Abstract

ABSTRACT:BACKGROUND:With the use of diagnostic laparoscopy widely accepted in the setting of the non palpable testes, now day's laparoscopic orchidopexy is an efficient and logical alternative to open orchidopexy.OBJECTIVE:We reviewed the efficacy of laparoscopy for diagnosing the intra abdominal testes.Also we review the efficacy of primary orchidopexy, one stage and two stages laparoscopic orchidopexy for the management of the intra abdominal testes.PATIENTS AND METHODS:Prospective Study includes 20 boys with 23 impalpable testes (3 patients were bilateral). underwent laparoscopy for a non palpable testes at urosurgical department between June 2007 and February 2009.Their age ranged between 3years and 7 years. Eight patients had primary laparoscopic orchidopaxy without division of spermatic vessels .twelve patients had one stage Fowler Stephen method, and Two patients had two stages Fowler Stephen method.RESULTS:During diagnostic laparoscopy: Bilateral abdominal testes was found in 3 patients, Left intra abdominal tests was found in 10 patients, while right intra abdominal testes was found in 7 patients .With the use of laparoscopic orchidopexy : twenty two testes were brought down to the scrotum: sixteen testes (72.7%) brought down to the normal scrotal site, while six testes (27.2 %) were placed at high scrotal position. Orchidectomy was done for one (4.3%) intra abdominal testis.CONCLUSION:Laparoscopy is extremely effective for diagnosis and treatment of patients with non palpable testes.Laparoscopic orchidopexy is the logic extension of diagnostic laparoscopy for intra abdominal testes.

Abstract

ABSTRACT: BACKGROUND: Diabetes mellitus is a chronic metabolic disorder characterized by a high blood glucose concentration (hyperglycemia ) which is due to insulin deficiency and / or insulin resistance . As a consequence of the metabolic derangements various complications develop , so the aim of this investigation is to study(histological ) the role of cinnamon – oil on the testis tissue of alloxan – induced diabetes rat . METHODS AND METHODS : Forty -six male rats were used and divided into 3 groups : group I , 6 = animal control . group II , 20 = treated with alloxan and the 3 rd group , 20 = treated with alloxan + cinnamon . RESULTS : Testis of group I revealed normal distribution of seminiferous tubules and spermatozoa filled the lumen of these tubules . Group II showed reduction in number of seminiferous tubules , and absence of spermatozoa in their lumen , besides the significant decrease in testis weight . Group III elicited retrained these changes to approximately its normal view . CONCLUSION : This herbs ( cinnamon ) decrease or protect the destructive effect of alloxan on testis tissue

Abstract

Abstract:In the present work, 25 male Balb/c mice have been used ( 5 mice are not infected with Echinococcusgranulosus and untreated with cisplatin used as a control group),(10 infected with Echinococcus granulosusuntreated mice) and (10 infected with Echinococcus granulosus treated with Cisplatin 100 mg/m2 weekly forthree weeks),these 20 mice considered as test group. the histopathological study of testis specimen show thatthe infected untreated groups have no histopathological changes in comparism to the control group, while theinfected and treated group shows significant histopathological changes such as that the seminiferrous tubulesdestroyed completely, there is loss of many seminiferrous tubules, extensive necrosis in rete testis and thecells of the efferent duct are flat and attenuated arrangement singly or in clusters with round nuclei.

الخلاصة:Echinococcus عرضت للإصابة بطفيلي (Balb/C( الد ا رسة الحالية شملت ) 52 ( من الفئ ا رن نوعاعتبرت كمجموعة سيطرة ) 01 ( فئ ا رن مصابه وغير معالجة cisplatin وزعت الى ( 2( فئ ا رن غير مصابه وغير معالجه بعقار Granulosusبجرعة 011 ملغم / م 5 أسبوعيا ولمدة 3 أسابيع واعتبرت هذه العشرين فأ ا ر لتمثل مجموعة Cisplatin و ) 01 ( فئ ا رن مصابة عولجت بالاختبار. تبين من الد ا رسة النسيجية المرضية لمقاطع الخصى المأخوذة من الفئ ا رن المصابه وغير معالجةعدم وجود أية تغي ا رت نسيجيةبالمقارنه مع مجموعة السيطرة في حين أظهرت مجموعة الفئ ا رن المصابه والمعالجه تغي ا رت في نسيج الخصى شملت تلفا كامل للنبيبات الناقلةفي حين )Rete testis( وفقدان العديد منها, كما تم ملاحظة تنخ ا رت واسعة في الشبكة الخصوية )Seminiferrous tubules( للمنيبتسطحها الرفيع والمرتبة بشكل مفرد او بشكل مجاميع وذات انويه مدوره. )efferent ducts( أظهرت الخلايا المكونة القنيات ألصادره

Abstract

ABSTRACTBackground and objectives: Recent advances of Invitro Fertilization (IVF) and Intracytoplasmic Sperm Injection (ICSI) combined with the progress in sperm retrieval from testes have provided a hope for azoospermic patients to become fathers. The aim of this study is to describe the cytological features of the cells aspirated from testes of patients with azoospermia and to select those who have mature spermatozoa in their aspirate for assisted fertilization.Design and setting: A descriptive study (case series) conducted in private clinic from Jan. 2007 to Sept. 2012.Patients and methods: Under local anasthesia Fine Needle Aspiration Cytology (FNAC) was performed on 250 patients whose sperm count was zero in at least three consecutive semen samples.The size of the testes was measured and recorded for each case. The cytological features of the aspirated cells were described and the patients who have mature spermatozoa were identified for assisted fertilization.Results: Normal spermatogenesis was found in 54 patients (21.6%), complete maturation arrest at primary spermatocytes in 95 patients (38%), complete maturation arrest at secondary spermatocytes in 32 patients (12.8%), germ cell aplasia in 62 patients (24.8%), and atrophic testis in 7 patients (2.8%). The mean age of the patients was 29.8 years. Regarding the greatest dimension of the testes, there was a significant difference in the greatest dimension of atrophic testes and in testes with germ cell aplasia, from the other groups. The procedure was well tolerated by the patients and no complications were encountered. Conclusions: The technique can differentiate obstructive from intrinsic causes of azoospermia. It is simple, quick, cost effective and free from complications.Keywords: FNAC, testis, infertility azoospermia.

الخلاصةالخلفية والأهداف: التطور الحاصل في الإخصاب في المختبر وزرق النطف داخل السايتوبلام بالإضافة إلى التطور في الحصول على النطف من الخصية وفر فرصة لمرضى إنعدام النطف لكي يصبحوا آباء. والهدف من هذه الدراسة هو وصف الخلايا الناتجة عن الرشف بالإبرة الدقيقة وإختيار من لديهم نطف ناضجة للإخصاب المساعد. التصميم والمكان: دراسة وصفية أجريت قي مختبر الباحث الخاص من كانون الثاني 2007 لغاية أيلول 2012.المرضى والطرق: تحت التخدير الموضعي تمت عملية الرشف بواسطة الإبرة الدقيقة لــ 250 مريض يعانون من عدم وجود نطف في السائل المنوي لثلاث فحوصات على الأقل بواسطة السرنج والإبرة الدقيقة23 G ، ثم سحب الخلايا من الخصية وعمل مسحة، ثم ثتبيت المسحة في كحول أثيلي تركيز 95%، ثم صبغها بصبغة هيماتوكسلين أيوسين. وتم وصف الخلايا المسحوبة وتم تشخيص المرضى الذين لديهم نطف ناضجة لغرض إجراء تخصيب مساعد. النتائج: كانت عملية تكوين النطف كاملة في 54 مريض 21,6%، وتوقف كامل لإنتاج النطف في مرحلة الخلايا النطفية الأولية في 95 مريضا 38%. التوقف الكامل لإنتاج النطف في مرحلة الخلايا النطفية الثانوية في 32 مريضا 12,8 %. عدم وجود خلايا بزرات النطاف في 62 مريضا 24,8 %، خصية ضامرة في 7 مرضى 2,8%. وكان المعدل العمري للمرضى 29,8 سنة. بالنسبة للأبعاد القصوى للخصية في حالة ضمور الخصبة وإنعدام الخلايا النطفية كانت تختلف إحصائيا" من المجاميع الأخرى. كانت عملية السحب متحملة من قبل المرضى ولا يوجد مضاعفات. الاستنتاج: بواسطة هذه الطريقة يمكن معرفة أسباب إنعدام النطف (الإنسداد أو أسباب في الخصية). كانت الطريقة سهلة وسريعة ولم تكلف كثيرا". وكانت خالية من المضاعفات.

Abstract

Morphologically,the testes of adult duck was suspended by mesorchium with the roof of the the abdominal cavity. The testes appear as elongated bean shaped and situated asymmetrically. The mean average of the testis was 1,48 cm in length, 0.47cm in width, and 1.54gm in weight. Histologically, each testis was covered by a connective tissue capsule.The stroma of the testis posses no mediastinum. Leydic cells were commonly found in groups within the connective tissue which filled the spaces between the seminiferous tubules. Germ cells and Sertoli cells constituent the main component of the seminiferous epithelium. There were type A-spermatogonia, type I-spermatogonia, and type B-spermatogonia. Type B-spermatogonia passes through proleptoten, leptotene, zygotene, pachytene, diplotene, and diakineses. The primary spermatocytes give rise to secondary spermatocytes. These latter cells enter the second meiotic division to form the spermatids which undergo through thirteenth stages to form the sperms.

Abstract

In this study, thirty adult Swiss male mice weighing with an average weight of twenty seven gram were used. The animals were housed in animal house of the college of Veterinary Medicine/ Tikrit University under normal conditions. The whole animals were divided into three groups each of ten mice, and subdivided into two periods (fourteen and twenty eight days), first group considered as control which were administered with distilled water only. The second group was treated with the metronidazole (MTZ) at a dose of 0.37 mg / mouse /day, the third group were administered with the sumac solution of 0.04 ml / mouse / day. All groups were administered orally.Histological technique was performed to study the effect of MTZ and sumac on the tissues of testis in addition of sperm qualification. The study showed that, the second group of (the two periods) represented the absence of the spermatogonia with the presence of leydig cells in the interstitial tissue. The third group demonstrated a histopathological effect on the testicular tissue such as seminiferous tubules. The study of the effect of MTZ and sumac on the proportion of male sperm showed abnormalities, and the effect of the MTZ showed significant (P≤ 0.01) abnormalities in the confirmation sperms compared with the control group and sumac

في هذه الدراسة تم استخدام ثلاثون حيوان من (ذكور الفئران البيض السويسرية) بمعدل وزن سبعة وعشرون غم حيث تم ايوائها في البيت الحيواني التابع لكليه الطب البيطري جامعه تكريت تحت ظروف طبيعية، قسمت الى ثلاثة مجاميع كل مجموعة تضم عشر فئران وللفترتين (اربعة عشر وثمانية وعشرون يوم)، المجموعة الاولى مجموعة سيطرة وتم معاملتها بالماء المقطر فقط، المجموعة الثانية تم تجريعها بالجرعة العلاجية الـ metronidazole (MTZ) (0.37 ملغم/فأر/يوم) للفترتين، المجموعة الثالثة جرعت بالمحلول المائي للسماق بمقدار 0.04 مل/فأر/يوم (للفترتين). تم اعطاءالعقار والسماق للحيوانات عن طريق الفم. وتم اجراء دراسة تاثير العقار والسماق على التركيب النسيجي للخصية بالاضافة الىتقييم النطف الذكرية للمجاميع الثلاثة. اظهرت النتائج فيما يخص التركيب النسيجي للخصية في المعاملات الاولى والثانية ولفترتي التجربة تغيرات معنوية مقارنة مع مجموعة السيطرة حيث كان هناك وجود تأثير سلبي واضح في المجموعة الثانية، تمثلت بخلو مركز النبيبات المنوية من النطف مع وجود خلايا لايدك في النسيج الخلالي بين النبيبات المنوية باعداد محددة ومتراصة مع بعضها، في حين ان تأثير المجموعة الثالثة كان اقل حدة. وتناولت الدراسة ايضا تأثير عقار الـ MTZ والسماق على نسبة التشوهات النطف الذكرية، حيث كان لعقارMTZ تاثيرات عالية المعنوية ((P≤0.01) في ظهور تشوهات النطف مقارنة بمجموعة السماق والسيطرة، بينما لم يكن هناك تأثير معنوي بين مجموعة السيطرة والسماق في نسب تشوة النطف.

Abstract

In the current study demonstrated the role of irradiation with stimulate the cells ondivision and accelerate of spermatogenesis in mice testis following low –level lasertherapy and anti-inflammatory process.Twenty -eight healthy male and female mice .They were maintained in individualcages with free access to water and a pellet balanced commercial diet for mice . Allanimals have been weighing by using sensitive balance and the mean body of mice(25+_5 gm) .The stages of Q-S Switch Nd:YAG laser set-up with 632 nm 10 ns of pulseduration and 10 Hz of frequency to get a dose or energy density were : 1-set a singlepulse on Q-Switch Nd:YAG laser using control panel with repetition rate of 10 Hz .Mice were irradiated to testes a 670 nm with intensity 4 J/cm2 /day .for seven days.But mice of control group without exposure any laser

Abstract

Background and objectives: Epidermoid cyst of the testis is a very rare benign lesion that accounts for about 1% of all testicular tumors.Case report: 16-year old male with epidermoid cyst of the testis is described in which the diagnosis was unsuspected clinically and diagnosed histopathologically. The clinical, histogenesis, pathology and management are briefly reviewed.

Abstract

Undescended testis is the most common genital anomaly that affects infants and boys. It has serious complications if left untreated at the proper time. The aim of treatment is to replace the testis to its normal scrotal position where normal testicular development and spermatogenesis occurs. A total of 60 patients with undescended testes were enrolled in this prospective study that extended over three years. Ages with the mode of presentations, methods of identification (physical examination vs. ultrasound study), types of surgery and the results of histopathology were all reported. Only 10 patients (16.6%) presented within the recommended time of surgery (< 2 years). Regarding the mode of presentations: for 26/60 patients (43.3%) the pathology was detected by their parents, 6/60 (10%) were detected by the patients themselves and the remaining 28/60 (46.6%) presented with complications. Identification and localization of the undescended testes were made clinically in 55/60 patients, whereas only in 41/60 patients were the undescended testes detected by ultrasound examination. Overall, 48/60 patients (80%) were treated by orchiopexy and 9/60 patients were treated by orchiectomy. The histopathology of the excised testes revealed permanent testicular damage in all the specimens. In-Conclusions: There is a lack of enough public awareness about this problem and its complication in our society. Orchiopexy at the proper time effectively prevents testicular damage, whereas orchiectomy is reserved for neglected damaged testes to avoid serious complications.

Abstract

Background: Undescended testis is a well recognized clinical problem in infants and young male , as well as being a frequent finding in adult individuals .Aim of work : To compare the results of MRI and ultrasound in the localization of undescended testis , in order to choose the line of treatment .Method : The study was conducted in two general hospitals in Baghdad between Oct. 2012 --- April 2013. Forty male patients with undescended testis ( 6 months ---20 years ) underwent abdominal and pelvic MRI as well as inguinal and pelvic U/ S to identify testicular location , findings were reviewed by three observers independently and compared with surgical and / or clinical follow up study.Results : Ultrasound succeeded in localizing 21 cases of undescended testis with sensitivity 83.3 % specificity 100% and accuracy 90 % . On the other hand MRI was able to demonstrate 24 cases with sensitivity 96%, specificity 100% and accuracy 97.5 % .Conclusion: Magnetic Resonance Imaging is a useful imaging tool in the localization and tissue characterization of a undescended testis , especially with the use of i.v contrast injection which is helpful in suspected cases of testicular atrophy and cases of abdominal and pelvic testis , however, ultrasound must be the first line of imaging investigation as it is readily available and fast, but highly operator dependent .
