Abstract

background: Surgery for stress urinary incontinence has been modernized by the introduction of tension free vaginal tape technique TVT (1). Minisling introduction has been simplifying this surgical procedure.Aim: To make an assessment of the success rate of minisling for ladies with stress urinary incontinence.Method: Our study enrols 40 ladies. The mean patients age was 44 years,. 75% of them with pure SUI and 25% had mixed UI. Patient’s assessment postoperatively was made by clinical assessment 3 months and 1 year after the procedure.Result: success rate was very good 97.5%. Post operative complications was 5% retention, and 12.5% UTI. The 1-3 months follow up shows a 5% de novo urgency and a 1 year follow up shows satisfaction rate of 97.5%. Conclusion: Minisling procedure for SUI has low morbidity with low rate of complications and high success rate.

Abstract

As it been mentioned by D.R Merk in 1994 that the method of indeterminate coefficients for constructing a Liapunov function is not suitable for every problem.Anew theory give in [ 2 ] and in this paper we generalized this idea in which is applicable for more extended problems. The prove of this idea is given with example.

في هذا البحث تم تعميم نظرية جديدة لتكوين دوال ليبانوف الخاصة باختبار استقرارية الانظمة اللاخطية ذات المعاملات الثابتة. وكذلك عرض برهان لهذه النظرية مع ذكر مثال تطبيقي لها.

Abstract

Laparoscopic cholecystectomy is currently considered the gold standard for the managment ofgallbladder stones. Many hospitals have employed short stay wards for monitoring patients aftersurgery. The meaning of the early discharge as true outpatient surgery is controversial. Toachieve this objective there is a need to shorten the hours of hospital stay by: Appropriateselection criteria and discharge protocol, Peroperative technical modification and manipulation,Procedures to control pain, nausea and vomiting. Abstract

Objectives: This study performed to determine Sociodemographic characteristics of the nurses and assess their awareness concerning the procedure of hemodialysis and related complications in under treatment patients with hemodialysis in Kirkuk General Hospital.Methodology: A descriptive study, which uses the assessment approach and it was conducted on nurses from February 6th, 2011 to June 2nd, 2011, using non-probability sampling a purposive sample of (19) subject (male and female nurses) who worked at heamodialysis unite in Kirkuk General Hospital/ The City of Kirkuk. The data were collected through the utilization of a questionnaire, which was developed for the purpose of the study by using the scale (I know, Uncertain, I don’t know) . It was comprised of three parts, which included the nurses' demographic characteristics, Information related to the heamodialysis procedure and the complication of hemodialysis. Data were analyzed through the application of descriptive statistical data analysis approach (frequency, percentage, mean of scores), Awareness of the nurses in items of (Part II and III) are determined as following:- (adequate =3-2.33), ( moderate =2.32-1.67), and (inadequate=1.66-1).Results: The study had revealed that an age group (23 – 27) year represent the grater percentage of the sample which constitutes (42.0 %), and more than half of the nurses were male which represent (58.0 %). Relative to the subjects' marital status, two third (68.0%) of the sample were married. Concerning the years of employment, one third (37.0%) of the nurses have worked for (<1) year and more than two third (68%) of the nurses were college graduate.Conclusions: The study concluded that the nurses had inadequate awareness regarding catheter placement, care of (muscle cramp, chest pain, and shortness of breathing). and majority of them did not document every nursing procedure. Grand mean for total mean of scores for nurses awareness concerning the procedure of hemodialysis was (2.31), and (2.30) for the complications of hemodialysis. The study concluded that nurses awareness were moderately adequate concerning both the procedure of hemodialysis and related complications.Recommendations: A standardized written protocol, special for hemodialysis procedure and managing related complications needs to develop for the nursing staff. In addition, the nurses should participate in continues nursing education to enforce their knowledge regarding their selection (hemodialysis and related complication, and generalization the necessity of nursing documentation.

الهـدف: تهدف الدراسة الحالية الى تحديد الخصائص الديموغرافية للممرضين وكذلك تقييم معارفهم فيما يتعلق بإجراءات الغسيل الدموي و المضاعفات المرتبطة بها في مستشفى كركوك العام.المنهجيّة: أجريت دراسة وصفية استخدم فيها اسلوب التقييم ,و تم تطبيقها على الممرضين للفترة من السادس من شباط 2011 ولغاية الثاني من حزيران 2011. أختيرت عينة غرضية (غير أحتمالية) مكونة من (19) ممرض وممرضة من الذين يعملون في وحدة الغسيل الدموي في مستشفى كركوك العام في مدينة كركوك لغرض الدراسة. تم جمع المعلومات باستخدام استمارة الاستبانة التي صممت لغرض الدراسة باستخدام مقياس (اعرف , غير متاكد, لا اعرف) و كانت الأستبانة مكونة من ثلاثة اجزاء حيث تضمنت الصفات الديموغرافية للممرضين ومعلومات لها علاقة باجراءات الغسيل الدموي و المضاعفات المرتبطة بها. تم تحليل البيانات من خلال تطبيق اسلوب التحليل الأحصائي الوصفي للبيانات (التكرارات، النسب المئوية و الوسط الحسابي).حددت معارف الممرضين في الجزئين (الثاني و الثالث ) من الاستبانة كالتالي: ( كافية = 2,33 – 3 ) , ( متوسطة = 2,32 – 1,67) ،( غير كافبة = 1,66 – 1 ).النتائـج: أشارت نتائجَ الدراسة أنّ الفئة العمرية (23 – 27) مثلت النسبة الاعلى (42%) من اعمار الممرضين , اكثر من نصف الممرضين (58%) كانوا من الذكور , و ثلثي افراد العينة (68%) كانوا من المتزوجين. فيما يتعلق بسنوات الخدمة, اظهرت الدراسة بان ثلث الممرضين (37%) كانت لديهم خدمة اقل من سنة و ان ثلثي عينة الدراسة (68%) هم من خريجي الكلية.الاستنتاجات: استنتجت الدراسة بان معارف الممرضين كانت غير كافية فيما يخص( وضع القثطرة الخاصة لاجراء الغسيل و كيفية العناية بالمريض المصاب بتشنج العظلات , الم الصدر و صعوبة التنفس بالاضافة لعدم توثيق الممارسات التمريضة من قبل الملاك التمريضي) كما استنتجت الدراسة ان معارف الممرضين فيما يتعلق باجراءات الغسيل الدموي كانت متوسطة لكل من اجراءات الغسيل الدموي والمضاعفات المرتبطة بها . التوصيات: أوصتْ الدراسةَ بأنّ يصمم كتيب مطبوع عن العناية التمريضية خاصة باجراءات الغسيل الدموى بالاضافة الى وجوب اشراك الممرضين في التعليم التمريضي المستمر لتعزيز معارفهم في مجال اختصاصهم و تعميم ظرورة التوثيق التمريضي.

Abstract

AbstractObjective: The study deafs with nursing performance in the surgical wards in general hospital at Baghdad city.Methodology : A descriptive evaluation design using, observational method was carried out. Non probability (purposive) sample of (151) nurses was selected for the study and comprised all nurses who worked in general surgical wards in the four health sectors( Rusaffa , Al-Karkh, Al-Yarmok, Medical city health sector) at time of collecting the data. A check list questionnaire was constructed by the researcher for the purpose of the study; it is composed of (2) major parts, part (I) is concerned with socio-demographic data and the second part is composed of two minor parts thev concerned with availability of equipment and nursing performance. Validity of the questionnaire was determined through panel of experts. Reliability of the questionnaire was determined through computation correlation coefficient of (93.3) was statistically acceptable.The data were analyzed through the application of the descriptive data analysis approach and the inferential statistics. Results : The findings of the study indicated that more than half of the sample (66.2%) was male, the majority of the sample (71.52%) were in group (25-30) year. Most of the sample (29.8%) was training course graduates. The higher percentage (58.3%) has no training session after ward.Findings of the study had pointed that there were severe shortages in availability of equipment related to patients admission procedure, preoperative and postoperative nursing care procedures.The results also revealed that the nurses performance were inadequate related to some nursing procedures such as admission procedure, preoperative nursing care procedure and postoperative nursing procedure. Abstract

A computational investigation on the simulation and design of symmetrical double polepiece magnetic electron lens using optimization by synthesis approach has been done. The field formula and its related axial functions requaired for synthesis design of the charged particle lens have been determined with aid of Simulation environment in Matlab. The objective focal properties of the double polepiece magnetic lenses with respect to the image space have been studied under the effect of the field parameters.

اجريت الدراسة باستخدام اسوب المحاكاة الحاسوبية لتصميم عدسة مغناطيسية ثنائية القطب متناظرة بأستخدام اسلوب امثلية التوليف. وبأستخدام بيئة المحاكاة في الماتلاب تم تحديد صيغة المجال والدوال المرتبطة به والمطلوبة لتصميم عدسة الجسيمات المشحونة. تم دراسة تأثير معاملات المجال على الخواص البؤرية الشيئية نسبة الى فضاء الصورة للعدسات المغناطيسية ثنائية القطب المتناظرة.

Abstract

In this paper an optimal (Bayesian) fixedsample size procedure for selecting the better oftwo Poisson populations is proposed and studied. Bayesian decision-theoretic approach withgeneral loss function and Gamma priors are usedto construct this procedure .The numerical result of this procedure aregiven with different loss functions constant ,linear and quadratic , in one equation we canobtain the Bayes risk for the three types of theloss functions : constant , linear and quadratic .in this paper the numerical results are given byusing Math Works Matlab ver. 7.10.0 .يتضمن هذا البحث طريقة ذات حجم عينة ثابت لاختيار أفضل مجتمع من بين مجتمعيين يتبعان التوزيع البواسوني. وقد استخدم أسلوب القرار البيزي مع دالة خسارة مشتركة مع توزيع سابق للتجربة متمثل بتوزيع كاما لبناء هذه الطريقة . وتضمن البحث أيضا نتائج عددية لهذا الاجراء مع دوال خسارة مختلفة ثابتة ، خطية وتربيعية حيث من معادلة واحدة بالإمكان الحصول على الخطورة البيزية لدوال الخسارة الثلاثة : الثابتة ، الخطية والتربيعية . النتائج العددية لهذا الأجراء تم ايجادها باستخدام نظام Matlab ver. 7.10.0

Abstract

ABSTRUCT:BACKGROUND:Tibial hemimelia is a very rare deformity and problematic syndrome usually associated with variable visceral and skeletal deformities. The treatment is always difficult and challenging. Amputation was the preferred treatment option specially in complete tibial absence, however, a conservative management sometimes used in other types of the deformity or in cases of amputation refusal. OBJECTIVE: To review the methods of management of chronic tibial defects and to study the results of operative treatment of 4 patients with type ii congenital tibial hemimelia.PATIENT AND METHOD:All along thirty-two months period and after a proper clinical and radiological assessment we treated four male children with a mean age of 8.5 years who were all right sided type 2 tibial hemimelia according to Jones and Kalamchi with absence of the distal tibia. Treatment consisted of proximal fibula transfer to the lower end of the tibial stump (brown's operation) followed by Syme's amputation and prosthetic fitting.RESULTS:In all cases the transferred fibular shaft to the distal end of the tibial remnant was united within two to three months. The fibulae grew in length and the size to be tibialized and the leg increased in length as we preserved the distal fibular epiphysis.CONCLUSION:The results were acceptable as we saved and reconstructed the limb, we gained an acceptable knee function and stable prosthetic fitting that aided proper rehabilitation.

Abstract

Lung cancer is the most common cancer in men while is third highest incidence among women and is second after breast cancer in mortality. Most cases occur in older people in age 65 or older, while less than 2% are younger than 45. The pathogenesis of NSCLC is complex and developed in multistep process in which several gene mutations will occur and coordinate with each other in genotyping and phenotyping outcome. Non-small cell lung carcinomas (NSCLC) which represent 85% of all lung cancers, it represents the most common type, usually grows and spreads more slowly than other types, histologically is divided into Adenocarcinoma (ADC), Squamous cell carcinoma (SCC) and large cell carcinoma (LCC). Its incidence and mortality rates vary markedly around the world. The aim of this study was to assess Anaplastic Lymphoma Kinase (ALK) over expression in Non-small cell lung carcinomas (NSCLC) and its correlation with type, grade and stage of the tumor.Forty cases of NSCLC was included in this study, collected randomly from the period of February 2016- April 2017 from Al-Harirey Teaching Hospital in Baghdad and Al-Sadder Teaching Hospital in Al-Najaf governate. The (40) patients (22 male and 18 females) of NSCLC, their median age 58.3 years range from (34-80) years. A group of 12 cases with benign brain tissue were included as a control group. A manual Envision procedure was used in the imunohistochemical analysis (Dako Cytomation Copenhagen, Denmark). ALK over expression was positive in (12.5%) of NSCLC, ALK immunohistochemical staining was more in ADC than SCC and its not correlated with grade and stage of NSCLC. These finding support the role of ALK in carcinogenesis of ADC and less commonly SCC of the lung.
