Comparative study between cryotherapy and salicylic acid in the treatment of plantar warts in Erbil - Iraq

Abstract

Background and objective: Plantar warts are hard, grainy growths that usually appear on the heels or balls of feet. Plantar warts are caused by the human papillomavirus, usually self-limiting, but treatment is generally recommended to lessen symptoms, decrease duration, and reduce transmission. The study aimed to evaluate the clinical effectiveness of topical 40% salicylic acid in comparison to cryotherapy in the treatment of plantar wart. Methods: This study was conducted from March 2015 to January 2016 on 70 patients with plantar wart who attended the consultation clinic of dermatology department in Rizgary teaching hospital. Patients were randomly allocated to two groups: cryotherapy treatment with liquid nitrogen every two or three weeks and self-application of salicylic acid daily.Results: Sixty patients aged 12 to 60 years were included in the analysis at 13 weeks. Cure rates were 66.7% in the cryotherapy group and 80% in the salicylic acid group. A significant association was found between the duration of wart before treatment and cure rates (P <0.001).Conclusion: No difference in effectiveness between cryotherapy and topical salicylic acid 40% in the treatment of plantar warts was found.

Treatment Of Verruca Vulgaris with Long Pulsed Nd:YAG Laser 1064 nm In Iraqi Patients

Abstract

Background: Verruca vulgaris is a common dermatological problem. The conventional treatments for verruca vulgaris are usually invasive, low efficacy, and need a long recovery period. Aims of study: evaluate the effectiveness and the safety of long pulsed Nd:YAG laser in the treatment of verruca vulgaris. Methodology: In this therapeutic clinical trial, thirty five patients with a total of 105 lesions were enrolled. Each patient was eligible for up to three treatment sessions at 2 weeks interval with long pulsed Nd:YAG laser. All verrucae were evaluated before every session and three months after the last treatment session. The response classifies as complete (when there is complete disappearance of lesions), no response when there was no change, and partial (when the reduction in verruca size 50% or greater). Results: complete response was seen in 96 (91.43%), partial response in 4 (3.81%), and no response in 5 (4.76%) verruca. The cumulative clearance rate after first, second, third treatment sessions was 60%, 80%, and 91.43 % respectively. At end of follow up, relapse rate was 2.08%. Side effects were generally mild and did not prevent normal activity. Conclusion: long pulsed Nd:YAG laser (1064nm) appears to be effective and safe for treatment of verruca vulgaris.Recommendations: 1. Increase the sample size. 2. خلفية البحث: داء الثآليل الشائع هو من مشاكل الأمراض الجلدية الشائعة. عادةً ما تكون العلاجات التقليدية لداء الثآليل غزويه وذات فعالية منخفضة وتحتاج إلى فترة نقاهة طويلة.الهدف: تقييم فعالية وسلامة ليزر النادميوم / النابض الطويل ذو الطول الموجي 1064 نانومتر في علاج داء الثآليل الشائع.المنهجية: في هذه التجربة السريرية العلاجية ، تم تسجيل خمسة وثلاثين مريضا مع ما مجموعه 105 آفات. كان كل مريض مؤهلا لمدة تصل إلى ثلاث جلسات علاج في فاصل 2 أسابيع مع ليزر نادميوم / نابض طويل. تم تقييم جميع الثآليل قبل كل جلسة وثلاثة أشهر بعد جلسة العلاج الأخيرة. تصنف الاستجابة على أنها كاملة (عندما يكون هناك اختفاء تام للآفات) ، لا استجابة عندما لا يكون هناك تغيير ، وجزئي (عندما يكون الانخفاض في حجم الثآليل 50٪ أو أكبر).النتائج: شوهد استجابة كاملة في 96 (91.43 ٪)، استجابة جزئية في 4 (3.81 ٪)، وعدم استجابة في 5 (4.76 ٪) من الثآليل. وكان معدل التخليص التراكمي بعد جلسات العلاج الأولى والثانية و الثالثة 60 ٪، 80 ٪، و 91.43 ٪ على التوالي, في نهاية المتابعة، كان معدل الانتكاس 2.08 ٪. كانت الآثار الجانبية خفيفة بشكل عام ولم تمنع النشاط الطبيعي.الاستنتاج: يبدو أن ليزر النادميوم / النابض الطويل ذو الطول الموجي 1064 نانومتر فعال وآمن لعلاج داء الثآليل الشائع.التوصيات: توصي الدراسة بما يلي :1. ادخال عدد اكبر من المرضى .2.عمل دراسة مقارنة بين هذا النوع من الليزر واستعمال علاج تقليدي لداء الثآليل.

Abstract

Background: Genital wart is the most common viral sexually transmitted disease. In fact, its incidence is rising rapidly. It is approximatedthat 30%–50% of sexually active adult have human papillomavirus (HPV) infection, but only 1%–2% of them have apparent genital wart.Mostly, cervical dysplasia and cancer are associated with oncogenic HPV types. HPV types are classified into high‑, intermediate‑, and low‑riskgroup. HPV Types 16 and 18 are considered as a high‑risk group cancer‑associated HPVs, whereas Types 6 and 11 concerned as a low riskand HPV types 31, 33, and 35 have an intermediate association with cancer. Objective: The objective of the study was designed to evaluatethe activity of CO2 laser to treat genital warts of female Iraqi patients. Materials and Methods: Fifteen female patients with genital wartswere involved in this study. Their age range from 22 to 49 years treated by CO2 laser at the Laser Medicine Research Clinics of Institute ofLaser for Postgraduate Studies from July 2017 to end of July 2018. Application of topical lidocaine cream 10% for 30 min or local infiltrationof 2% xylocaine had been performed. CO2 laser in chopped mode was used with a peak power: 151 watts and repeated time 35 s in D mode,pulse duration 1.3 ms. Treatments by vaporization of the warts were done in one session except one patient with a large number and multiplelocations two sessions were done. Results: Three patients expressed mild pain during the procedure, but it could be tolerated by them. Noedema or oozing observed during the 1st week. No postoperative infections or scars. Two cases of recurrence 2/15, (13.3%) after few monthsfrom treatment. Conclusion: CO2 laser vaporization is an effective and safe method for treatment of genital warts and can be done in theoutpatient clinic

Abstract

forty sample was collected from marjan hospital ,The examination was done by using light microscope according to dermatologist manifestation depending on the histological and pathological features, The Human papilloma viruses type 6,11 were diagnosed by using in situ hybridization( ISH) technology. the results show that:. Heights percent 60% of patients with HPV type 6,11 were occur at age group less than 21 years, (30%) of patient who had common wart, meanwhile, ( 40% ) of patient had genital wart on their genital area .

تم فحص 40 مقطع نسيجي لعينات مصابه بالثاليل جمعت من مستشفى مرجان التعليمي /بابل. تم تشخيص الثاليل من قبل الطبيب المختص وبالاعتماد على الخصائص المرضيه في النسيج وتم فحص الفيروس الحليمي البشري Human papilloma virus(HPV 6 &11) باستخدام تقنيه التهجين الموضعي ذات الحساسيه العاليه لدنا الفيروسي in situ hybridization( ISH) حيث كانت نسبه الاصابه بالفيروس الحليمي البشري نمط 6 و 11 60% للاشخاص المصابين بالثاليل الجلديه وبعمر اقل من 21 سنه وبنسبه (40 %)من المصابين بالثاليل كانت للنوع التناسلي Genital wart ,نسبه (% 30 ( يسببها النوع الشائع common wart.

Abstract

The aim of this study was to immune estimation of cytokine profile including serum concentration of each (IL-6, IL-10, IFN-γ andTNF-α) in genital wart patientsin Babylon Province-Iraq.This work was applied on (40) genital wart patients who admitted to the outpatient department of Dermatology and Venereology in Merjan Medical City, Hilla-Iraq and private clinicsduring the period from November (2013) to March (2014).A total of 40 patients (16 males and 24 females) were investigated with their age ranged from 2-48 years, in addition to 20 apparently healthy males and females be used as control with an age range approximately matched to genital wart patients.Blood samples were collected from both patients and control to estimate the serum levels of the immune parameters Interleukine-6 (IL-6), Interlukine-10 (IL-10), Interferon gamma (IFN-γ) and tumor necrosis factor alpha (TNF-α) by ELISA (Enzyme linked Immunosorbent Assay) method.The cytokine profile showed that there is a significant decrease (P< 0.05) in the mean serum concentration of each (IL-6, IFN-γ and TNF-α) between study groups, indicated a decreasing in Th1 response which is responsible for cell-mediated immunity, while there was no significant difference (P >0.05) in the mean serum concentration between genital wart patients and control groups for IL-10.
