Abstract

BackgroundSemen samples are prepared for assisted reproduction by selecting a population of highly motile, morphologically normal sperm and removing the seminal plasma, leukocytes and bacteria. Culture media provide the spermatozoa with needs that maintain optimal function of spermatozoa to give rise excellent results during semen preparation.ObjectiveThe aim of this study was to investigate the effect of alcoholic extraction of turmeric (AET) on sperm parameters during in vitro sperm activation (ISA) for asthenozoospermic (AZ) and oligoasthenozoospermic (OAZ) patients.MethodsSeventy four infertile patients were included, and classified into two groups according to their sperm parameters. Semen sample was divided into 3 aliquots. One mL of Earl's medium either alone (control group) or supplied with one concentration of AET (5 µg/mL or 10 µg/mL) was over layered the pellet, and the three tubes were incubated at 37 oC for 30 min in air incubator. Sperm concentration, motility, grades activity, progressive motility, normal morphology and agglutination were assessed pre- and post-activation in vitro.ResultsResults revealed an enhancement of most sperm parameters for control and both treated groups post-activation as compared to pre-activation. Post-ISA, sperm parameters for both treated groups were better than the control group. However, best results for improvement of sperm parameters were assessed within treated group (5 µg/mL of AET).ConclusionsThe lower concentration of alcoholic turmeric extraction enhanced human sperm parameters during ISA without any harmful effects on sperm physiology. The results are also useful as a guide for further standardization of turmeric extracts used for pharmaceutical purposes in the techniques of assisted reproduction.Key words Male infertility, Sperm activation, Turmeric.

Abstract

Current study used the acridine orange test to investigate the effect of two techniques of human sperm preparation involved direct swim up technique and centrifugation swim up technique on the integrity of sperm chromatin structure, and also the supplementation of two concentrations of Human serum albumin (HSA) in the culture medium that has been used in the previously mentioned methods. Assay results demonstrated that, both techniques reduce the percentages of isolated or activated spermatozoa with fragmented DNA as compared with their percentages pre-preparation, whither this decrement was significant or not (P=0.05). Also we found that using culture medium supplemented with concentrations of HSAenhance the isolation of spermatozoa with intact DNA, and (10%)HSA was the better.Therefore, the present study concluded that neither direct swim up nor centrifugation swim up techniques with single centrifuge step (559g) for 8minutes can cause further DNA damage in the activated spermatozoa by both, also the involvement of HSA in sperm preparation media enhances sperm parameters and the isolation of spermatozoa with intact DNA in-vitro.

تهدف الدراسة الحالية الى معرفة تأثير تقنيتين من تقنيات تحضير النطف البشريةوهما تقنية السباحة المباشرة(Direct swim up)وتقنية سباحة النطف المنتبذة مركزياً(Centrifugation swim-up) على بنية الصبغين للنطف وكذلك تأثير تضمين تركيزين من زلال المصل البشري في الوسط الزرعي المستخدم في التحضير بالطريقتين الأنفة الذكر , باستخدام فحص (Acridine orange).أظهرت نتائج الفحص ان كلا الطريقتين قد خفضت من نسبة التجزؤ في دنا النطف المفصولة او المنشطة بالمقارنة مع نسبها قبل التنشيط سواء كان هذا الانخفاض معنوياً أو غير معنوي(P=0.05) ,وان تضمين تراكيز الزلال الى الوسط المستخدم في التحضير يزيد من المحافظة على دنا النطف المفصولة والأفضلية كانت للتركيز (10%) من زلال المصل البشري. لذلك نستنتج من الدراسة الحالية بأن تقنية سباحة النطف المنتبذة مركزياً التي تتضمن خطوة طرد مركزي مفردة وبقوة (559g) لمدة 8 دقائق, و تقنية السباحة المباشرة كلاهما لايسببان ضرراً إضافيا للنطف المنشطة في المختبر, و ان وجود زلال المصل البشري في وسط التحضير يعزز من مؤشرات النطف بالإضافة إلى انه يزيد من فرصة الحصول على نطف سليمة ألدنا بعد التنشيط.

Abstract

This study was aimed to investigate the effect of sperm parameters on the response to in vitro sperm activation (ISA) and to compare sperm parameters according to the age and type of infertility.One hundred twenty-eight subjects employed in this study, with age range (18-49) years and duration of infertility (1-28) years. From each subjects semen sample was obtained and seminal fluid analysis (SFA) was done. Simple medium for assisted reproductive techniques; (SMART) was used for in vitro sperm activation by direct swim-up technique and centrifugation Swim-Up technique. Sperm concentration, sperm motility (%), sperm grade activity (%), normal sperm morphology (%) and sperm agglutination (%) were assessed. Crude data were statistically analyzed. From the results of present study, it was appeared that most sperm parameters were significantly (P<0.05) enhanced post-activation in vitro as compared to pre-activation using SMART medium by direct swim-up technique and centrifugation Swim-Up technique. Sperm concentration for post-activation was significantly reduced (P<0.05) compared to pre-activation groups and the percentages of progressive sperm activity with grade A, total progressive sperm motility (grades A and B) and normal sperm morphology were significantly increased (P<0.05). Based on the results of this study, it can be concluded that sperm motility and concentration were considered most important parameters that have the importance of activation and sperm morphology one of the sperm parameters that have role of the influence on the sperm qualities after activation.

صممت هذه الدراسة لتقييم مدى تأثير متغيرات النطف على الاستجابة للتنشيط خارج الجسم ولمقارنة متغيرات النطف مع العمر ونوع العقم. شارك في هذه الدراسة مئة وثمانية وعشرون شخص، وكان متوسط أعمارهم (18-49) وفترة العقم (1-28) سنة. أخذت عينة السائل المنوي من كل مريض واجري لها فحص السائل المنوي. استعمل الوسط الزرعي البسيط للتقنيات المساعدة على الإنجاب لتنشيط النطف خارج الجسم باستخدام تقنية سباحة النطف للأعلى مباشرة وتقنية الطرد المركزي. تم تقيم تركيز النطف والنسبة المئوية للنطف المتحركة والنسبة المئوية لنشاط النطف والنسبة المئوية لشكل النطف الطبيعي والنسبة المئوية لتكتل النطف.كما تم تحليل النتائج الأولية إحصائيا. أظهرت نتائج الدراسة الحالية تحسن معنوي (P<0.05) في اغلب متغيرات النطف بعد التنشيط خارج الجسم عند المقارنة مع قبل التنشيط باستخدام وسط سمارت وتقنية سباحة النطف للأعلى مباشرة وتقنية الطرد المركزي. تركيز النطف بعد التنشيط انخفض بشكل معنوي( P<0.05) عند المقارنة مع قبل التنشيط والنسب المئوية لكل من الحركة التقدمية (A) للنطف والحركة التقدمية الكلية((A+B للنطف وشكل النطف الطبيعي أظهرت زيادة معنوية(P<0.0).لذلك نستنتج اعتمادا على معطيات هذه الدراسة بأن حركة النطف وتركيزها من أهم الصفات التي لها أهمية بالتنشيط خارج الجسم وشكل وشكل النطف الطبيعي واحد من الصفات التي لها دور بالتأثير على صفات النطف بعد التنشيط خارج الجسم.

Abstract

BackgroundPretreatment of the semen samples of the asthenozoospermia patients with certain sperm I stimulant agents prior to use for assisted reproductive techniques (ART) has been reported to I have beneficial effects on fertilization rates.ObjectivesInvestigating the influence of addition of different concentrations of oxytocin (OT) to semen I samples of asthenozoospermic patients for in vitro sperm activation as a new method for I stimulation of sperm motility in these patients and exploring the concentration of OT that I harvests a maximal number of motile sperms.Methods:Simple layer technique for in vitro sperm activation using culture medium enriched with OTwasIstudied in asthenozoospermic patients (No. 90). Semen samples were examined before spermIactivation, and then they were divided into three equal groups. In the first group, threeIconcentrations of OT (2,4, and 6 lU/ml) were applied. Based on the results of the mentionedIwork, OTat the concentration of 2 lU/ml was applied with sperm preparation medium (SPM) andI without in the second and third groups, respectively.ResultsResults showed that there was a significant improvement in the percentage of sperm motility and I sperm gradeactivity of the OT-treated samples atthe concentration of 2 lU/ml ascompared to the I control samples. Moreover, there was a tendency for a decrease in sperm grade activity with I increase OT concentration. The outcome of sperm activation in presence of SPM revealed a I significant increase in the sperm motility characteristics (the percentage of sperm motility and I sperm grade activity) in the control and OT- treated samples and the best improvement in these I sperm motility characteristics was observed in the OT-treated samples. When SPM was absent, I a significant improvement in the sperm motility characteristics was observed only in the OT- I treated samples.ConclusionThe results of this study indicate that OT at the concentration of 2 lU/ml has got a stimulatory I effect on sperm motility characteristics; also, the use of SPM succeeds to support this effect

Abstract

Background:Semen preparation techniques were developed to separate motile sperm that aremorphologically normal from seminal plasma to optimize successful assistedreproductive technology cycles which seem to be the most effective options in casesof a male factor infertility.Objective:The objective of the present study was to compare between outcomes of two spermpreparation techniques for asthenozoospermic patients includes:1-Centrifugationswim up technique.2-Glass wool filtration technique.Subjects, Materials and Methods:Fifty three infertile males were participated in this study during their attendance tothe Infertility Clinic at High Institute for Infertility Diagnosis and AssistedReproductive Technologies; Al- Nahrain University. Semen samples were collectedand SFA was done according to WHO (2010 and 1999). Each semen sample wasdivided into two aliquots. The first one prepared using centrifugation swim uptechnique, while the other one prepared using glass wool filtration technique thensperm parameters were assessed for both techniques and the results were statisticallyanalyzed.Results:After in vitro sperm activation using both techniques, there was significantreduction (P<0.05) in the sperm concentration, significant improvement (P<0.05) inthe percentages of sperm motility and morphologically normal sperm whencompared to pre-activation. Present study appeared that the glass wool filtrationtechnique resulted in significantly (P<0.05) better results for sperm concentrationand total number of progressive motile sperm than the swim-up technique.The present study proved that there was significant (P<0.05) improvement in spermparameters (increment for sperm motility (%), progressive sperm motility (%), totalnumber of progressive motile sperm and normal sperm morphology (%), whilereduction for round cell count and sperm agglutination percent) for all cases usingglass wool filtration techniques. In contrast, there was failure of sperm activation for10 cases using centrifugation swim up technique.Conclusions:From results of the present study, the sperm parameters outcomes using glass woolfiltration technique was superior to the outcomes of centrifugation swim uptechnique when prepare semen of asthenozoospermic patients.

Glycyrrhizaglabm(Gg) is a herbal medicine used in the treatment of different syndromes. Recently, the effect of this plant on the reproductive system has been studied. Abstract

BackgroundThe effect of L-carnitine (LC) on the male infertility have been previously reviewed. However there are few studies about the effect of LC on sperm function parameters in vitro.ObjectivesThe objective of this study was to improve certain sperm function parameters in vitro of asthenozoospermic infertile men using LC as a motility stimulant substance which may enhance the medium used for assisted reproductive technologies (such as artificial insemination) in future.Methods:Semen was collected from 50 infertile men involved in the current study. Each semen sample was divided into two portions. One part was considered as a control and in vitro activated by using culture medium only. The other portion was considered as treated group and in vitro activated by adding LC (0.5mg) to the culture media. Certain sperm function parameters were examined before and following in vitro activation using simple layertechnique.ResultsThe results revealed highly significant increment (P<0.01) in the percentage of total sperm motility grades (A+B+C) and progressive sperm motility grade (A) with a significant improvement (P<0.05) in the percentage of progressive sperm motility grade (B) and morphologically normal sperm when using LC medium in comparison with control medium after 30 minutes incubation.ConclusionIt is concluded from the results of the present study that adding 20% LC to the culture medium of the in vitro sperm activation leads to an improvement in certain sperm function parameters.

Abstract

BackgroundThere are many factors and media affecting sperm characters which are defined by classical I parameters namely, concentration, motility and morphology, according to standard World I Health Organization (WHO).ObjectivesTo study the effect of incubation time on certain sperm function parameters following in vitro I centrifugation technique by sperm wash,FertiCult™ medium for the purpose of assisted I reproductive techniques (ART) in couples presenting sub-fertility.Methods;Two groups of 40 semen samples (groupl: fertile men with normal semen parameters and group I 2): abnormal semen parameters) underwent sperm activation test (SAT) in the ART laboratory, I High Institute of Infertility Diagnosis & ART/AI-Nahrain University. Semen analysis was I assessed. In standard WHO (1999).Wash and spin activation technique by FretiCultTM I medium was done for all samples. The tubes which was sealed, inclined vertically and kept at I 37°C for 30 minute. The supernatant contained actively motile sperms was transferred to I another test tube to separate that active sperm from pellet and record sperm parameters after IResultsThere is a significant (p<0.05) improvement in certain sperm parameters after 30min. of I incubation following SAT in both groups compared to before activation and following 30min. I incubation in SAT. Certain sperm characters are significantly decline in group 2 after 1 hr. I incubation compared to before activation.ConclusionsSperm activation test is very important method to perform before undergone artificial I insemination (AIH) . Incubation of prepared sperm in vitro by sperm wash, FertiCult medium is I benefit for the firs thalfhourtoallsemen samples to have a success in artificial insemination .
