending on exposure time

and Medical Immunohistochemical expression of Aromatase in malignant breast tumours

Abstract

Twenty tumour biopsies of Iraqi breast cancer female patients were examined for immunohistochemical (IHC) expression of aromatase. The expression was correlated with age and histopathological type, grade and clinical staging of tumour. Results revealed that out of the 20 investigated cases, 75.0% showed positive IHC expression of aromatase, which was dis- tributed as 35.0% for score 2+ and 40.0% for score 3+. The remaining ve negative cases (25.0%) were observed to have the lowest age mean (38.6 ± 4.4 years), which was signi cantly different from the recorded age mean in patients with the score 2+ (53.6 ± 3.2 years; p = 0.008) or 3+ (49.4 ± 2.8; p = 0.04), but no signi cant difference was observed between age means of positive cases. A histpathological examination of tumours revealed that they were invasive ductal carcinoma (IDC) but of two types: tubular IDC (5 cases; 25.0%) and 15 cases (75.0%) were de ned as not otherwise speci ed (IDC-NOS). In tubu- lar IDC, most of cases (80.0%) had 3+ score, while in IDC-NOS, it was 26.7%. Such difference was signi cant (p = 0.023). Distributing patients by grade and stage of tumours revealed no signi cant difference in IHC scoring of aromatase or age. It is possible to conclude that positive IHC expression of aromatase is tended to occur in older ages (around 50 years) of breast cancer patients.

اجريت الدراسة على عشرين مريضة عراقية مصابة بسرطان الثدي. اخذت مقاطع نسجية للورم واجري التقييم النسجي-الكيميائي-المناعي ل رومتيز. ارتبط التعبير ل رومتيز مع التقدم في العمر ونوع النسيج والرتبة والتدريج السريري للورم. واظهرت النتائج ان ٪75.0 من المريضات كانت موجبة لفحص ا رومتيز، والتي كانت موزعة على النحو( ٪35.0 للـ +score2 و٪40.0 للـ +3 score(، وقد لوحظت خمس حا ت سالبة لفحص ا رومتيز )٪25.0( وكان لديهم أقل معدل العمر )38.6 ± 4.4 سنة(، واختلف معنويا عن المريضات الموجبة للفحص 3.2 ± 53.6( +2 score سنوات؛ 0.008 = P( و± 49.4( +3 score 2.8؛ 0.04 = P(، ولكن لم ي حظ أي اخت ف معنوي بين عمر العينات الموجبة للفحص. وكشف الفحص النسجي المرضي للمريضات انه من نوع سرطان القنوات الغازي، ولكن بنوعين: ا نبوبي 5 tubular IDC حا ت )٪25.0( و15 حالة )٪75.0( تم تعريفها بأنها )IDC-NOS(. في النوع ا نبوبي وأن معظم الحا ت )٪80.0( موجبة +3 score، بينما في IDC-NOS، كان ٪26.7. وهنا الفرق معنويا )0.023 = p(. فمن الممكن أن نستنتج ان التعبير IHCا يجابي ل رومتيز ويميل للحدوث في ا عمار المتقدمة )حوالي 50 عاما( من مريضات سرطان الثدي.

Abstract

Background and objective: The distinction of hydatidiform mole from hydropic abortion remains a problem because of interobserver and intraobserver variability. This study aimed to determine the utility of p57Kip2 as a diagnostic marker in differentiating complete hydatidiform mole from its mimics.Methods: A total of 97 formalin fixed paraffin embedded material including forty cases of complete hydatidiform mole, 36 cases of partial hydatidiform mole and 21 cases of hydropic abortion were selected randomly from the files of histopathology laboratory of Maternity Teaching Hospital in Erbil. The samples were reviewed by two pathologists, afterward; immunohistochemical staining was performed by using a p57Kip2 marker. We considered p57Kip2 positive only if nuclear p57Kip2 staining was identified in at least 10% or more of all in a tissue section. Results: Negative immunostaining was seen in 77.5% of the complete hydatidiform mole in both villous cytotrophoblast and stromal cells. In contrast, 86.1% of partial hydatidiform mole showed positive immunostaining for p57Kip2. All cases of hydropic abortion 100% were positive for p57Kip2 immunostaining. In all gestations, p57Kip2 was strongly expressed in decidua which served as internal positive control. The concordance between the initial histological diagnosis and p57Kip2 immunostaining was statistically significant (P <0.001). Conclusions: p57Kip2 immunostaining is a highly sensitive and specific marker for diagnosis and classification of hydatidiform mole. p57Kip2 staining has the advantage of differentiating hydropic abortuses from the complete hydatidiform mole.

Abstract

ABSTRACT:BACKGROUND:Carriage of Helicobacter Pylori in the human stomach is associated with increased risk of peptic ulcerdisease, distal gastric adenocarcinoma and gastric B-cell mucosa associated lymphoid tissue lymphoma.OBJECTIVE:To study the immunohistochemical expression of bcl-2, as apoptosis makers in the gastric mucosa ofpatients infected with cagA Helicobacter Pylori demonstrated by insitu hybridization method.PATIENTS MATERIALS AND METHODS:Gastric antrum biopsies from 99 patients presented with dyspeptic symptoms (50 men, 49 women,median age 40) were analysed for the presence of H. pylori, and were classified according to updatedSydney system. Insitu hybridization technique was done to detect cagA H. pylori.Immunohistochemical expression of bcl-2 using (Avidin- Biotin method) was performed on paraffinembedded biopsy specimens.RESULTS:Forty four patients (44.44%) had H. pylori cagA positive starin. Atrophy of gastric mucosa was presentin 14 (14.14 %) patients. Intestinal metaplasia was present in 8 (8.08%) patients. The frequency ofatrophy and intestinal metaplasia were significantly higher in cagA H. pylori gastritis than non-cagA H.pylori gastritis (p=0.023 and 0.041respectively). Bcl2 expression was not significantly higher in H.pylori gastritis than non-H. pylori gastritis (p= 0.101). Bcl2 expression was significantly higher in thepresence of atrophy (p<0.001). Bcl2 expression was significantly higher in the presence of intestinalmetaplasia (p<0.001).CONCLUSION:The rate of apoptosis decreases when lesions (gastric atrophy and intestinal metaplasia) are present.

Abstract

Background: Carriage of Helicobacter Pylori (H. Pylori) in the human stomach is associated with increased risk of peptic ulcer disease, distal gastric adenocarcinoma and gastric B-cell mucosa associated lymphoid tissue lymphoma. Several studies have shown increased evidence of increased cell proliferation in the gastric mucosa both in human carrying H. Pylori, and animal model of H. Pylori infection.Objective: To study the immunohistochemical expression of Proliferating cell nuclear antigen (PCNA), as a proliferative marker in the gastric mucosa of patients infected with CagA Helicobacter Pylori demonstrated by insitu hybridization method.Methods: Gastric antrum and corpus biopsies from 99 patients with dyspeptic symptoms (50 men, 49 women, and median age 40) were analyzed for H. pylori, presence of chronic inflammation, intestinal metaplasia, and atrophy according to updated Sydney system. Insitu hybridization technique was done to detect cagA H. pylori. Immunostaining for PCNA (Avidin- Biotin method) was performed on paraffin embedded tissue specimens.Results: Forty four patients (44.44%) had H. Pylori cagA positive strain. Atrophy of gastric mucosa was present in 14 (14.14 %) patients. Intestinal metaplasia was present in 8 (8.08%) patients. The frequency of atrophy was significantly higher in cagA H. Pylori gastritis than non-cagA H. Pylori gastritis (p=0.041). The frequency of intestinal metaplasia was significantly higher in cagA H. Pylori gastritis than non-cagA H. Pylori gastritis (p=0.023). PCNA labeling index (LI) of the gastric glands was significantly higher in presence of atrophic alterations (p <0.001), intestinal metaplasia (p <0.001) and in cagA strain H. Pylori positive gastritis (p<0.001).Conclusion: The rates of gastric glandular atrophy, intestinal metaplasia, and epithelial proliferation increase in the presence of H. Pylori infection, and are further increased when H. Pylori is of cag A strain.Key words: cag A H. pylori gastritis, PCNA immunohistochemical expression.

Abstract

Background:Immunohistochemistry seems to be important method for differentiation between papillary thyroid carcinoma which is the commonest thyroid cancer and Grave's disease which may be challenging by light microscopic features only.Objectives:The aim of the study was to evaluate the immunohistochemical expression of CK 19 antibody that is considered a marker of malignancy in papillary thyroid carcinoma and in Grave's disease and to determine whether CK19 is effective in the discrimination between these two pathological conditions.Methods:In this retrospective study paraffin embedded archival materials from 40 cases including 20 papillary thyroid carcinoma and 10 Grave's disease in addition to ten normal thyroid tissue were used as a control group, collected from the department of pathology of Al-Yarmook Teaching Hospital for the period between Jan 2009 to Jan 2011. The immunohistochemical expression of CK19 markers was assessed for intensity and extent of staining in semiquantative method.Results:Sixteen of the 20 papillary carcinomas showed diffuse and intense cytoplasmic staining with CK19 (80%), 4 cases showing diffuse faint staining (20%). Seven of the 10 Grave's cases (70%) are completely negative. The remaining 3 cases showing focal weak staining with CK19 (30%). There was a significant difference in the extent of staining between papillary thyroid carcinoma and Grave's disease and there was highly significant difference in intensity of staining between them.Conclusions:The staining features of CK19 may be helpful in the differential diagnosis between papillary carcinoma and Grave's disease with papillary carcinoma like structures. This immunoreactivity should be evaluated with histopathological findings in order to prevent over diagnosis of papillary carcinoma.Keywords:CK19 immunohistochemical expression, papillary thyroid carcinoma, Grave's disease.

Abstract

Background: Tooth eruption is a localized process in the jaws which exhibits precise timing and bilateral symmetry.Develop within the jaws and their eruption is a complex infancy process during which they move through bone totheir functional positions within the oral cavity. For species with more than one set of teeth, eruption of the second setalso accomplishes. The key to the successful clinical management of tooth eruption consists of understanding thatthis process consists largely of the local regulation of alveolar bone metabolism to produce bone resorption in thedirection of eruption and shift and formation of bone at the opposite side.The amniotic sac contains a considerablequantity of stem cells. These amniotic stem cells are able to differentiate into various tissues, which used in many field.Vascular endothelial growth factor (VEGF) is an important angiogenic factor reported to induce migration andproliferation of endothelial cells, enhance vascular permeability, and modulate thrombogenicity. VEGF expression incultured cells (smooth muscle cells, macrophages, endothelial cells) is controlled by growth factors and cytokines.The aim of this study was to study the administration of cell molecules of (Chorion, Amnion and Amniotic fluid)around developing mouse tooth and studying the expression of VEGF marker.Materials and Methods: forty eight albino Swiss mice of one day old age injected with isolated amniotic stem cells inthe anterior region of maxilla (incisors area) other 16 mice injected with saline represents control. Sacrifice 4 mice foreach period (4, 7, 10, and 13) day old age. The result were studied histologically and immunohistochemistry.Results: VEGF marker localized and identified in 3 areas; pulp, P.D.L, and Bone. In pulp. The mean value of positiveVEGF expression showed to be highest in Amnion group in comparison to the other studied groups. The marginalmean value of all periods reported to be highest in Amnion groups followed by Chorion group. The period 10 dayshowed highest marginal means value for positive VEGF expression for all groups. In P.D.L. area Amniotic fluid recordsthe highest mean and marginal mean value specifically at day-10 in comparison to other studied groups. In Bonearea Amniotic fluid records the highest mean and marginal mean value among the studied groups followed byChorion group. Period 7-day and 10-day shows high mean value for VEGF expression. Coincidence test for VEGFmarker illustrates to be affected by Amniotic fluid application in P.D.L. and in bone area while Amnion and Chorionapplication showed to be concerned with pulp.Conclusion. It reported that amniotic fluid application affected on expression of VEGF in P.D.L and bone whileamnion and chorion showed to affect on expression of VEGF in pulp.The present study highlighted on clinical andresearcher application of Amniotic fluid and Chorion for supplement of stem cell in dental tissue engineering or evenin other body tissues.

Abstract

ABSTRACT: BACKGROUND:The prognostic biomarkers in a disease provide information regarding outcome irrespective of therapy .Her-2/neu oncogene expression is an important prognostic and predictive marker for breast cancer .Her-2 /neu expression can be determined by immunohistochemistry ,which became a routine investigation to predict the response to tamoxifen therapy. There is evidence that over-expression of Her-2 /neu is involved in breast cancer progression and there is suggestion that Her-2/neu over-expression plays a role in the early stages of breast tumorigenesis . OBJECTIVE: To evaluate Her-2/neu over-expression in breast carcinomas in the studied sample and to assess its immunhistochemicalscore status in term of accepted conventional clinicopathological parameters . PATIENTS AND METHODS:This is a descriptive study which was carried out from 2007-2012 on 64 breast carcinoma specimens. Paraffin embedded tissue of patients underwent modified radical mastectomy with axillary clearance were processed for Her-2/neu assessment from almost all breast patients in the province . Then the evaluation of the staining and assigning a score were performed which ranged from 0 -3 .The tumor grade was asses using modified Bloom-Richardson grading system .Data on the patients age , the size of the tumor and lymph node status were obtained .Statistical analysis were produced by the use of Chi square test and at the level of significant alpha < 0.05 to compare the IHC results for HER-2 expression with the prognostic clinicopathological parameters . RESULTS : HER-2 over expression was seen in 23.5% (15/64) and the lymph nodes involvement status had a significant association with the immunhistochemical score for HER/2 oncogene and no siginificant association seen between HER-2 overexpression and other clinicopathological parameters including age , tumor size and tumor grade. CONCLUSION : The study reveals that the prevalence of HER-2 overexpression in breast cancers is less in our study compared with the western world and demonstrates association of the immunhistochemical score of HER-2 /neu with some of the well-established pathological parameters of breast carcinoma , specially lymph node involvement. It justifies the use of the immunhistochemical score in assessing breast carcinoma and modified its treatment

Immunohistochemical detection of p16INK4a proteins expression in paraffin embedded sections of colorectal cancer tissues.

Abstract

Background: Uncontrolled tumor cell proliferation is a reality in tumor cells, and the progression from a normal cell into a transformed cell probably includes genetic events affecting checkpoints in the cell cycle machinery.Materials and methods: This study investigated the immunoexpression of p16INK4a in the paraffin sections from 43-cancers of colorectal tissue (CRC),26-hyperplastic polyps and adenomas(CRHPA) and 35-normal tissues (CRN),using immunohistochemical assay. We correlated the expression patterns with tumor histopathological type, site of the tumor, distance metastasis according to the TNM system. Results: In colorectal cancer (CRC) patients’ p16INK4a,immunoexpression was detected in 32/43, there was significant association for p16INK4a, which showed high reaction with monoclonal antibodies in the distal colon and rectal area with P value < 0.001. There was no significant correlation between tissue invasion stage and p16INK4a. There was no significant difference in the immunoexpression of p16INK4a in relation to the two groups of patients with the stage of the disease. Conclusion: p16INK4a, expression have high level in CRHPA than in CRN tissue. Also, their expression in CRC is more than in CRN tissue. In addition, p16INK4a immunoexpression in CRC was significantly more than in CRHPA.Keywords: colon cancer, p16INK4a, immunohistochemical assay, cell cycle checkpoints
