Abstract

Aim: Demonstration of a new method which is liner measurement to record the amount of caninerotation after orthodontic treatment, in addition to the ordinary angular method for measuring caninerotation. Materials and Methods: Forty five patients (20 males and 25 females) were selected: Thoseneed extraction of the first premolars for correction of malocclusion. Their age varied from 12–23years. A study model was taken for them pre and post treatment, then measurements were done directlyon the cast and other measurements indirectly converted on a paper to measure other variables.Results: There were no significant differences among age groups and between the sexes for the angularand linear methods. Also, there was insignificant canine rotation in the mesial or distal sides toward oraway from the reference line within the different groups; this clearly seen in the linear method. In theangular method, also canine rotation can be seen within the age groups but not demonstrated the side ofrotation like the linear method. Conclusion: Canine rotation occur within the treatment work in the twomethods of measuring canine rotation, but this research proved that the linear direct measurementwhich is the new method followed in this work was more beneficial and accurate for clinical work thanthe indirect angular measurements.

Abstract

Aims: To evaluate the amount of rotation, tipping and extrusion using different wires and to Estimate the difference in the amount of tipping, rotation and extrusion between Stainless steel and Teflon ligature in different wires. Materials and Methods: The standardizing criteria were all Typodont teeth situated in well–aligned, covered and immobilized by the acrylic bite except canine; The available space for canine sliding was (14 mm) measured by digital vernia. Elastic chain exerting 180 gm of force on canine measured carefully by tension gauge. In both vertical and horizontal direction a photographs that were taken for Typodont using digital camera. The angle between bite plane extension bar and Canine extension bar were (90o) measured by protractor directly on the photograph. Preformed band with its attachments, ready made stainless steel, composite coated and TMA arch wires all were (0.018×0.025) with the use of stainless steel and Teflon ligature, Standard titanium spring 11 mm length, Typodont components, within six types of connection the two types of ligation material had been applied to the three different wires then the 1st premolar space tend to be closed by distal canine displacement then the resultant rotation, tipping and extrusion were measured. Statistical Descriptive analysis: One–way and, Two–ways Analysis of Variance were done to detect the variability between methods and which is the best. Results: Three important results showed in the study: First, canine sliding over the composite coated arch wire with the use of stainless steel ligature gave rise to significant decrease in rotation, tipping and extrusion when compared with other methods. Second, stainless steel ligature when compared with Teflon ligature of the same corresponding method gave rise to a significant decrease in the degree of rotation tipping and extrusion. Third, Composite coated arch wires showed lowest degree of rotation then followed by TMA arch wires ordinarily this is due to their rough surface. The largest rotation seen in the stainless steel arch wire this is due to its smooth surface texture, and the same thing for the tipping of the composite coated wires that gave rise to the lowest degree of tipping followed by the stainless steel wires, these result possibly due to their stiffness then higher tipping seen at TMA wires ordinarily a result of their flexibility this sequence were the same for both types of ligations. Conclusions: Best method of space closure is the use of either composite coated arch wire or stainless steel arch wire with stainless steel ligature that give us highest resistance to tipping, lowest extrusion and relatively little rotation when compared with other methods.

Abstract

Aims: To determine the effect of different points of force application on dimensional positions and the ratio of space closure of maxillary canine using sliding mechanics. Mater– ials and Methods: The study included eight groups which were categorized according to the differences in the points from which the retraction force was applied. A 180 gm was applied by short elastic chain to retract the right maxillary canine on 0.0180.025'' rectangular stainless steel (SS) wire and along 13mm available space. In both vertical and horizontal direction, photographs were taken by digital camera and the angle between canine extension bar and bite plane extension bar was measured by protractor to determine tipping and rotation whereas rate of space closure was measured by digital vernia. Results: The results showed that the maximum rate of space closure was achieved when elastic chain was attached between molar hook to canine hook and the minimum degree of tipping was achieved when elastic chain attached between premolar bracket to canine hook while minimum rotation occurred when the attachment was between premolar hook to canine hook. Conclusions: Changing the distance between the points of force application significantly affects the rate of space closure, tipping and rotation. The use of hook significantly increases the rate of space closure and decreases tipping and rotation

Abstract

Aims This retrospective cephalometric study was designed to evaluate and correlatethe hyoid bone position and angulations in two groups of subjects exhibitingvertical and horizontal mandibular rotation.Material & method: Each group consisted of 50 subjects (25 males and 25 females)with skeletal class I pattern. Cephalometric data were obtained and fourmeasurements were used to locate the hyoid bone in 3 aspects 1. Vertical, 2.Horizontal and 3. Angular. Thirteen angular and linear measurements were usedto evaluate mandibular rotation and facial morphology.Result: There were statistically significant differences in vertical position, horizontaldistances and angulations of hyoid bone measurements between the vertical andhorizontal group subject (P<0.05). These measurements were significantlycorrelated with mandibular rotation angles.Conclusion: In vertical mandibular rotation group subjects, the hyoid bone showedto be more inferior and posterior position with downward inclination. While inhorizontal mandibular rotation group subjects, the hyoid bone was more superiorand anterior position with relatively upward angulations.

Abstract

Background: Facial type plays an important role in the information of an orthodontic treatment plan and prognosisthat is whether an individual has long, short or average face and it is not possible to apply the same norms andtreatment objective to cases with different facial patterns. This study aims to determine the characteristics of Bimler'sfacial types (dolichoprosopic, mesoprosopic, leptoprosopic) and their relationship with mandibular rotation in Iraqiadults with different skeletal relations.Materials and methods: The sample consisted of 230 digital true lateral cephalometric radiographs of age rangebetween 18-30years (104 males and 126 females), the sample was classified according to ANB angle into threeskeletal classes (class I, class II and class III) and each class was classified depended on PP-MP angle into three faces.Thirteen cephalometric measurements (five angular, seven linear and one ratio) were measured for each individualradiograph using AutoCAD program 2006.Results: The mean values of all measurements for males were significantly higher than females, except for the GA,UGA and LGA, The mean values of GA, LGA, AFH and LAFH were high in leptoprosopic face, while their valuesdecreased in dolichoprosopic face and the reversed were found with JR, PFH, RL and MAXL, moreover all thesemeasurements revealed a highly significant difference between the three facial types.Conclusions: The mandibular rotation was not changed in all skeletal classes that belong to the same facial type asit did not affected by the anteroposterior relation within the same profile

Abstract

This study is attempted to find out if the actual and effective symphysis are correlated with the measurements of mandibular rotation and can be used to detect the direction of mandibular growth. Ninety five Iraqi adult patients (54 females and 41 males)with an age ranged between 18-31 years collected among patients having Cl I skeletal and occlusal relations and full permanent dentition regardless the third molars were chosen for this study. Each person was subjected to clinical examination and digital true lateral cephalometric radiograph. The radiographs were analyzed by using AutoCAD 2007 computer program to measure the two symphyseal measurements with eleven measurements for mandibular rotation. Descriptive statistics were obtained from the measurements of both genders; independent samples t-test was performed to evaluate the gender differences, while Pearson’s correlation coefficient test was used to find the correlation of actual and effective symphysis with the mandibular rotation measurements. Actual and effective symphysis were not significantly differing between genders, while facial heights were significantly higher in males. Regarding the mandibular rotation angles, only SN-MP, saddle (N-S-Ar), and the sum of the posterior angles (Sum PA)were significantly higher in females, while PP-MP, FMA, articular (S-Ar-Go), and gonial (Ar-Go-Me) angles showed non-significant higher mean values in males. For both genders and the total sample, the actual symphysis didn’t show any significant correlation with the facial heights and the mandibular rotation angles, on the contrary the effective symphysis showed in both genders and the total sample significant positive correlations with Jarabak ratio, and significant negative correlations with SN-MP, PP-MP, FMA, and the sum of the posterior angles. In males and the total samples, the saddle angle (N-S-Ar) significantly positively correlated with the effective symphysis, while gonial angle (Ar-Go-Me) was significantly negatively correlated with it. Articular angle (S-Ar-Go) only significantly negatively correlated with the effective symphysis in the total sample. Effective symphysis is a good predictor of the direction of mandibular rotation, while actual symphysis is a poor predictor.

مِن مسائل النحو العربيّ التي ترِدُ كثيرا عند النحويين والمفسّرين أنّهم قد يجدون في الآية القرآنية فعلاً يُعدَّى بغير فعل التعدية المعروف معه في العربية ، فيلجأ فريقٌ منهم إلى أن يقول : معنى حرف التعدية هذا يُراد به معنى حرف آخر غيره ، إذ ذهب الكوفيون إلى أنّ حروف الجر في العربية ينوب بعضها عن بعض فقوله تعالى : (ولأصلِّبنَّكم في جذوعِ النخل) معناه على جذوع النخل ، على حين ذهب البصريّون إلى أنّ أحرُف الجر لا ينوب بعضُها عن بعض ، وما أوهم ذلك فهو عندهم على تضمين فعل التصليب معنى فعل يتعدى بذلك الحرف ، أو يكون التركيب مؤولا تأويلاً يقبله اللفظ ، فقالوا : شُبّه المصلوب لتمكُّنه من الجذع بالحالّ في الشيء .ومن ثمّ صارت المسألة ظاهرة لدى المحدثين فألّفوا فيها كُتبا تحمل عنوان (ظاهرة تناوب حروف الجر) أو (ظاهرة التضمين) ، أو أشباه هذين . فتعدّدت المصطلحات تبعا للخلاف بين الفريقين في هذه المسألة ، أينوب حرف الجر عن غيره أم إنّ الفعل هو الذي يضمّن معنى آخر فيُعدَّى تعديته ؟ولمّا كثرت الآيات القرآنية التي فُسّر فيها حرف الجر (في) بأنه ينوب عن (على) انتخب البحث واحده منها للبحث والتحليل وهي قوله تعالى (وَلَأُصَلِّبَنَّكُمْ فِي جُذُوعِ النَّخْلِ) [ طه : 71 ] إذ تعدّ هذه الآية الشاهد المبرَّز على هذا النمط من النيابة الحرفية لديهم جميعا . فطوّل البحث في الاستدلال على نقض الاستشهاد بها على مجيء (في) بمعنى (على) . ونقض أن يكون فعل التصليب متضمنا معنى فعل الغرس والدفن .والبحث محاولة لنقض النيابة الحرفية في القرآن الكريم وذلك برد الرأي الكوفي القائل بالنيابة الحرفية ونقض الرأي البصري القائل بالتضمين، ففي قوله تعالى : (ولأصلِّبنَّكم في جذوعِ النخل) خلص البحث إلى أنّ المراد بالجذوع أسافل النخل لا سيقانها الضخمة التي تحمل الرأس ومن ثم يكون التصليب فيها ظاهرا حقيقة دونما مجاز ولا تضمين أو تحوّل في مدلول حرف التعدية . وأما تلك السيقان الضخمة التي يعتمد عليها رأس النخلة فقد سمّاها القرآن الكريم أعجاز النخل كما في مشاهد القصص القرآني لهلاك قوم عاد في سورتي [القمر : 20] و [الحاقة : 7] .

ELLIPTICAL ROTATION FLAP FOR COMPLICATED PILONIDAL SINUS

Abstract

Background:Pilonidal sinus is one of the most common sinuses seen in general surgical practice and usually seen in the natal cleft. Postoperative wound complications have always been the main cause of concern followed by the risk of recurrence in the surgical treatment of the pilonidal sinus disease. Various techniques evolved so far mainly aimed at solving these problems. This clinical study conducted to discuss the results of elliptical rotational flap in chronic pilonidal sinus.Objective:To assess the outcome of patients operated by rotational flap procedure in chronic pilonidal disease in term of healing time, complications and recurrence rate.Methods:The study was conducted in two different places from December 2007 till April 2011. Eighty two patients with chronic pilonidal sinus were treated by rotation flap. The setting was in two different places, at Nizwa General Hospital, Sultanate of Oman and from Al-Imamain Al-Kadhemain Medical City, Baghdad. Evaluation of operative and post-operative results, complications and recurrence in addition to demographic data.Results:Out of 82 Patients included in study, 2 cases (2.4%) failed to heal due to partial necrosis of the flap and underwent redo surgery, while 80 patients (97.4%) were cured completely and no further surgery was needed. Eighteen patients (22%) went home in the end of same day and the rest (78%) discharged in second day. Forty six Patients (56%) had dry dressing till suture removed after 8 days, while 20 Patients (24.4%) continued to discharge serosangunious fluid and spent more than 10 days to heal. Eight Patients (9.8%) developed seroma needed further time to becomes dry and the last 6 Patients (7.3%) developed wound infection and treated by proper antibiotics and surgical drainage in some case.Conclusion:Flap procedures may be promising especially in complex and recurrent pilonidal diseases as it is simple to be done and carry low recurrence rate and relatively shorter hospital stay in comparison to other surgical procedureKeywords:Chronic pilonidal sinus, elliptical rotation flap.

Abstract

It is the central political dilemma in Morocco since getting the 1956 independence to reconcile the reality of the ruling monarchy and democracy, as the correlation between them is of Tnazeia dimension, and has been the result dominant feature of the political landscape in Morocco even after the appointment of the so-called (rotation Government) in 1988 in order to come to terms with the basic components of the Moroccan opposition party.

تكمن الاشكالية السياسية المركزية في المغرب منذ الحصول على الاستقلال عام 1956 في التوفيق بين واقع الملكية الحاكمة وتحقيق الديمقراطية ، اذ ان التلازم بينهما يكتسي بعدا تنازعيا ، وظل بالنتيجة السمة المهيمنة على المشهد السياسي في المغرب حتى بعد تعيين ما سمي ب ( حكومة التناوب) عام 1988 من اجل التصالح مع المكونات الاساسية في المعارضة الحزبية المغربية.
