Could Interleukin (IL)-6 Values be Used in the Identification of Glucose Intolerance in Obese Pediatric Population

Abstract

AbstractBackground: Adipose tissue is the source of a wide variety of molecules involved in the regulation of energy output and carbohydrate metabolism. Among these, cytokines, such as tumor necrosis factor (TNF)-α and interleukin (IL)-6, appear to play a role in modulating insulin sensitivity in peripheral tissues and have been associated with the development of insulin resistance in adults. Aim of study: The aim of the study was to ascertain the relationship among the degree of adiposity, circulating fasting insulin levels, with plasma IL-6 in an obese child and adolescent population with or without glucose intolerance.Subjects and Materials: Eighty obese children and adolescents were studied. Plasma interleukin (IL)-6 were measured for those obese with normal oral glucose tolerance Test (OGTT), and obese with glucose intolerance. These patients were compared with eighty healthy children matched for age and sex.Results: Plasma fasting insulin concentration and HOMA (Homeostatic model assessment) were significantly higher in glucose intolerance obese patients [(28.2±14.9µIU/ml),(5.7±4.8)] compared with their results in normotolerance obese patients [(20.9±8.3µIU/ml),(4.5±2.7)] and control subjects[(14.8±6.3µIU/ml),(3.2±1.9)] (p 0.005 ,0.001) respectively, Plasma levels of (IL-6) in glucose intolerance obese patients were significantly higher (2.9±1.3 ng/ml) compared with normotolerance obese patients (2.0±1.1 ng/ml) and control subjects (1.5±0.9 ng/ml)(p 0.005).Interleukin (IL-6) was positively correlated with BMI, (r=0.37; p 0.001), waist and hip circumferences(r=0.35; r=0.33) respectively, (p 0.001), insulin levels (r=0.30; p 0.001), TG and VLDL-C (r=0.27; p 0.001)Conclusion: Plasma IL-6 appears to be involved in glucose metabolism, insulin resistance, and dyslipidemia in obese children and adolescents with glucose intolerance.

الخلاصةالخلفية العلمية: الأنسجة الدهنية هي مصدر لطائفة واسعة من الجزيئات المشتركة في تنظيم إنتاج الطاقة والتمثيل الغذائي للكاربوهيدرات ,من بين هذه الجزيئات الحركيات الخلوية(السايتوكينات) مثل عامل نخر الورم (TNF-) والانترلوكين-6(IL-6) ويبدو انها تلعب دورا في تحوير حساسية الانسولين في الأنسجة المحيطية و كذلك ارتبطت مع تطور مقاومة الانسولين في البالغين الهدف من الدراسة: ان الهدف من الدراسة هو التحقق من العلاقة ما بين درجة السمنة و مستويات الانسولين الصيام الدوار مع نسبة الانترلوكين (IL-6) في بلازما الأطفال البدناء والمراهقين المصابين وغير المصابين بعدم تحمل الكلوكوز.المواد والطرق: تم دراسة 80 من الأطفال و المراهقين البدينين , حيث تم قياس نسبة الانترلوكين-6 (IL-6) في بلازما هؤلاء البدناء ذو التحمل الطبيعي للكلوكوز الفموي والبد ناء المصابين بعدم تحمل الكلوكوز. قورنت هذه النتائج مع نتائج ثمانين من الأطفال الأصحاء من نفس الفئة العمرية والجنس.النتائج : أظهرت النتائج وجود زيادة نوعية في نسبة تركيز الأنسولين و HOMA عند البد ناء المصابين بعدم تحمل الكلوكوز ](28,2± 14,9مايكرو وحدة قياسية/مليلتر) (5,7 ± 4,8 )[ عند مقارنتها عند البد ناء ذو التحمل الطبيعي للكلوكوز الفموي ](20,9 ± 8,3 مايكرو وحدة قياسية/مليلتر) (4,5±2,7) [ والأشخاص الأصحاء ](14,8 ±6,3مايكرووحدة قياسية/مليلتر) (3,2 ± 1,9[(l ] 0,005 p< و0,001 [p< على التوالي. كذلك اظهرت النتائج وجود زيادة نوعية في نسبة الانترلوكين-6 في بلازما البد ناء المصابين بعدم تحمل الكلوكوز](2,9 ±1,3 نانوغرام/مليلتر) [عند مقارنة نسبها في البد ناء ذوي التحمل الطبيعي للكلوكوز الفموي](2,0 ± 1,1 نانوغرام/مليلتر)[ والأشخاص الأصحاء(1,5 ±0,9 نانوغرام/مليلتر), (0,005p<), كذلك أظهرت الدراسة وجود علاقة موجبة بين نسبة الانترلوكين-6 في بلازما المرضى المصابين بعدم تحمل الكلوكوز مع دالة كتلة الجسم (0,37 r= 0,001 P<), محيط الخصر و الورك (0,33و0,35 r=)على التوالي (0,001p<), نسبة الانسولين (0,30 r=)(0,001 p<) والدهون الثلاثية والدهون ذات الكثافة الخفيفة جدا ( 0,27 =r )( 0,001 p<)الخلاصة : يبدو أن مستوى الانترلوكين-6 في بلازما الدم تشارك في استقلاب الكلوكوز,مقاومة الانسولين,واضطراب الدهون لدى الاطفال واليافعين البدناء المصابين بعدم تحمل الكلوكوز.

Abstract

Background:Oligozoospermia is one of male infertility type which related with sperm lowering count in seminal fluid under limited reference. There are many factors leads to oligozoospermia like, congenital defects, endocrinal disorders, genetic abnormalities, immunological factors , life style , or in some cases unknown reasons. Immune and some non-immune cells produce immuno-chemical messengers proteins called (Interleukins) work as alerts proteins and contribute in immune body defend. These mediators found in all body fluids, also found in seminal fluid,and it’s presence of certain concentrations reflects the state of health of the body.Objective:Measure the level of interleukin-6 in fertile (normospermia) and infertile (Oligozoospermia).Materials and Methods:Microscopically and macroscopically ejaculates analysis according to WHO (2010) criteria for eighty -seven ejaculate from normospermia and oligozoospermia patients with age rang (21-47)yrs. , in duration of ( Nov- 2013 to Mar- 2014), study divide in to three groups according to the concentration ranges in to (normospermia > 15X 106 cells/ml), (Mild to Moderate Oligozoospermia 15-5 X106cells/ml) , (Severe Oligozoospermia< 5X106). Then store all samples in(-20) , IL-6 was detected through the (ELISA) specific way and kit .Results:Interleukin-6level is less in normospermia group than Oligozoospermia groups at (P<0.05), IL-6 level not correlated with sperm count, except weak correlation in severe group at (P≤ 0.05) and (t α=0.05).Conclusions:There are no correlation between IL-6 level and sperm cells concentration but the variation of interleukin-6 level among the groups indicate that some sperms parameters influence on interleukin-6 levels among the groups like sperm motility or morphology and also that the causative factors which lead to oligozoospermia also effect on IL-6 level and effect on spermatogenesis

Abstract

Background Tinea versicolor is a common dermatological problem worldwide in distribution, caused by dimorphic fungus called Malassezia furfur, they exist at the very interface between commensal and pathogen and, as such, their interaction with the human immune system is of great interest. Immuno-inflamatory activity mediated by different cytokines could have a role in the etiology of tinea versicolor.
Patients and Method: The study covered 46 subjects .It comprised 26 patients with TV compared with 20 healthy normal controls. Serum IL-6 analysis was done by ELISA.
Results: The level of IL-6 was elevated in patients with tinea versicolor, when compared with healthy control. Conclusion: IL-6 increases during the infection and could be part of a complex immune response leading to fungal elimination from human body.

Abstract

Summary :Background: The role of cytokines in cancer immunity and carcinogenesis in general has been well established, which play an important role in pathogenesis of many solid cancer. This study aimed to shed light on the possible role of IL-6 in pathogenesis of CRC and its relationship with serum CEA level. Patients and methods: This study covered 50subjects. It comprised of 30 patients with CRC, compared with 20 healthy normal controls. Serum IL-6 and CEA analysis were performed by ELISA.Results: This study revealed a significant elevation in serum IL-6 level among CRC patients (16.2 pg/ml) in comparison to that of healthy control (4.8 pg/ml) (p<0.001). As well as an interesting significant increase of mean serum CEA level in CRC patients (7.5± 6.62 ng/ml) as compared to healthy control (2.4±0.90 ng/ml). On the other hand there was strong linear correlation between serum CEA level and serum IL-6 level p<0.001. Conclusion: There are a significant correlation between the elevation of the serum IL-6 and CEA levels and the progression of this cancer. Moreover, positive association was found between IL-6 and CEA and this may help us to understanding the pathogenesis of this disease and ultimately may be use in the development of a new therapeutic technique.Key words: Colorectal Cancer, Interleukin -6, Carcinoembryonic antigen

Abstract

Since the inflammatory factor IL-6 known by its critical role in different metabolic reactions in the body, therefore, this study was designed to detect it is effect in the thyroid gland function by injecting three groups of rats, the first with IL-6 , the second with carbimazole (antithyroid drug) and the third with normal saline (Control).The experiment included three groups of rat; each was injected with recombinant human interleukin-6 (rhIL-6), Carbimazole, or normal saline (Control). The results of experiments showed that both IL-6 and carbimazole caused a decrease in the levels of thyroid hormones (T3 and T4) in animal sera, and a significant decrease in animal body weight, but had no effect on the liver weight.There was a significant decrease in glucose level in the animals injected with carbimazole, while in the IL-6 treated animals there was no change in glucose level. There was a significant increase in serum level of triglyceride and very low density lipoprotein cholesterol in the animals treated with IL-6, while in the animals treated with carbimazole, there was no such change. Additionally, injection of the animals with IL-6 or carbimazole caused a significant increase in lymphocyte apoptosis of peripheral blood

نظراً للدور المهم الذي يؤديه العامل الالتهابي مادة بين إبيضاض-6 (IL-6) في العمليات الأيضية المختلفة في الجسم ؛ لذلك وجدنا إنه من الضروري دراسة تأثيره في وظيفة الغدة الدرقية و فعاليات أخرى و مقارنته مع تأثير عقار الكاربيمازول. وقد تم ذلك عن طريق حقن ثلاث مجاميع من الجرذان ، الأولى بمادة بين إبيضاض-6 ، و الثانية بعقار الكاربيمازول ، و الثالثة بالمحلول الملحي الفسيولوجي بوصفها سيطرة. بينت هذه التجربة إن كل من مادة بين إبيضاض-6 و الكاربيمازول يسببان إنخفاض مستويات هرمونات الدرقية (T4,T3) في مصل الدم عند حقنها في الحيوانات ، كما و يسببان إنخفاضاً معنوياً في وزن الجسم في حين لم يكن لهما تأثير في وزن الكبد. و لوحظ إنخفاض مستوى الكلوكوز في الدم لدى الحيوانات المحقونة بعقار الكاربيمازول ، بينما لم يحدث تغير في مستواه بتأثير مادة بين إبيضاض-6.أما بالنسبة لأنواع الدهون فقد لوحظ إرتفاع معنوي لمستوى كل من Triglyceride(T.G) وVery low density lipoprotein cholesterol (VLDL-C) في مصل دم الحيوانات المعاملة بـ مادة بين إبيضاض6- ، في حين لم تتأثر مستويات الدهون في الحيوانات المعاملة بالكاربيمازول. وأدى حقن الحيوانات بمادة بين إبيضاض-6 و الكاربيمازول أيضاً إلى زيادة موت الخلايا اللمفية المبرمج في الدم المحيطي .

Abstract

The aim of the study is to assess the blood levels of (interleukin-6) at different intervals of time after surgical removal of impacted lower third molars.. Materials and Methods: The study group comprised 26 patients with impacted lower third molars .All extractions were performed under local anaesthesia using the inferior alveolar nerve block together with vestibular infiltration. A buccal full thickness two sided mucoperiosteal flap was raised. Sterile low-speed straight hand pieces and sterile distilled water were used for ostectomy. After tooth removal the wound was closed with 3-0 black silk suture. Three blood samples were drawn from each patient; one before the operation, the second 2hr. post operation and the third 24hr. post operation. The blood samples were collected through venipunc-ture, serum separated and stored in at -20ºC. Analysis was performed using the Enzyme linked Im-munosourbant assay ( ELISA). Results: The level of IL-6, in this study, showed an increase following surgery, in comparison with the level before operation. This increase in the level of Interleukin-6 re-mained high until 24hr after operation. Conclusion: Interleukin-6 (IL-6) is higher after surgical extrac-tion of lower third molars and remained with high levels to 24hr. after operation

Abstract

Background: Multifunctional cytokines play important and only partially defined roles in mammary tumor development and progression. Normal human mammary epithelial cells constitutively produce interleukin 6(IL-6) and a non-secreted form of tumor necrosis factor. Transformation of mammary epithelial cells by different oncogenesis is frequently associated with alterations of cytokine/ growth factor production and responsiveness. Methods: We measured levels of 1L-6 in 84 females with breast cancer and examined their correlation with clinicopathological variables including stages of the disease and estrogen and progesterone receptor expression on tumor cell.Results: Our results revealed significantly higher serum IL-6 in breast cancer patients (12.98 pg/ml) compared to a healthy group (2.05pg/ml) (P<0.05) and a direct association with different clinical stages. And we found that expression of this cytokine was inversely associated with estrogen and progesterone receptor cells.Conclusion: Serum levels of IL-6 are highly elevated in breast cancer patients and correlate with tumor progression. Assay for serum levels IL-6 can be used as predictive non-invasive tests for tumor progression in breast cancer patients; also our data suggest that cytokine could be involved in the aggressiveness of ER-PR negative breast tumors. Expression of interleukin 6(IL-6) correlates with estrogen and progesterone receptor in breast cancer patients. Keyword: Interleukin 6, breast cancer, estrogen-progesterone receptor

Abstract

Objective: To determine the relationship of serum level changes of IL-6, C-RP with retinopathy in two groups of type 2 diabetic patients (with and without retinopathy) in addition to control groups.Method and material: The study was conducted from December2012 to March 2013. On well controlled thirty five patients of type 2 diabetic retinopathy (group one) (23 male and 12 female) with age range (41-75) years. Thirty patients of diabetes without clinical retinopathy (group two) (15 male and 15 female) with age range from (40-75) years were included in this study; duration of diabetes is more than five years in both groups. Disposable syringes and needles were used for blood collection. Blood samples (5ml) were obtained from diabetic patients and control groups by vein puncture. About one ml of whole blood put in EDTA tube for estimation of HbA1c, the remaining blood samples were allowed to clot at room temperature, and then centrifuged at 3000 Xg for 10 minutes. Sera were transferred carefully and stored at -17ºC until analysis time in suitable serum tubes.Statistical analysis: The results were analyzed using Anova and student`s t–test .Statistical analysis was carried out using the Statistical Package for the Social Sciences (SPSS version 10). A p value less than or equal to 0.05 was considered statistically significant.Results: The mean sera level of IL-6 was found to be significantly elevated (p< 0.05) in DR as compared with the control group. On the other hand, no significant variations were detected in the levels of the IL-6 in the group of DNR with respect to the control group. C-RP level was found significantly elevated (p< 0.01) in patients with DR when compared with control individual, while there was no significant variation of the serum levels of C-RP in DNR (p = N. S.) and control group. Conclusion:Patients with increase serum levels of IL-6 and C-RP need more intensive ophthalmological examination to detect early retinal changes in aim of providing early management of diabetic retinopathy.

الهدف: لتحديد العلاقة بين تغيرات مستوى انتر ليكين -6 وسي ريئكتف بروتين مع التغيرات الحاصلة في الشبكية لدى مجموعتين من مرضى السكري النوع الثاني ( مجموعة مرضى سكري مع تغيرات في الشبكية ومجموعة اخرى بلا تغيرات في الشبكية )طريقة العمل: استمرت الدراسة من تشرين الثاني 2012 الى اذار 2013 . قسم المرضى الى مجموعتين , المجموعة الاولى تكونت من 35 مريض مصاب بالسكري مع وجود تغيرات في الشبكية وكانو 23 رجل و12 امراة ومعدل الاعمار كان 41-75 سنةالمجموعة الثانية وتتالف من 30 مريض ، 15 رجل و15 امراة معدل الاعمار كان 40-75 سنة وبدون تغيرات في الشبكية . فترة الاصابة بالسكري كانت اكثر من خمسة سنوات ، تم اخذ 5 مل من عينات الدم من المرضى لتحديد نسبة السكر في الدم ولاجراء بقية التحاليلالنتائج: اظهرت النتائج ان مستوى انترليكين 6 في الدم كان مرتفعا بدرجة مهمة عند مرضى السكري والذين لديهم تغيرات فى الشبكية ، بينما لم يكن هناك تغيرات مهمة بمستوى الانترليكين 6 عند مرضى السكري الذين لديهم شبكية طبيعية. كما اظهرت النتائج ان مستوى السي ريئكتف بروتين كان مرتفعا بنسبة مهمة عند مرضى السكري الذين لديهم تغيرات في الشبكية مقارنة بمجموعة المرضى الذين لديهم شبكية طبيعية الاستنتاج: مرضى السكري الذين لديهم ارتفاع في مستوى النترليكين 6 والسي ريئكتف بروتين في الدم يحتاجون الى فحص دوري مكثف لاكتشاف اى تغيرات جديدة في الشبكية حتى يتم معالجتها مبكرا.

Abstract

This study involved (50) patient’s that complaining rheumatoid arthritis from both genders (7 females and 43 males). Their ages range from (26-68) years there are taken from hospitals and clinics, in addition to the control which involve (30) person that look normal and their ages ranges from (23-54).Enzyme linked immuno-sorbent assay (ELISA) has been used to measure the level of IL-6 in serum, in which the medium of IL-6 in patients was significantly higher (p ≤ 0.01) than control group.The study show’s significant difference (p ≤ 0.01) in level of IL-6 between males and females and between age groups.

شملت هذه الدراسة (50) مريضاً بالالتهاب المفاصل الرثوي من كلا الجنسين (7 إناث و43 ذكور) تراوحت أعمارهم بين (26-68) سنة من ضمن الحالات الواردة إلى المستشفيات والعيادات الخارجية، فضلاً عن مجموعة السيطرة التي شملت (30) شخصاً من الأصحاء ظاهرياً وقد تراوحت أعمارهم بين (23-54).تم استخدام اختبار الامتزاز المناعي المرتبط بالانزيم Enzyme Immuno-sorbent assay (ELISA) لقياس تركيز IL-6 في المصل وقد كانت قيمة المتوسط الحسابي للـ IL-6 لدى المرضى أعلى وذات فرق احصائي عالي المعنوية (P ≤ 0.01) عنها في مجموعة السيطرة.وقد أظهرت الدراسة اختلاف عالي المعنوية (P ≤ 0.01) في مستوى IL-6 بين الذكور والإناث وكذلك بين المجاميع العمرية.

Abstract

Cytokines are important small protein molecules one of them is Interleukin-6 (IL-6) .Many tissues secretes IL6 like activated leukocytes .In immune system , IL6 plays main role in production of acute-phase proteins like C- reactive protein . Human labial salivary glands, contain Messenger RNA for IL-6 .Aim of study was to demonstrate the effect of exam stress onsalivary concentrations of interleukin- 6 (IL-6) in healthy secondary school students . Salivary specimens were collected from thirteen male students before and after final academic exam. Cytokine levels were measured by commercial enzyme linked immunoassay. Salivary IL-6 was insignificantly higher after final exam when compared with its level before exam. (P >0.05).students after exam have increased level of inflammatory cytokine (IL-6) in their saliva. TheIL6 level may be used for monitoring the stress.
