Open Reduction and Internal Fixation Compared to Closed Reduction and External Fixation in Distal Radial Fractures Arandomized Study of 40 Patients

Abstract

Objective: to investigate if open reduction and internal fixation would produce better outcome than external fixation of distal radial fractures.Patients and Methods: 40 patients with unstable or comminuted distal radial fracture where divided randomly into two groups using 2 methods of treatment, in group one closed reduction &bridging external fixation was used ,in group tow open reduction &internal fixation was used. The primary outcome parameter was grip strength, but the patients were followed for 1 year with objective clinical assessment, subjective outcome and radiographic examination.Results & Discussion: At one year postoperatively , grip strength was 89%(standard deviation 15) of the injured side in the internal fixation group and 75%(15) in the external fixation group. Pronation /supnation was 150 (15)in the internal fixation group &136 in(20) in the external fixation group at 1year.4 patients in the external fixation group where reoperated due to malunion as compared to 2 in internal fixation group.6 others are classified as radiographic malunion, 5 in the external fixation ,1 in the internal fixation group.Conclusion: internal fixation gave better grip strength &and better range of motion at one year, and less malunion than external fixation. No difference could be found regarding subjective outcome.

Abstract

The aim of the present study was to evaluate treatment of 20 dogs with femur fractures. Twenty (20) dogs with different ages, breeds, and gender admitted to the clinic of Department of Surgery, Anesthesiology and Radiology-Faculty of Veterinary Medicine-Zagazig University from January 2013 to December 2015 with a complaint of hind limb lameness and were diagnosed as femur fractures through clinical and radiographic examination. German shepherd dogs (85%) and immature dogs (70%) were more susceptible to femur fractures. The most common site of the fracture was the femoral diaphysis (65%) followed by metaphysis (20%). Open reduction and internal fixation techniques were applied using intramedullary (IM) pins with or without cerclage wire (40%) and bone plate (15%) for treatment of diaphyseal fractures, and two cross pins (30%) for treatment of metaphyseal and distal physeal fractures. Pin migration after complete healing (1 case) and bone re-fracture due to jumping (1 case) was the postoperative complications. It was concluded that open reduction and internal fixation methods are the proper treatment for femur fractures if the proper surgical techniques were applied.

هدفت هذه الدراسة لتقييم علاج عدد (20) كلبا يعاني من كسور عظمة الفخذ. عدد (20) كلبا من مختلف الأعمار، السلالات، والجنس وردت لعيادة قسم الجراحة والتخدير والأشعة- كلية الطب البيطري-جامعة الزقازيق في الفترة من يناير 2013 الي ديسمبر 2015 مصحوبة بشكوي عرج في الأطراف الخلفية وتم تشخيصها علي أنها كسور عظمة الفخذ وذلك من خلال الفحص الاكلينيكي واستخدام الأشعة السينية. وقد كانت سلالة الراعي الألماني (85%) والكلاب غير الناضجة (70%) أكثر عرضة لكسور عظمة الفخذ. ويعتبر الجزء الأوسط من عظمة الفخذ أكثر الأماكن عرضة للكسر (65%) ويليه الجزء السفلي للعظمة (20%). قد تم اعادة العظمة من خلال الفتح الجراحي وتثبيت الكسور داخليا باستخدام المسمار النخاعي مع او بدون استخدام سلك من الاستانلس ستيل حول العظمة (40%)، وباستخدام شرائح العظم (15%) لعلاج كسورمنتصف العظمة، وباستخدام مسمارين نخاعيين معاكسين لبعضهما (30%) لعلاج كسور الجزء السفلي من العظمة. مضاعفات ما بعد الجراحة وجدت في حالة واحدة نتيجة هجرة المسمار النخاعي بعد التئام الكسر وحالة أخري نتيجة حدوث الكسر مرة أخري بعد التثبيت. وقد خلصت هذه الدراسة الي أن الفتح الجراحي واعادة العظمة وتثبيتها داخليا هو العلاج الأمثل لكسور عظمة الفخذ في حالة تطبيق طرق العلاج الجراحية الصحيحة.

Abstract

AbstractThe basic goal of fracture fixation is to stabilize the fractured bone, to enable fast healing ofthe injured bone, and to return early mobility and full function of the injured extremity [1]. Thecomputerized tomography scan slices of the femur bone of one patient were translated to theANSYS V.(10) program and then the solid model (3-D) model was built and analysed by the finiteelement method under different loading conditions for each method of fixation. Finite elementanalysis would help to reveal stress pathways across the shaft of the femur bone and the sites of the8 screws or pins fixed at the shaft of the femur above and below the fracture site.In the internal fixation method two types of screws' materials can be studied. Firstlystainless steel and secondly cobalt chromium. The stress distributions were studied for each screwmaterial for different body weights. While in the external fixation method the stress distributions foronly stainless steel pins were studied.The stress distributions for six pins or screws fixed at the fractured bone above and belowthe fracture site for each method of fixation were studied.The von mises stresses of the cobalt chromium screws fixed internally are less than the vonmises stresses of the stainless steel for each of the body weight. The stresses of the pins fixedexternally are greater than the stresses of the same screws fixed internally.When decreasing the number of the screws or the pins the stress values will increase;therefore when using eight screws or pins the stresses are less than those when used six screws orpins for each method of fixation (internal and external).

Abstract

ABSTRACT:BACKGROUND: Fractures of the distal third of the tibia are the major cause of morbidity in patients with lower extremity injuries. Most fractures are sustained in young adults during high velocity injuries.PATIENTS AND METHODS: Forty patients with closed fractures of distal third of the tibia were treated in Al-Yarmouk Teaching hospital between May 2012 and April 2014. 20 patients had been treated with open reduction and internal fixation by heavy-duty plate and screws and were matched to 20 patients treated with closed reduction and intramedullary nailing fixation, with regard to gender, age, and the AO classification of the fracture.RESULTS: There were no significant differences in regard to: time of surgical procedures, non-union, hardware failure or deep infections between plate fixation and intramedullary nailing. Open reduction and plate fixation was associated with higher risk of: delayed union, osteopenia, ankle joint stiffness and algodystrophy. Closed reduction with intramedullary nailing was associated with higher rates of rotational malalignment and knee joint pain/stiffness. The limb length difference was of no clinical significance in all patients.CONCLUSION: Intramedullary nailing is the preferred method for treating closed distal tibial fractures but open reduction and plate fixation may provide superior results in terms of optimal alignment.

3.INTRAMEDULLARY NAILING VERSUS FIXED ANGLED BLADE PLATING FOR TREATMENT OF SUBTROCHANTERIC FEMORAL FRACTURE

Abstract

Background: The subtrochanteric fractures represent 7-44% of the fractures of proximal femur caused by low energy trauma in elderly patients or high energy trauma in younger age group. Different surgical options used to treat and fix this fractures that could be Intramedullary devices like intramedullary nail or extramedullary devices like fixed angle blade plate.Objective: Comparing the result of close reduction and internal fixation with intramedullary nail to the open reduction and internal fixation with 95-angled blade plate for treatment of subtrochanteric fractures.Methods: Prospective multicenter study was done in Al-Imamein Al-kadhimein Medical City and Al-Wasity Teaching Hospital for thirty patients with close and open (Gustilo type-1) subtrochanteric femoral fractures between December-2014 and September-2016. Eighteen patients (8 closed + 10 open fractures) treated with Intramedullary nail (IMN) were compared to twelve patients (9 closed + 3 open fractures) treated with open reduction and 95-angled blade plate fixation (BP).Results: There were significant statistical differences between the two groups. The IMN group show better outcome regarding the mean union rate time (IMN were 16 weeks while in the BP were 22 weeks). Mean hospitalization stay (IMN were 82 hours while in BP were 110 hours) and rate of infection (IMN 0% while in BP were 16%), but no statistical difference regarding the mean operation time (IMN were 1.59 hour while in BP were 1.43 hour) and functional outcome (HHS in IMN were 82 while in BP were 79).Conclusion: Closed reduction and internal fixation with Intramedullary Nail is preferable (for Close and Open Gustilo’s type-1 Subtrochanteric fractures) when compared to the open reduction and Internal Fixation with 95-angled Blade Plate.Keywords: Subtrochanteric femur fractures, closed reduction intramedullary nail fixation, open reduction internal fixation, fixed angle blade plate.Citation: Joda AI, Aldookhi AA, Abd Ali AS. Abstract

OPEN REDUCTION AND INTERNAL FIXATION WITH CORTICOCANCELLOUS BONE GRAFT FOR TREATMENT OF VOLARY MALUNITED FRACTURE OF DISTAL RADIUS Falih Waheed Hashim MB,ChB, FICMS Orthopedics, Lecturer, Department of Surgery, College of Medicine, University of Basrah, IRAQ. Abstract Malunion of the distal radius is actually a late complication, not salvageable by re-manipulation or a change of immobilization, it requires different management technique. The aim of this study is to choose a technique that corrects the deformity as much as possible, with less complications. An assessment was done for 24 patients with unilateral malunited fractures of the distal radius. The mean age was 42 years (range 25-61 years). They were treated by open-wedge volar osteotomy with corticocancellous graft from upper tibia. Both osteotomy and graft were fixed by volarly applied plate and screws. Preoperative antero-posterior and lateral wrist radiographs were obtained and fracture pattern and radio-carpal alignment were assessed. Radial length and palmar tilt were also measured. The clinical outcome was assessed depending on the modified Gartland and Werley score. Radiological assessment of the parameters was done postoperatively to detect how much these parameters were corrected. This study shows that volar open-wedge osteotomy with corticoocancellous bone graft and internal fixation is a method for correction of malunited distal radial fractures with encouraging results. About 90% of the patients were satisfied with their results regarding the correction of the deformity and improvement of wrist and hand function, as well as limited complications when it is done carefully with appropriate facilities. In conclusion, the corrective osteotomy should be considered only when there is a clear-cut indication. It is actually a method to correct the deformity, rather than treatment of symptoms, specially pain.

Abstract

Background: Proximal fractures of the humerus constitute 5-8% of all fractures although more than 80% of these heal without surgical intervention, the rate of nonunion vary between 1% and 23% in displaced non impacted fractures of the surgical neck. The most commonly used classification for these fractures is Neer´s classification. The aim of treatment is to attain a painless functional shoulder.Objectives: To compare the results of surgical versus conservative treatment of fracture neck of humorous Neer´s types II & III regarding functional outcome.Patients and methods; Prospective study including 40 patients having fracture proximal humerous Neer`s type II& III attending Al-Yermouk teaching hospital from Nov 1st 2011 to Nov 1st 2013. Twenty patients were treated surgically and twenty were treated conservatively, all the patients were subjected to early physiotherapy and followed up at 3 months then functional evaluation of shoulder was done using Constant's Scoring System; Results: The study sample included 40 patients; we got better results regarding pain, strength, range of motionand daily life activities in surgical treatment than conservative treatment. Conclusion: Early operative management with adequate reduction and early physiotherapy is worth trial to gain early functional outcome.

كسور الجزء القريب لعظم العضد تشمل 5-8% من كافة الكسور واشهر طريقة لتصنيفا هي طريقة نير والهدف من علاجها هو للحصور على كتف وضيفي خالي من الالم.اربعون مريض مصابون بهذا الكسر تم معالجتهم في مستشفى اليرموك التعليمي اعتبارا من 1تشرين الثاني 2011 الى 1 تشرين الثاني 2003, 20منهم تم معالجتم بالطريقة التحفضية و 20 بالطريقة الجراحية, جميعهم تم تعرضهم للعلاج الفيزيائي المبكر وتوبعوا لمدة ثلاثة اشهر بعدها تم تقييم الحاصل الوظيفي للكتف اعتمادا على تدرج كونسانت. تم الحصول على نتائج افضل للمرضى المعالجين بالطريقة الجراحية من ناحية الالم وقوة المفصل ومدى حركته والوضائف اليومية. عليه تبين نتيجة البحث ان العلاج الجراحي لهذه الكسور والعلاج الفيزيائي المبكر كان محاولة جيدة للحصول على وضائف الفضل للكتف.
